In cold damage (Shanghan) disease, chest and hypochondriac fullness refers to the sensation of qi obstruction and stuffiness in the chest and hypochondrium. It is not fullness below the heart (epigastrium). Hypochondriac fullness refers to distension and fullness due to qi stagnation under the ribs; it is not abdominal fullness.
If the original Taiyang disease (Taiyang syndrome) has not resolved and the pathogen transfers to the Shaoyang channel, symptoms such as hardness and fullness below the hypochondrium, dry retching, inability to eat, alternating chills and fever, and a wiry-tight pulse will occur. Xiao Chai Hu Tang (Minor Bupleurum Decoction) should be used as the primary treatment.
If the pathogen causes obstruction in the middle jiao (middle burner), Zhi Zi Chi Tang (Gardenia and Fermented Soybean Decoction) should be used to induce vomiting. If there is palpable hardness and stuffiness in the chest with qi rushing upward to the throat causing difficulty in breathing, this indicates cold-fluid retention in the chest. Gua Di San (Melon Seed Powder) should be used for treatment.
Another question asks: In cases where irregular diet leads to discomfort in the chest and diaphragm, and cold pathogen invades the Yin channels, how should it be treated? The answer is: Urgently prepare Li Zhong Tang (Regulate the Middle Decoction) by adding Qing Pi (Green Tangerine Peel) and Chen Pi (Aged Tangerine Peel).
