Chen Zishan said: Dysentery is divided into two types: bloody dysentery and white dysentery. People generally say that red indicates Yang and heat, while white indicates Yin and cold; or they claim that without accumulation (food stagnation), dysentery cannot form; or they interchangeably use methods for treating cold dysentery and hot dysentery, which will certainly fail to achieve efficacy. If one does not investigate the root cause, how can treatment be effective? Moreover, when the eight winds of the four seasons invade the human body, and the Five Movements and Six Qi interact in mutual overcoming, people often suffer from dysentery in summer and autumn. The *Neijing* states: "If wind is injured in spring, diarrhea with undigested food occurs in summer." How can one rigidly adhere to the saying that "without accumulation, there is no dysentery"? Is it true that throughout the year, only in summer and autumn do people have accumulations? Generally, when wind pathogen enters the stomach, Wood overcomes Earth. If it does not result in sudden violent diarrhea, it may become dysentery. When red and white are mixed, this indicates a lack of distinction between Yin and Yang. The treatment method should primarily focus on clarifying Yin and Yang. It is also stated: In cases of dysentery complicated by heat, red stool is discharged because wind-heat agitates the blood. Treatment method: Push San Guan (Three Gates), Tui Liu Fu (Push the Six Limbs/Qi Bone), Qing Xin Jing (Clear the Heart Meridian), He Yin Yang (Harmonize Yin and Yang), Tui Da Chang (Push the Large Intestine), Bu Pi Tu (Tonify Spleen Earth), Yun Ba Gua (Rotate the Eight Trigrams), Bu Shen Shui (Tonify Kidney Water), Rou Qi (Knead the Navel) and Gui Wei (Tortoise Tail).
Chen Zishan said: In cases of dysentery complicated by cold, pure white stool is discharged. There may be pinkish oil on the surface, or it may resemble coagulated blood like pig liver; these are all Yin syndromes. This is because blood congeals due to cold. Treatment method: Push San Guan (Three Gates), Yun Ba Gua (Rotate the Eight Trigrams), Bu Pi Tu (Tonify Spleen Earth), Tui Da Chang (Push the Large Intestine), He Yin Yang (Harmonize Yin and Yang), Tui Tian Men Ru Hu Kou (Push Heavenly Gate into Tiger's Mouth), Rou Qi (Knead the Navel) and Gui Wei (Tortoise Tail).
Chen Zishan said: If summer heat is injured in summer, malaria will inevitably occur in autumn. This is because the pores open and sweat is released, leading to exposure to wind. Or, water vapor from bathing lingers in the skin. Because Wei Qi (Defensive Qi) fails to guard, pathogenic factors gather, and the disease begins. Symptoms include yawning, chills, pain in the waist and back, and painful joints. After the cold subsides, both internal and external heat appear, accompanied by headache and thirst. This results from the struggle between Yin and Yang Qi, where deficiency and excess alternate. If Yin Qi alone prevails, Yang becomes deficient, hence chills precede. If Yang Qi alone prevails, Yin becomes deficient, hence fever precedes. When Yin prevails and Yang is deficient, both internal and external are cold; when Yang is excessive and Yin is deficient, both internal and external are hot. If both Yin and Yang decline, and Wei Qi separates from the pathogenic Qi, the illness heals. If Yin and Yang contend, and Wei Qi gathers with the pathogenic Qi, the illness recurs. This depends on where Wei Qi is located and how it combines with the invading pathogen.
Tu Weisheng said: Chen Zishan's discussion on malaria in this section seems to have unique insights. However, his explanation of the mechanism behind the cold and heat fluctuations in malaria lacks sufficient detail and precision. One will understand this naturally by referring to the explanation of "wind malaria prevalent in autumn" in Volume 1.
Treatment Method 1: (For malaria accompanied by vomiting) Push San Guan (Three Gates), Fen Yin Yang of Pi Tu (Spleen Earth), Yun Ba Gua (Rotate the Eight Trigrams), Rou Qi (Knead the Navel).
Treatment Method 2: (For sudden onset of malaria) Push San Guan (Three Gates), Qing Fei Jing (Clear Lung Meridian), Fen Yin Yang (Divide Yin and Yang), Yun Ba Gua (Rotate the Eight Trigrams), An Xian Cuo Mo (Press Strings and Rub).
Treatment Method 3: (For chronic malaria not healing, with weak Spleen) Bu Pi Tu (Tonify Spleen Earth), Fen Yin Yang (Divide Yin and Yang), Yun Ba Gua (Rotate the Eight Trigrams).
Treatment Method 4: (For evil malaria occurring in the evening) Push San Guan (Three Gates), Bu Pi Tu (Tonify Spleen Earth), Fen Yin Yang (Divide Yin and Yang), Yun Ba Gua (Rotate the Eight Trigrams), Tui Liu Fu (Push the Six Limbs/Qi Bone), Tui Tian Men Ru Hu Kou (Push Heavenly Gate into Tiger's Mouth).
Treatment Method 5: (For Dan Malaria with heat but no cold) Push San Guan (Three Gates), Bu Pi Tu (Tonify Spleen Earth), Fen Yin Yang (Divide Yin and Yang), Yun Ba Gua (Rotate the Eight Trigrams), Qing Fei Jing (Clear Lung Meridian), Tui Liu Fu (Push the Six Limbs/Qi Bone), Tui Nei Guan (Push Inner Pass), Tui Tian Men Ru Hu Kou (Push Heavenly Gate into Tiger's Mouth), Rou Dou Zhou (Knead Dou Zhou), Jian Shi (Jianshi Point).
