[Song]
At the time of childbirth, do not underestimate the situation. Suddenly, Qi rushes upward causing the eyeballs to roll back. Cyan lips, clenched jaw, and drooling foam indicate that both mother and child are entering a deadly crisis. If the tongue is bright red but the lips and face are black, the fetus may survive while the mother dies in a short time. If the face is red and the tongue is black, the fetus is hard to save; one must urgently use Duoming Dan (Life-Rescuing Pill) to snatch life from death. For transverse or inverted difficult labor, Longtui San (Dragon-Shedding Powder) can be used to induce labor; for dry placenta hindering delivery, Huatai San (Slippery-Fetus Powder) can rescue the condition. To expel a dead fetus, Xionggui San (Ligusticum and Angelica Powder) is used, and crushed castor beans applied to the soles of the feet can assist expulsion. When blood enters the uterus causing distension and oppression, if it rushes upward to the heart and stomach, death is imminent. Puxiao San (Mirabilite Powder) combined with Safflower Wine, or taking Duoshen Dan (Divine Life-Rescuing Pill), will drive the blood downward and bring peace. After birth, blood confusion leading to syncope requires urgent administration of Qinghun San (Clear Soul Powder) to restore life. Massive hemorrhage leaves the heart without mastery, causing dim vision, dizziness, and profuse sweating like rain. Qi departing with blood is difficult to save, but large doses of Ginger, Cinnamon Twig, and Ginseng may still rescue the patient. Blood rushing to the heart causes chest tightness and madness; panting and impending suffocation require Pu Huang San (Cattail Pollen Powder), which should not be neglected. Pain of the "child pillow" (postpartum abdominal pain) is due to lochia retention; Dang Gui Shi Xiao San (Angelica Joy-Laughing Powder) has extraordinary efficacy. Sudden jaw clenching and inverted speech, seeing ghosts, are originally not due to Wind or Cold. This is敗血 (stagnant blood) rushing upward and consuming Heart Blood; Miaoxiang San (Wonderful Fragrance Powder) combined with Heilong Dan (Black Dragon Pill) treats this.
[Discussion]
For women during childbirth, life and death turn in an instant. Those skilled in treatment can indeed revive the dying; slight failure to rescue urgently often leads to premature death. If rescue methods are incorrect and medicines fail to act promptly, life cannot be preserved. Excessive effort during delivery, with the fetus failing to rotate in time, results in the fetus dropping into the hip cleft unable to be delivered. There are cases of difficult labor due to transverse or inverted fetal positions. There are cases due to obesity and excessive fat, coupled with a sedentary lifestyle. There are cases due to a large fetus, and cases due to narrow pelvises ("short-stone women") where the pubic symphysis fails to open. There are cases where the membranes rupture one or two days prior, causing dryness inside the uterus and difficult labor. There are cases of retained dead fetus in the womb; cold abdomen and black tongue serve as verification. There are cases of fluid accumulation in the uterus ("Bao Shui"), characterized by abnormal abdominal enlargement and weak, fine pulse. Physicians unfamiliar with this may mistake it for twin pregnancy; at delivery, over a dou of fluid must be expelled before birth. The infants have soft, short limbs and rarely survive; even if they live, they suffer physiological disabilities because water has soaked the fetus. Physicians recognizing this should medicate early to drain the fluid, ensuring safety. There are cases of massive postpartum hemorrhage leading to immediate death; there are cases of blood rushing upward causing fainting; there are cases where the fetus is delivered but the placenta remains, filled with stagnant blood preventing its descent; removing the uterine blood allows the placenta to descend. There are cases where midwives injure the internal organs while removing the placenta, resulting in chronic illness or immediate death. Caution! Caution! These conditions cannot be exhaustively listed. Generally, women of lower socioeconomic status give birth easily because their labor keeps the fetal Qi flowing; those of wealth, nourished richly and living idly, with robust bodies, find childbirth extremely difficult.
[Pulse Diagnosis]
During labor, the pulse beats six times per breath, known as the "Li Jing" (Departing from the Channel) pulse. If it is deep, fine, and slippery, with no other ominous signs, birth is imminent. A floating and large pulse indicates difficult labor, especially if accompanied by frequent chills and fever. These are ominous signs at this time; one must urgently observe facial signs. Black or cyan color on the cheeks, lips, and tongue is taboo. A red face indicates the mother survives, but the fetus's fate is doomed. If the fetus remains in the abdomen, both mother and child perish.
[Difficult Labor]
For sitting on the grass too early, excessive effort, fetal rotation failure, or membrane rupture with dried fluids causing difficult labor, use Huatai San (Slippery-Fetus Powder). For transverse/inverted positions or dry membranes, Shenying San (Divine Response Powder) taken in three doses acts like fish in water, ensuring natural rotation and birth. For failure to deliver or narrow pelvis with closed symphysis, use Xionggui Tang (Ligusticum and Angelica Decoction). For severe transverse/inverted labor nearing death, use Yimu Wan (Motherwort Pill). If hands or feet present first, prick the infant's extremities about two fen deep; pain and惊 startle cause retraction and normal delivery.
[Huatai San]
Treats difficult labor due to premature sitting and excessive effort.
Hua Shi (Talcum, 6 qian, water-flowered) Dong Kui Zi (Malva Seeds, 5 qian) Gan Cao (Licorice Root, 1 qian)
Grind to powder. Take 2 qian each time, mixed with white boiled water or wine.
[Shenying San]
Treats transverse/inverted difficult labor or premature membrane rupture; acts like fish in water.
Bai Zhi (Angelica Dahurica, 1 liang) Bai Cao Shuang (Soot from Hundred Herbs, 1 liang)
Grind to powder. Take 3 qian, mixed with child's urine and rice vinegar.
[Yimu Wan]
Treats severe transverse/inverted labor nearing death.
Yi Mu Cao (Motherwort Herb, any amount)
Dry in shade, grind to powder, mix with honey to form pills the size of a bullet. Take with child's urine and wine.
[Xionggui Tang] (See Pre-natal section for formula.)
[Inducing Labor]
For labor pain in the lower back with bleeding, use Cuisheng Ruyi San (Labor-Inducing Ruyi Powder). For irregular physiology, difficult birth, or transverse/inverted positions, take Cuisheng Dan (Labor-Inducing Pill) with warm water; the sign of efficacy is the baby grasping the pill upon exit. Alternatively, use Cuisheng Longtui San (Labor-Inducing Dragon-Shedding Powder). If hands/feet present first, apply salt to the infant's soles and scratch urgently, apply salt to the mother's abdomen, and take Shenying San.
[Cuisheng Ruyi San]
Only take when labor backache occurs.
Ren Shen (Ginseng Root, 1 qian, remove rhizome) Ru Xiang (Frankincense Resin, 1 qian) Chen Sha (Cinnabar, 5 fen)
Mix medicine with one egg white, then take with ginger decoction.
[Cuisheng Dan]
Treats irregular physiology, transverse/inverted positions, and difficult birth.
Tu Nao Sui (Rabbit Brain Marrow, one, from December, ground to paste) Ru Xiang (Frankincense Resin, 5 fen, separately ground) She Xiang (Musk, 5 fen, separately ground) Mu Ding Xiang (Clove, 2.5 qian, powdered)
Mix with rabbit brain to form pills the size of a Chinese bayberry seed. Dry in shade, wrap in oil paper. Take one pill with warm water. Efficacy sign: baby grasps the pill.
[Cuisheng Longtui San]
