Siwei Baizhu San (Four-Ingredient Atractylodes Macrocephala Decoction) [used for women's category 13: stomach deficiency with vomiting of water] Fuling Wan (Poria Pill) [used for women's category 39: nourishing and warming the stomach, treating epigastric fullness and distension, aversion to food] Zhuru Tang (Bamboo Shavings Decoction) [used for women's category 33: clearing phlegm and stopping vomiting] Wufu Tang (Black Aconite and Ligusticum Decoction) [used for women's category 35: harmonizing qi and nourishing the stomach]
During pregnancy, improper management or emotional stagnation and anger may lead to rebellious Qi, often resulting in symptoms of Qi rushing upward and pressing. If the condition is characterized by rebellious Qi and excess causing distension and pressure, Jiegan Jian (Liver-Resolving Decoction) is indicated. If it is caused by cold in the stomach combined with excess, Hewei Yin (Stomach-Harmonizing Drink) is indicated. If there is stomach fire accompanied by stagnation, Zhike Tang (Immature Bitter Orange Decoction) is indicated. If there is spleen deficiency accompanied by stagnation, Zisu Yin (Perilla Leaf Drink) is indicated. If spleen deficiency leads to impaired Qi movement, Sijunzi Tang (Four Gentlemen Decoction) is indicated; for severe cases, Bazhen Tang (Eight Treasure Decoction) is used. If spleen Qi deficiency is accompanied by cold, Wu Junzi Jian (Five Gentlemen Decoction) is indicated. If spleen and kidney Yang deficiency causes failure of Qi movement, Liyin Jian (Yin-Nourishing Decoction) is indicated. If spleen and kidney Qi deficiency is accompanied by fire, Xiaoyao San (Free and Easy Wanderer Powder) is indicated, or Huangqin (Scutellaria Root), Zhike (Immature Bitter Orange), and Sharen (Amomum Fruit) can be added. If the fetus has died in the uterus, with cold Qi rushing upward causing nausea, vomiting, and a greenish complexion, the treatment should follow the guidelines under the section "Fetal Restlessness Threatening Miscarriage."
A formula for treating fetal Qi rushing upward, accompanied by hot pain and bleeding, or severe restlessness and distress, uses twenty stalks of scallion boiled strongly in water for drinking. If the fetus is alive, it will be secured; if dead, it will be expelled. If ineffective, it may be taken again. For fetal restlessness with irritability, cyanosis of the lips and mouth, and cold extremities, Danggui Tang (Angelica Sinensis Decoction) is required.
When a pregnant woman experiences unsecured menstrual blood, it is called Tai Lou (Fetal Leakage). The causes of Tai Lou include those due to fetal Qi and those due to pathological Qi. The causes related to fetal Qi are of two types. I once treated a woman whose pulse was slippery and rapid, without other signs of wind-heat or disease. Upon inquiry, her menstruation continued as usual but was slightly less than before. I said, "This must be pregnancy." This occurs because the fetus is small while the blood is abundant and surplus. Later, after three months, her menstruation stopped, and she indeed gave birth to a son. Thus, many pregnant women have such conditions. It is common now to see births at seven or eight months of gestation. People consider the cessation of menstruation as the limit, calling it premature, yet conception occurred before menstruation ceased, and by this time the fetus is full-term and robust, which people do not realize. However, such fetal Qi conditions also differ in the balance of Yin and Yang. If the mother's Qi is strong, providing ample nourishment to the fetus leading to blood overflow, although blood leaks, the newborn remains strong; this indicates strong Yin, requiring no treatment. If the father's Qi is weak, and the fetus cannot fully receive nourishment leading to blood leakage, it results from a deficiency of both essence and blood, causing the newborn to be small and withered; this indicates weak Yang, which is also unknown to many. All these stem from congenital factors. Although some cases seem beyond intervention, cultivating the root certainly offers ways to reverse the situation. Cases of turning nothing into something or weakness into strength are rare and cannot be explained by ordinary terms. As for leakage caused by disease, it is simply treated according to the underlying disease.
For pregnancy-related leakage due to blood heat, choose Bao Yin Jian (Kidney-Yin Preserving Decoction) or Qinghua Yin (Clearing Transformation Drink). For leakage caused by liver fire triggered by anger, use Bao Yin Jian; for severe cases, Huagan Jian (Liver-Transforming Decoction) is primary. For spleen deficiency failing to control blood, use Shoupi Jian (Spleen-Nourishing Decoction) or Sijunzi Tang (Four Gentlemen Decoction). For spleen deficiency with blood heat and Qi stagnation, use Sixian San (Four Saints Powder). For simultaneous spleen and kidney deficiency, use Wuyin Jian (Five Yins Decoction). For deficiency of Qi and Blood in the Triple Burner, use Wufu Yin (Five Blessings Drink) or Qifu Yin (Seven Blessings Drink). For bleeding caused by fatigue and overwork, use Shoupi Jian (Spleen-Nourishing Decoction) or Guipi Tang (Spleen-Restoring Decoction). For accidental injury causing bleeding, use Wufu Yin (Five Blessings Drink) or Antai San (Fetal-Securing Powder). For deficiency of Qi in the Chong and Ren vessels, failing to constrain blood leading to easy slipping and movement, use Guyin Jian (Kidney-Yin Consolidating Decoction) or Miyuan Jian (Source-Sealing Decoction).
Xue Lizhai stated: For the above conditions, if caused by Qi heat, use Fangfeng Huangqin Wan (Saposhnikovia and Scutellaria Pill); if caused by blood heat, use Jiawei Xiaoyao San (Modified Free and Easy Wanderer Powder); if caused by blood deficiency, use Erhuang San (Two Huangs Powder); if caused by excessive blood loss, use Bazhen Tang (Eight Treasure Decoction), and if ineffective, Buyi Zhongqi Tang (Tonify the Middle and Augment the Qi Decoction); if caused by liver fire, use Chaihu Qinggan San (Bupleurum Liver-Clearing Powder); if caused by spleen fire, use Jiawei Guipi Tang (Modified Spleen-Restoring Decoction); if caused by stress-induced bleeding and pain, use Bazhen Tang (Eight Treasure Decoction) with Ejiao (Donkey-Hide Gelatin) and Shuai Ai (Prepared Mugwort Leaf); if caused by spleen and stomach weakness, use Buyi Zhongqi Tang (Tonify the Middle and Augment the Qi Decoction) with Wuweizi (Schisandra Berry); if caused by sinking of spleen and stomach Qi, use the previous decoction with increased doses of Shengma (Cimicifuga Rhizome) and Chaihu (Bupleurum Root); if evening fever and internal heat occur, Xiaoyao San (Free and Easy Wanderer Powder) is appropriate.
