Chen Zishan said: Various types of heat syndromes have different causes at their onset. Heat that occurs at regular intervals is called tidal fever Heat that comes and goes suddenly belongs to deficiency syndromes. Heat that occurs without chills is classified as bone-steaming consumption
He further stated: The so-called fetal heat refers to a condition in infants between three days and ten days after birth, characterized by eyes closed and red, swollen eyelids, frequent groaning, incessant crying, occasional fright and irritability, generalized high fever, and yellow urine. This is caused by the mother contracting seasonal heat-toxins during pregnancy, or mistakenly taking warm-tonifying medicines, or overeating pungent spices (five pungents), leading to internal heat accumulation that steams and affects the fetal qi. Upon birth, the infant presents with these symptoms, named fetal heat. If left untreated for a long time, it may develop into conditions such as thrush (鹅口疮), double tongue (重舌), wooden tongue (木舌), or red-purple danliu (赤紫丹瘤). At this stage, one must not use harsh cold-natured drugs to attack the heat, as retreating heat may invite cold pathogens, leading to other complications. Caution is essential.
Treatment method: Push San Guan (Three Gates), Retreat Liu Fu (Six Bowels), Divide Yin and Yang, Clear Tian He Shui (River of Heaven Water), Clear San Jiao (Triple Burner), Rub Wai Lao Gong (Outer Labor Palace), Move Ba Gua (Eight Trigrams) (from Kan to Kun position, preferably twice more), Pinch Shen Shui (Kidney Water), Push Wu Zong Jing (Five General Meridians), Pinch Shi Wang Xue (Ten Kings Acupoint), Move Dou Zhou (Dou Elbow), Water Bottom Retrieve Bright Moon Apply fire-moxibustion once each on the Tiger's Mouth (Hukou) and Qu Chi (Quchi, LI11).
Tu Weisheng noted: The origin of Shi Wang Xue (Ten Kings Acupoint) is lost; it is suspected to be Zhong Chong (Zhongchong, PC9) at the tip of the middle finger. Uncertainty remains, so it is preserved here for future verification. Fire-moxibustion should also be used judiciously.
Chen Zishan said: Tidal fever involves alternating periods of fever and defervescence, occurring daily at specific times, like the tide without error. Generally, this is due to excessive blood and qi stagnation, wind-heat in the five zang organs steaming outward, possibly combined with latent heat or retained cold. Latent heat manifests as yellow, foul-smelling stool; retained food manifests as white, sour-smelling stool.
Treatment method: Push San Guan (Three Gates), Tonify Xin Jing (Heart Meridian), Move Ba Gua (Eight Trigrams), Divide Yin and Yang, Pinch Shi Wang Xue (Ten Kings Acupoint), Pinch Middle Finger, Retreat Liu Fu (Six Bowels), Water Bottom Retrieve Bright Moon (水底捞明月), Move Dou Zhou (Dou Elbow).
Chen Zishan said: Fright-heat occurs when one is startled by strange objects or frightened by loud metallic sounds. Consequently, the heart is affected by fright, heart qi becomes obstructed, slight fever appears, dreams are filled with fright, the face shines with sweat, pulse is rapid, and there is irritability. Treatment should follow the same method as for acute wind syndrome.
Treatment method: Push San Guan (Three Gates), Clear Fei Jing (Lung Meridian), Divide Yin and Yang, Push Er Men Shan (Two Gate Mountains), Clear Xin Jing (Heart Meridian), Clear Wu Jing Wen (Five Palm Lines), Move Dou Zhou (Dou Elbow), Water Bottom Retrieve Bright Moon (水底捞明月), Fly Meridians and Move Qi (飞经走气).
Chen Zishan said: Wind-heat presents with body fever, greenish complexion, burning sensation in the mouth, and intermittent irritability and shouting. It should be treated by dispersing wind and clearing heat. If heat is severe accompanied by hot stools, purgation may be appropriate.
Treatment method: Push San Guan (Three Gates), Purge Da Chang (Large Intestine), Pinch Xin Jing (Heart Meridian), Tonify Shen Shui (Kidney Water), Move Ba Gua (Eight Trigrams), Pinch Zong Jing (General Meridian), Clear Tian He Shui (River of Heaven Water), Two Dragons Playing with a Pearl (二龙戏珠), Move Dou Zhou (Dou Elbow).
Chen Zishan said: Irritable heat is characterized by excess of both qi and blood, excess-heat in the zang-fu organs, heat in both interior and exterior, restlessness, and intense skin heat. If the palms and soles are extremely hot, it is termed "five-center heat" (五心烦热).
Treatment method: Push San Guan (Three Gates), Pinch Middle Finger, Pinch Shi Wang Xue (Ten Kings Acupoint), Purge Wu Jing Wen (Five Palm Lines), Move Ba Gua (Eight Trigrams), Rub Wai Lao Gong (Outer Labor Palace), Divide Yin and Yang, Retreat Liu Fu (Six Bowels), Water Bottom Retrieve Bright Moon (水底捞明月), Horse Strikes Across River of Heaven (打马过天河), Move Dou Zhou (Dou Elbow).
Tu Weisheng noted: Most cases of irritable heat belong to yin-blood deficiency and cannot be generally diagnosed as qi-blood excess or zang-fu excess-heat. Since blood is scant, fire lacks nourishment. However, such cases are less common in children.
Chen Zishan said: Slight retraction of the tongue tendons and frequent tongue protrusion are due to accumulated heat in the spleen. Cold-natured medicines should not be used recklessly.
Treatment method: Push San Guan (Three Gates), Retreat Xia Liu Fu (Lower Six Bowels), Tonify Pi Tu (Spleen Earth), Purge Xin Huo (Heart Fire), Tonify Shen Shui (Kidney Water), Move Ba Gua (Eight Trigrams), Divide Yin and Yang, Pinch Zong Jin (General Tendon), Water Bottom Retrieve Bright Moon (水底捞明月), Move Dou Zhou (Dou Elbow).
Chen Zishan said: Deficiency heat occurs when qi and blood are insufficient after illness, limbs are emaciated, and fever occurs frequently, three to five times a day. This is exogenous heat taking advantage of deficiency. Regulating qi and harmonizing blood will naturally resolve the heat.
Treatment method: Push Shang San Guan (Upper Three Gates), Retreat Xia Liu Fu (Lower Six Bowels), Twist Five Fingers, Move Ba Gua (Eight Trigrams), Water Bottom Retrieve Bright Moon (水底捞明月), Pinch Zong Jing (General Meridian), Divide Yin and Yang, Fly Meridians and Move Qi (飞经走气), Move Dou Zhou (Dou Elbow).
Chen Zishan said: Dizziness, red cheeks, oral heat, dark and scanty urination, constipation, and abdominal distension indicate excess-heat syndrome. Purging is indicated. Once the heat in the zang-fu organs is eliminated, the disease will heal itself.
Treatment method: Push Shang San Guan (Upper Three Gates), Purge Da Chang (Large Intestine), Clear Shen Shui (Kidney Water), Move Ba Gua (Eight Trigrams), Push Pang Guang (Bladder), Divide Yin and Yang, Water Bottom Retrieve Bright Moon (水底捞明月), Retreat Liu Fu (Six Bowels), Horse Strikes Across River of Heaven (打马过天河), Fly Meridians and Move Qi (飞经走气), Move Dou Zhou (Dou Elbow).
Chen Zishan said: Accumulated heat presents with swollen eyelids, sallow complexion, cold feet, fever intensifying from head to abdomen, aversion to food odors, vomiting, nausea, and abdominal pain.
Treatment method: Push Shang San Guan (Upper Three Gates), Retreat Xia Liu Fu (Lower Six Bowels), Clear Wu Jing Wen (Five Palm Lines), Tonify Pi Tu (Spleen Earth), Push Da Chang (Large Intestine), Clear San Jiao (Triple Burner), Tonify Shen Shui (Kidney Water), Move Ba Gua (Eight Trigrams), Pinch Zong Jing (General Meridian), Divide Yin and Yang, Water Bottom Retrieve Bright Moon (水底捞明月), Fly Meridians and Move Qi (飞经走气), Move Dou Zhou (Dou Elbow).
Chen Zishan said: Gan heat (wasting-thirst heat) results from overeating, causing stagnation in the middle jiao which turns to heat over time. If spleen stagnation is not treated, it transmits to other zang organs, forming the five types of Gan diseases. If the spleen pathology resolves, the other organs remain peaceful.
Treatment method: Push Shang San Guan (Upper Three Gates), Retreat Xia Liu Fu (Lower Six Bowels), Tonify Pi Tu (Spleen Earth), Push Da Xiao Chang (Small and Large Intestines), San Jiao (Triple Burner), Move Ba Gua (Eight Trigrams), Pinch Zong Jin (General Tendon), Divide Yin and Yang, Water Bottom Retrieve Bright Moon (水底捞明月), Fly Meridians and Move Qi (飞经走气), Move Dou Zhou (Dou Elbow).
Chen Zishan said: Blood heat occurs every day during Chen and Si hours (7:00–11:00 AM) and cools down at night. People often mistake it for Gan heat, but it is actually blood heat.
Treatment method: Push Shang San Guan (Upper Three Gates), Push Xia Liu Fu (Lower Six Bowels), Divide Yin and Yang, Move Ba Gua (Eight Trigrams), Clear Wu Jing Wen (Five Palm Lines), Pinch Zong Jin (General Tendon), Clear Shen Shui (Kidney Water), Water Bottom Retrieve Bright Moon (水底捞明月), Rub Dou Zhou (Dou Elbow), Press String and Rub (按弦搓摩), Fly Meridians and Move Qi (飞经走气).
Tu Weisheng noted: Blood heat differs from deficiency heat. Blood heat involves robust fire in the blood level, which consumes yin fluids. Deficiency heat arises from prior yin-blood deficiency, leaving fire unanchored. Thus, one originates in the morning, the other in the afternoon. Regarding strong heat (Zhuang Re), it mostly originates from the qi level. Although heat is close to fire, fire is the essence of heat, and heat is the evaporation of fire.
Chen Zishan said: Bone-steaming heat is heat steaming from the bones. There is heat but no chills, and night sweats cease only upon waking. It is not superficial burning. It is caused by infants eating meat too early, habitual consumption of roasted/baked foods, preference for fruits like peaches, plums, waxberries, or melons, wearing thick cotton clothes in winter consuming body fluids, residual toxins from Gan disease transmitting to bone-steaming, or abdominal masses causing intermittent pain.
Treatment method: Push San Guan (Three Gates), Push Liu Fu (Six Bowels), Move Wu Jing Wen (Five Palm Lines), Divide Yin and Yang, Clear Tian He Shui (River of Heaven Water), Water Bottom Retrieve Bright Moon (水底捞明月), Tonify Shen Shui (Kidney Water), Pinch Zong Jing Si Heng Wen (General Meridian Four Transverse Lines), Horse Strikes Across River of Heaven (打马过天河).
Tu Weisheng noted: Bone-steaming heat represents extreme yin deficiency. When internal yin fluids are exhausted, water-yin cannot generate marrow, so heat reaches the bones. Although Zishan's explanation is not entirely accurate, food-stagnation heat indeed consumes yin-blood.
Chen Zishan said: Strong heat (Zhuang Re) persists continuously. It is caused by qi-blood obstruction, heat generation in the five zang organs steaming internally, resulting in intense body heat, restless sleep, and mental confusion. Steaming outward causes heat in both interior and exterior, potentially generating wind in severe cases.
Treatment method: Push San Guan (Three Gates), Retreat Liu Fu (Six Bowels), Clear Fei Jing (Lung Meridian), Divide Yin and Yang, Push Er Men Shan (Two Gate Mountains), Clear Xin Jing (Heart Meridian), Clear Tian He Shui (River of Heaven Water), Clear Wu Jing Wen (Five Palm Lines), Pinch Zong Jin (General Tendon), Move Dou Zhou (Dou Elbow), Water Bottom Retrieve Bright Moon (水底捞明月), Fly Meridians and Move Qi (飞经走气).
Chen Zishan said: Warm-strong heat is similar to strong heat but milder. "Warm" implies heat that is not severe. If stomach qi is disharmonious, qi stagnates and accumulates, causing internal heat, it is named warm-strong heat. Yellow, foul stool indicates latent heat in the abdomen. White, sour stool indicates retained food stagnation, requiring mild purgation.
Treatment method: Push San Guan (Three Gates), Push Liu Fu (Six Bowels), Clear Fei Jing (Lung Meridian), Divide Yin and Yang, Push Er Men Shan (Two Gate Mountains), Clear Xin Jing (Heart Meridian), Clear Tian He Shui (River of Heaven Water), Clear Wu Jing Wen (Five Palm Lines), Pinch Zong Jin (General Tendon), Move Dou Zhou (Dou Elbow), Water Bottom Retrieve Bright Moon (水底捞明月), Fly Meridians and Move Qi (飞经走气).
Chen Zishan said: Bian Zheng heat (Developmental heat) is the steaming of yin-yang and water-fire within qi and blood, facilitating physical maturation. "Bian" (Change) refers to the formation of the five zang organs; "Zheng" (Steam) refers to the nourishment of the six fu organs. At 32 days after birth is one "Bian"; at 64 days is one "Zheng". After ten Bians and five Zhengs, totaling 320 days, development completes, and the infant becomes an adult. Infant developmental heat is like silkworms molting, dragons shedding bones, or tigers changing claws—all due to biological transformation and growth. Symptoms include steaming fever, upper qi rebellion, fright, cold ears, slight sweating, white bubbles on the upper lip like pearls, or swelling like a sleeping silkworm. Severe cases involve fever, mental confusion, abdominal pain, crying, and inability to nurse. Do not administer drugs or use tuina/fire-moxibustion recklessly. Mistreatment can be fatal. Therefore, no treatment method is prescribed; this is recorded for awareness.
Tu Weisheng noted: The theory of infant developmental heat (Bian Zheng) is unknown whether fabricated by ancient sages or based on evidence. If fabricated, the described days and shapes seem credible. If based on evidence, the descriptions vary significantly among texts, lacking consistency. I offer my perspective: In human life, zang-fu organs are the root, and limbs/branches are the leaves. Zang-fu must be complete before limbs form, and limbs before senses arise. It is unheard of for senses and limbs to exist before zang-fu develop. If limbs require completion and zang-fu develop through Bian Zheng, what digests milk before 32 days? What generates blood? What stores blood and governs qi? If internal structures were incomplete, survival would be brief, let alone rapid growth. Consider a clock: internal mechanisms must be complete to function accurately. Damage causes inaccuracy. How can a living infant grow rapidly with incomplete internals? I preserve this theory for reference due to its prevalence but clarify discrepancies to correct errors.
