[General Discussion] Among surgical diseases, the most important are carbuncles (Yong) and sores (Ju). The most urgent are throat wind (Houfeng) and hard sores (Dingzhong). Those afflicted face life or death immediately. "Yong" means obstruction. Large and elevated lesions belong to Yang syndrome. Lesions that appear red, swollen, and extensive are Yong. (Painful and solid indicates Heat; painful and itchy indicates Cold.) Lesions occurring in the Six Fu Organs are easy to treat but difficult to completely heal. (Superficial and large is Yong; deep and malignant is Ju.) "Ju" means blockage. Flat and inward-developing lesions belong to Yin syndrome. Their onset is fierce and malignant. Initially, they resemble millet grains, white and scorched in appearance. Touching them causes pain radiating to the heart; these are Ju. They must not be neglected. Although difficult to treat, they can recover easily. If there is no pain at all, it is a more malignant condition. The sites of occurrence are most忌讳 (taboo/avoided) for Fa Bei (Back Carbuncle), Fa Nao (Head Carbuncle), Fa Bin (Temple Carbuncle), Fa Mei (Eyebrow Carbuncle), and Fa Yi (Cheek Carbuncle). At the initial stage, if one feels high fever with aversion to cold, stiffness and headache, mental restlessness, irritability, and desire for cold drinks, the sore will be severe. If daily activities are peaceful, diet is normal, and bowel/bladder functions are regular, it is easy to treat. Diameter above one cun is called Yong-Ju; below one cun is called Chuang-Jie. (Fever on the skin surface, swelling with a small root, even if large, is still Chuang-Jie.) Generally, Yong belongs to Heat and should be treated with cooling medicines (do not use after ulceration). Ju belongs to Cold and should be treated with warming and tonifying medicines.
[Back Carbuncle] At the initial stage, regardless of the date, whether Yin or Yang with swelling and pain, immediately use single-clove garlic. (Slice to a thickness of two or three qian.) Place on the sore head, and moxa with large moxa cones. (Replace garlic slice every three moxa burns, regardless of dozens or hundreds of burns.) For severe pain, moxa until pain stops; for numbness without pain, moxa until pain appears, then stop. Early detection and early moxibustion are best; after seven days, it is ineffective. If the back carbuncle is red, hot, swollen, and painful, making the head indistinguishable, simply cover the back with wet paper and observe immediately. The place where the paper dries first is the sore head. If ten or more heads grow together, crush garlic into a paste, make a cake, place it on the heads, and burn gathered moxa on the garlic cake. This can also save lives. Moxa fire can disperse toxic qi through heat. (All initial toxins can be moxa-ed, except those on the top of the head which belong to the Yangming channel and absolutely must not be moxa-ed.) If treatment is missed initially and the sore becomes established, one must examine deficiency/excess and cold/heat. Excess-Heat requires clearing; Deficiency-Cold requires warming. Only when the toxin disperses and pus erupts is it treatable. (If extremely itchy, prick with a needle, squeeze out bad blood; after several times when itching stops, it is easy to treat.)
[Tuoli Huxin Pill] After moxibustion, take Bait Fan (Alum) frequently. (1 liang 2 qian) Huang La (Yellow Beeswax) (1 liang) Xiong Huang (Realgar) (1 liang 2 qian) Zhusha (Cinnabar) (6 qian water-ground) Hupo (Amber) (1 qian). All ground into fine powder. First melt the yellow beeswax, mix in the medicine powder evenly. Must be rolled by multiple hands into pills the size of tung tree seeds. Take 30 pills each time, washed down with plain boiled water, three times a day. This can prevent symptoms such as mouth and tongue ulcers turning black and rotting. For all Yong-Ju toxic diseases, taking several doses is beneficial. For established cases, it relieves pain best; for unbroken cases, it eliminates internally. Suitable for all regardless of Yin/Yang, age, or sex. (Original formula uses 1 liang Alum powder, 7 qian yellow beeswax, melt wax and roll by multiple hands. Take 30 pills each time; taking more yields miraculous effects.)
[All Carbuncles, Back Carbuncles, and Opposite Sores] Treats all regardless of Yin/Yang flat/swollen, and nameless swellings/toxins. Large toad. (One; for severe toxicity, two or three. Best if grown under eaves; in winter, under loose stones or in riverbank crevices.) Add 2-3 qian Ming Fan (Alum), crush together with intestines and stomach until smooth. Apply thickly around the perimeter, leaving a hole in the center. For heavy toxicity, it will smell foul after 1-2 hours; replace with freshly crushed mixture. Continue until improvement. For severe cases with coma and loss of pain sensation, change over ten times; no case will fail. This is a stable prescription for drawing out and dispersing toxins.
[Back Carbuncle Swelling, Hardness, Deep Pain] Huai Mi (Sophora Flower Bud). (5 liang, fresh is better) Stir-fry until slightly yellow/brown. While hot, add 2 bowls of wine, boil for over ten boils. Strain and drink hot. Sweating leads to cure. Unsuppurated cases resolve in 2-3 doses; suppurated cases reduce in 3-4 doses. Crush residue and apply to affected area. Do not take continuously for those with Stomach Cold. Very effective. For swelling, hardness, and solid pulse indicating pathogen inside, use Da Huang (Rhubarb Root and Rhizome). (Amount不限, half fire-roasted, half raw) Sheng Gan Cao Jie (Raw Licorice Joint) equal parts ground to powder. Take 1-2 qian each time, mixed with warm wine on an empty stomach, 2-3 times, until purgation occurs.
[Yin Ju with Flat External Appearance, No Elevation, Dark Color] Ai Ye (Mugwort Leaf). (1 jin) Liu Huang (Sulfur), Xiong Huang (Realgar). (Each 5 qian) Boil together in water for half a day. Crush until smooth, wait for warmth, and apply. Re-boil if cool. Change over ten times. Non-painful areas become painful; if pain is deep in flesh, swelling and pain moving outward indicates regeneration. Zhu Dan Zhi (Pig Gallbladder Juice). (Stir-fried...) [Note: Text fragmentary].
[Yong-Ju Half-Yin Half-Yang] Swelling not very high, persisting for days without resolving. Bai Zhi (Angelica Dahurica), Shi Chang Pu (Acorus Tatarinowii), Chi Shao Yao (Red Peony Root). (Stir-fry each 2 liang) Zi Jing Pi (Bark of Wisteria Sinensis). (Stir-fry 5 liang) Du Huo (Angelica Pubescens). (Remove joints, stir-fry 1 liang) All ground to powder. Decoct Cong Tou (Green Onion Head) in strong soup, mix with powder, apply around perimeter. Swelling rises and becomes easy to treat.
[All Carbuncles and Back Carbuncles] Arises from internal stagnation of Seven Emotions, accumulating Heat internally, pressing with hot qi. Extremely effective. Also especially effective for Ru Yong (Breast Carbuncle) and Ru Jie (Breast Abscess). Yuan Zhi (Polygala Root). (Soak in rice wash water, remove heart) Dry by roasting, grind to powder. Take 3 qian each time, with one cup of wine, boil for several boils, filter clear liquid and drink. Apply residue to affected area, continue until healed.
[Chan Su Pill] Treats all carbuncles, back carbuncles, ding toxins, and malignant conditions. Taking this medicine makes non-painful areas painful, stops existing pain, awakens confused patients, resolves vomiting, eliminates unformed toxins, and ruptures formed toxins. It truly has the power to revive the dead and is the supreme treasure for malignant conditions. Should be prepared and administered. Chan Su (Toad Venom). (Freshly taken, sun-dried 2 qian, dissolve in burning wine when using) Qing Fen (Calomel) (5 fen) Ku Fan (Burnt Alum), Tong Lv (Chlorocrysocolla), Dan Fan (Chalcanthite), Han Shui Shi (Halloysitum). Ru Xiang (Frankincense). (Roast on bamboo leaf to remove oil) Mo Yao (Myrrh). (Roast on bamboo leaf to remove oil) She Xiang (Musk). (Each 1 qian) Wo Niu (Garden Snail). (21 pieces) Zhusha (Cinnabar) (3 qian). All medicines ground to fine powder. Follow the method of combining Taiyi Dan. In a quiet room on Dragon Boat Festival or Qixi Festival, first crush snails, then mix with Chan Su, finally add other medicines, pound extremely evenly. Roll into pills the size of mung beans. Each dose (mild cases 3 pills, severe cases 5 pills). Use 5 cun of scallion white, chew by patient, spit into left hand of male or right hand of female, wrap... [Text cuts off].
[Xian Fang Huo Ming Yin] Treats all carbuncles, nameless swellings/toxins. Eliminates internally if pus not formed; ruptures immediately if pus formed. This is a divine formula for pain relief and detoxification. Suitable for all regardless of deficiency/excess, age. Chuan Shan Jia (Pangolin Scale). (Remove oil) Mo Yao (Myrrh). (Remove oil, each 1 qian, increase for severe cases) Jin Yin Hua (Honeysuckle Flower), Chen Pi (Tangerine Peel). (Each 3 qian) For lesions on the back Shu points, use Zao Jiao Ci as the sovereign herb. For chest lesions, add Gua Lou Ren (Trichosanthes Seed) 3 qian. For limbs, use Jin Yin Hua as sovereign. For Ding toxins, add Cao He Che Gen (Paris Polyphylla Root) 3 qian. Decoct herbs in half water and half wine until concentrated. Grind Ru Xiang and Mo Yao finely, mix with decoction and take. Two doses per day. Lie on side for a moment for effect. For Yin toxins that are flat and do not rise, remove Tian Hua Fen and Jin Yin Hua. For severe Heat with solid pulse and constipation, add purgative herbs.
[Qi Li San] Treats all nameless malignant toxins unresponsive to other medicines. Chi Lian She (Red-banded Snake). (Ashed, retain nature) Grind to fine powder, bind with rice paste into pills the size of mustard seeds. Take 7 pills each time, increase to 14 for severe cases. Wash down with good wine. Four to five doses eliminate completely. This formula is very effective. Contraindicated in pregnancy.
[Da Gui Tang] Treats all carbuncles, large swellings, and toxins. Eliminates immediately if newly formed; relieves pain and promotes healing if ulcerated. Effective immediately upon ingestion. Da Quan Dang Gui (Large Chinese Angelica Root). (1 liang sliced) Sheng Huang Qi (Raw Astragalus Root), Jin Yin Hua (Honeysuckle Flower). (Each 5 qian) Sheng Gan Cao (Raw Licorice Root). (2 qian) Decoct in half water and half wine until concentrated, drink warm. Severe cases twice a day. Shang Bu Teng (Upper Vine). (5 liang, lightly pounded with wooden mallet, do not break) Gan Cao Jie (Licorice Joint) 1 liang. Two bowls of water, slow boil in clay pot to one bowl. Add one bowl of raw wine, boil ten more times. Divide into two doses, finish in one day. Effective when bowels and bladder are unobstructed. If not unobstructed, take again. (This formula is miraculous.)
[Encircling Medicine for All Carbuncles, Sores, and Toxins] Regardless of location, for red, swollen, elevated lesions, use Ye Zhu Ma Gen (Wild Ramie Root). (Take white and tender parts) Chi Xiao Dou (Adzuki Bean). (For pain... [Text fragmented]). For severe pain, it can relieve pain. Take skin of one large toad, wrap around perimeter, leave center hole. Two to three applications yield rapid results. Treats all regardless of Yin/Yang. Live snails. (Any number.) Add 2 qian Yin Zhu (Vermilion), crush together, apply around perimeter. 100 live snails, put in clean bottle with one cup well water, seal from evening to morning. Water becomes like saliva, mix with Zhen Ge Fen (True Clam Powder), apply around perimeter. Effective for back carbuncles. Crush scallion white, mix with honey, apply around perimeter. Mild cases effective immediately. On Dragon Boat Festival, take Cang Er Cao (Xanthium Herb). (Roots and leaves air-dried, ground fine, or ashed). Place cucumber in brine drip. At initial stage of back carbuncle, apply thin cucumber slices. Change once a week to resolve. For long-standing, unhealing ulcers, decoct Hua Jiao (Sichuan Pepper) or Gan Cao (Licorice) to wash away rotten meat. Also apply cucumber slices. Change once daily. Can close wound in 2-3 days. If all medicines for back carbuncle fail, crush Ba Jiao Gen (Banana Root) and apply. Effective.

Cover-boil in soup pot until medicinal aroma emerges. Remove. Divide and take daily until sore heals. (This formula uses mild ingredients, has great detoxification power, does not disturb Zang-Fu organs or damage Qi-Blood. Effective repeatedly for all Yin/Yang, swollen/ulcerated cases.) For severe Back Carbuncle or Brain Carbuncle, first use garlic moxibustion. If pulse is solid and bowels/bladder are blocked, first use purgatives, then this formula. Its effect is swift. If Fire-Toxin has subsided, no pus forms, does not rupture, further Tuoli (supporting interior) is needed. If ulcerated but not closing with clear pus, strong tonification is needed, such as Shi Quan Da Bu Tang (Ten Significant Tonifying Decoction).
[Ding Tong San] For all swellings/toxins, back/carbuncle pain unbearable. Stops pain upon ingestion. Bai Zhi (Angelica Dahurica). (5 qian unruptured, 2.5 qian ruptured) Bei Mu (Fritillaria Bulb). Remove fat, take only clear juice. Add previous medicines (approx. 1 liang), boil ten more times. Strain. Wash with old... piece warm. Bad flesh falls off easily. Avoid wind. Do not let people blow breath on it. Avoid menstruating women, mourners, cats, and dogs entering patient's room. Next, take Yu Hong Gao (Jade Red Ointment) with brush, melt in palm, smear evenly on new/flesh tissue. Cover externally with Hei Gao (Black Plaster). Wash and change twice morning and evening. Internally take large Spleen/Stomach tonifying warm medicines. Dead flesh sheds naturally, new flesh grows. Many formulas for washing sores exist, but this is the most effective. However, for mild cases, still use raw Gan Cao decocted strongly for washing.
[Yu Hong Gao] Bai Zhi (Angelica Dahurica). (5 qian) Gui Shen (Angelica Sinensis Root Body). (2 liang) Zi Cao (Arnebia Root). (2 qian) Gan Cao (Licorice Root). Take small amount, sprinkle on rotten flesh. Removes within one day/night. (Can also mix with sesame oil, i.e., Wu Jin Gao, treasure for removing rot and generating new tissue.)
[Sore Toxin Unstopping Ulceration] Bai Yan Mei (White Salted Plum) same as Zao Jiao (Gleditsia Fruit) ashed, retain nature, grind to powder. Bind around perimeter. For back carbuncle ulcerated exposing viscera, use one large crucian carp. Remove intestines, insert sheep manure inside. Bake until charred black, grind to fine powder. Sprinkle on sore opening, closes spontaneously. Once someone was dying from ulceration, transmitted this formula, used it with effect. Later tested repeatedly, verified. Use when pus is scant and muscle generation begins.

Frequently check if mud cracks; patch with wet cloth if so. Avoid mourners, menstruating women, chickens, dogs seeing it. When cool, the substance rising on the bowl is San Xian Dan. Treats all swellings/toxins, knife injuries broken down forming pus. Use small amount, sprinkle into sore. Cover with plaster. Unruptured cases convert to pus; ruptured cases draw out toxin and close wound. No need to change medicine. If adding Zhusha (Cinnabar) and Xiong Huang (Realgar). (Each 3 qian water-ground) sublimate together, it is Wu Xian Dan, beneficial for closing wounds. After sublimation for one year, use... enters sore without pain.
