夫产难子死腹中者。多因惊动太早。或触犯禁忌。致令产难。皆因秽污先下。产时未到。
秽
须顺四时方面。并避五行禁忌。若有触犯。多招灾祸也。胞衣未出。急于胎之未降。子死腹
中。危于胞之未下。盖胞胎未下。子与母通其呼吸。若子死腹中。胞脏气寒。胎血凝聚。由
于子死。气不升降。所以难产。不观古之方论。多以行血顺气之药。及硝石、水银、 砂之
类。盖胎死则血凝。气聚不能转动。再服至寒之药。不惟无益。而害母命者有矣。然古人用
药。不能无意。但后觉不分尔。子死理有二端。用药寒温。各从其宜。有妊娠崩漏。血尽子
死者。有缘颠扑内伤子死者。有缘久病胎萎子死者。若此之类。遂令坐人提挈妊妇。使身体
平正。仍以 褥自腰抵定。令仰坐开足向地。如欲产势。便进附子汤三服。方见于后。即以
衣裹温瓦数片。三二人不住手。自脐腹腰胁。左右周遭熨之。使胞脏温暖。凝血流动。盖附
子汤。内能破寒堕胎。服之少顷。觉腹内痛阵连作。恶血渐动。良久必产。此用温药之意也
。有妊妇因伤寒热病温疟之类。胎受寒邪。热毒瓦斯内外攻逼。因致死胎不即下。留于胞脏。
古人虑胎受毒瓦斯。必致贻害。故能使死胎化成腐烂。随恶血行下。此用寒药之意也。临产更
令坐人以手垫体后。胎有横有逆。有死有生。以意治之。庶免留难。凡生产之理。胎活则所
生之子。日月已周。脐系不牢。所以自下。犹实果既熟。不候剪伐。若胎死。日月未足。脐
系尚固。犹之果实未熟。本质虽枯。不折不离。所以难治。行之有法。用之有度。斯无虞矣
。凡审死胎当宜熟辨。有妊妇缘病胎隐伏不动。难认存亡者。宜先令坐人以暖手就脐下款款
按至胎处。觉冷应是胎已死。稍温即非。更看妊妇外证。面青舌冷。舌下脉青黑者。皆子死
之候也。
方
真珠汤
熟真珠（一两） 榆白皮（切一升） 上二味。以苦酒三升。煮取一升。顿服之。死胎
立出。
治妊娠未足月。胎卒死不出。产母欲死方。
以苦酒浓煮大豆。每服一升。胎立出。不能顿服。一用醇酒煮大豆。亦治积聚成瘕。
治胎死腹中。苦母病欲下之方。
取榆白皮细切。煮浓三升。服之即下。难生者亦佳。
瞿麦散 治妊娠三五个月。胎死在腹内不出。
大腹皮 赤芍药 榆白皮（各三两） 当归（一两炒） 滑石末（二分） 瞿麦 葵子

茯苓 粉草 子芩（各半两） 上为粗末。每服四钱。水一盏。煎七分去滓。不拘时候。温

夺命散 治胎死腹中危甚。
以白扁豆生去皮为末。米饮调下一钱。未下煎数服亦可。

（附论）
所谓产难且胎儿死于腹中的情况，大多是因为过早惊动孕妇，或者触犯禁忌，导致分娩困难。这都是因为污秽之血先于产期流出，而真正的产时还未到。
必须顺应四季的方位规律，并避开五行相克的禁忌。如果有触犯，往往会招致灾祸。胞衣未排出时，其危险程度仅次于胎儿尚未降下；而胎儿死于腹中，其危险程度则超过胞衣未排出的情况。这是因为胎胞尚未排出时，胎儿与母体通过呼吸相通。如果胎儿死于腹中，胞脏之气变得寒凉，胎血就会凝聚。由于胎儿已死，气机不能升降，所以导致难产。
不查看古代的方论，大多使用行血顺气的药物，以及硝石、水银、丹砂之类的药材。大概是因为胎死则血液凝滞，气聚而不能转动，如果再服用过于寒凉的药物，不仅无益，反而可能害及母亲性命。然而古人用药，并非没有意图，只是后人未能分辨罢了。
胎儿死亡的道理有两端，用药的寒温，应各自适宜。有因妊娠期间崩漏，血液耗尽而导致胎儿死亡的；有因跌倒撞击内伤而导致胎儿死亡的；有因久病导致胎萎不长而死亡的。像这样的情况，就让接生人扶持孕妇，使身体保持平正，然后用褥子从腰部固定住，让孕妇仰卧坐立，双脚张开朝向地面。如果有临产的征兆，就立即服用三服附子汤，方剂见后。随即用衣服包裹几片烧热的瓦片，由两三人不停地轮流在脐部、腹部、腰部和胁部左右周围熨烫，使胞脏温暖，凝血流动。因为附子汤，内能破除寒气堕下死胎，服用片刻后，感觉腹内疼痛阵发连作，恶血逐渐松动，很久之后必然产出。这就是使用温热药物的用意。
有孕妇因伤寒、热病、温疟等疾病，胎受寒邪，热毒瓦斯内外攻逼，因此导致死胎不能立即排出，滞留于胞脏。古人担心胎受毒瓦斯，必定会遗留危害，所以能使死胎化成腐烂，随着恶血下行。这就是使用寒凉药物的用意。
临产时更让接生人手垫在产妇身后。胎位有横有逆，有死有生，凭心意治疗，以免留下阻碍。大凡生产的道理，胎儿存活则所生的孩子，月份已满，脐带系结不牢，所以自然向下，就像成熟的果实，不用等待剪伐自会掉落。如果胎儿死亡，月份未满，脐带系结尚牢固，就像果实未熟，本质虽然枯萎，但不折断就不会分离，所以难以治疗。施行要有方法，使用要有法度，这样就没有忧患了。
凡是审查死胎应当仔细辨别。有孕妇因病胎隐伏不动，难以辨认存亡的，应先让接生人用暖手在脐下慢慢按压至胎处，感觉冷则是胎已死，稍温则不是。再看孕妇的外证，面青舌冷，舌下脉青黑的，都是子死的征候。
方
真珠汤
熟真珠（一两） 榆白皮（切一升） 以上两味药，用苦酒（醋）三升，煮取一升，一次性服下。死胎立刻排出。
治妊娠未满月，胎突然死亡不出，产母濒死方。
用苦酒浓煮大豆，每次服一升，胎立刻排出。如果不能一次服完，可用醇酒煮大豆，也能治疗积聚形成的瘕块。
治胎死腹中，苦于母病想要下胎方。
取榆白皮细切，煮浓汁三升，服下即下。难产者服用也很好。
瞿麦散 治妊娠三五个月，胎死在腹内不出。
大腹皮 赤芍药 榆白皮（各三两） 当归（一两炒） 滑石末（二分） 瞿麦 葵子（炒） 茯苓 粉草 子芩（各半两） 以上为粗末，每次服四钱，用水一盏，煎至七分，去渣，不拘时候，温服。
又须详细根据母亲的虚实冷热来使用。母亲患热病，熏蒸至此，可以使用。
疗死胎腹内不出，母已危殆，不得已者的立验方。
水银（半两） 桂末（一钱） 以上只做一服，用温酒调下，粥饮也可以。另一方法是用故鞋底加热熨烫小腹上下。
又方
以利斧赤色部分置入酒中，待温后饮之，胎儿便下。
锡粉丸 治妊娠胎死腹中，其母面赤者，宜用此方。
水银 锡粉（各一钱） 以上同研，直到不觉有水银为止，以枣肉和丸如豌豆大小，煎瞿麦汤送下。
十全博救疗胎死腹中。

This is usually caused by foul blood flowing out before the actual time of delivery. One must conform to the directional laws of the four seasons and avoid contraindications related to the Five Elements. Violations often invite disaster. When the placenta has not been expelled, the danger is second only to the fetus not having descended; however, when the fetus dies in the uterus, the danger exceeds that of an unexpelled placenta. This is because, before the placenta is expelled, the fetus and mother are connected through respiration. If the fetus dies in the uterus, the Qi of the uterine organ becomes cold, causing fetal blood to coagulate. Due to the death of the fetus, Qi cannot ascend or descend, resulting in difficult labor. Examining ancient formulas, they mostly use herbs that promote blood circulation and smooth Qi, as well as substances like Shishi (Niter), Shuiyin (Mercury), and Dansha (Cinnabar). This is likely because when the fetus dies, blood stagnates and Qi gathers without movement. Administering excessively cold medicines would not only be ineffective but could also harm the mother's life. However, ancient physicians did not use medicines without intent; it is merely that later generations failed to distinguish appropriately. There are two main reasons for fetal death: the appropriate temperature of medicine should be chosen accordingly. Some cases involve fetal death due to hemorrhage during pregnancy exhausting the blood; some due to internal injury from falls or blows; and some due to fetal atrophy from prolonged illness. In such cases, the midwife should support the pregnant woman to keep her body straight, secure her waist with a cushion, and have her sit supine with legs spread facing the ground. If signs of impending labor appear, administer three doses of Fuzi Tang (Aconite Decoction), see formula below. Immediately wrap several heated tiles in cloth and have two or three people continuously apply heat around the navel, abdomen, waist, and flanks to warm the uterine organ and mobilize coagulated blood. Fuzi Tang internally breaks cold and expels the dead fetus. After taking it, one feels intermittent abdominal pain, the lochia begins to move, and after some time, delivery occurs. This illustrates the rationale for using warm medicines.
In cases where a pregnant woman suffers from Cold Damage, febrile diseases, or warm malaria, the fetus may be affected by cold pathogens while heat toxins attack from within and outside, causing the dead fetus to remain in the uterine organ. Ancient physicians feared that toxic vapors would cause lasting harm, so they used methods to rot the dead fetus so it could be discharged with the lochia. This illustrates the rationale for using cold medicines. During labor, the midwife should place hands behind the mother. Fetal positions vary (transverse, breech), and there are living and dead fetuses; treatment should be adjusted intuitively to avoid obstruction. Generally, if the fetus is alive and full-term, the umbilical cord is loose, and the baby descends naturally, like a ripe fruit falling without cutting. If the fetus is dead and not yet full-term, the umbilical cord remains tight, like an unripe fruit that does not detach even if dried. Thus, it is difficult to treat. Methods must be applied correctly and moderately to ensure safety.
When examining for a dead fetus, careful differentiation is required. If the fetal position is hidden and immobile, making survival uncertain, the midwife should gently press below the navel with warm hands. Coldness indicates fetal death; warmth suggests otherwise. Observe external signs: cyanosis of the face, cold tongue, and dark blue-black pulses under the tongue are all signs of fetal death.

Shu Zhenzhu (Prepared Pearl, 1 liang) Yubai Pi (Cut Elm Bark, 1 sheng). The above two ingredients are boiled in Kujiu (Vinegar, 3 sheng) until reduced to 1 sheng. Take in one dose. The dead fetus will be expelled immediately.
Formula for treating sudden fetal death before full term with no expulsion, approaching maternal death.
Boil Da Dou (Soybeans) strongly in Kujiu (Vinegar). Take 1 sheng per dose. The fetus will be expelled immediately. If unable to take in one dose, boil soybeans in Chunjiu (Mellow Wine); this also treats accumulations forming masses.

Shiquan Bojiuliao (Ten Thousand Remedies for Saving Lives)

Use Hao Chensha (Good Cinnabar, 1 liang), boil in water briefly, grind fine, and take 2 qian mixed with wine.

Take one chicken egg yolk, mix with 1 he of ginger juice until uniform, and take in one dose. After delivery, eating Youcai (Rapeseed) porridge is beneficial. Another method uses Jiu (Wine) to help expel the dead fetus.
Guixin Mo (Cinnamon Heart Powder), Muxiang Dangmenzi (Musk, 1 piece). Grind together, take with warm wine. Soon, the fetus will feel as if pushed out by hand. Compared to using mercury, this does not damage Qi and blood. Another version without Musk uses 2 qian of Guimo (Cinnamon Powder). When contractions are frequent, take with warm child's urine. Named Guanyin Jiusheng San (Guanyin Life-Saving Powder). Treats difficult labor and transverse presentation.

Then add 0.5 liang of finely ground Piaoxiao (Mirabilite), boil 3-5 times, pour out, and take warm when slightly cool. The fetus will turn into bloody fluid and discharge. Another method takes it with child's urine. Lu Zai from Jinxiang used this for cats and cattle.

Shu Dihuang (Prepared Rehmannia Root, soaked in wine), Zhen Puhuang (True Cattail Pollen), Guishen (Angelica Root Body), Jiaozhi Gui (Cinnamon from Cochinchina), Yang Shaoyao (Peony), Junjiang (Ginger, peeled), Fencao (Licorice) (each 1 liang); Xiao Heidou (Small Black Soybeans, 4 liang); Baicao Shuang (Soot from Chimneys, 5 qian). Add 0.5 liang of good Huashi (Talcum Powder), 50 Kuizi (Abelmoschus Seeds), crush 7-8 Huangzhou Ye (Yellow Oak Leaves), 2 cun of Congbai (Green Onion White). Decoct with Shun Liu Shui (Smooth Flowing Water) and take. Talcum promotes urination; Yellow Oak moves Qi and drives out blood; Green Onion connects interior Yang Qi. Abundant Qi and blood lead to delivery. This formula has many effects.
Sha San (Sand Powder)
Treats failure to expel dead fetus.
Sha (Sand/Cinnabar, finely ground), Dang Gui (Angelica Sinensis) (each 1 liang). Grind Dang Gui extremely fine separately, grind Sha fine. Divide into two doses. Take with warm wine.
