Zhu Danxi stated: Nocturnal emission and spermatorrhea are primarily attributed to Heat. Heat causes the uncontrolled flow of essence; treatment should focus on nourishing Yin and descending Fire. For cases caused by excessive mental labor and overthinking, one should calm the Spirit and nourish the Heart. If the condition persists for a long time leading to deficiency collapse, tonifying herbs combined with astringent herbs must be used, and there is no case that will not heal.
Xue Lizhai stated: Regarding the aforementioned conditions, if Kidney Qi is insufficient, Yizhi Tang (Yi Zhi Decoction) or Jin suo Zheng yuan Dan (Golden Lock Correcting Origin Pill) should be used; if there is Deficient-Heat in the Liver and Kidneys, Liuwei Wan (Six-Ingredient Pill with Rehmannia) or Jiawei Xiaoyao San (Modified Free and Easy Wanderer Powder) should be used; if there is Spleen deficiency with Heat, Liuwei Wan or Buzhong Yiqi Tang (Tonify the Middle and Augment the Qi Decoction) should be used. All these belong to deficiency syndromes suitable for Shiquan Dabu Tang (Ten Complete Great Tonifying Decoction), or using Bilixi Qingyin (Bilixi Clearing Drink) to deliver Bawei Wan (Eight-Ingredient Pill). He further stated: Regarding the aforementioned conditions, they result from the depletion of the three Yin meridians of the foot. If there is Deficient-Heat in the Liver and Kidneys, Siwu Tang (Four Substance Decoction) added with Chai Hu (Bupleuri Radix), Shan Zhi (Gardeniae Fructus), Shan Zhu Yu (Corni Fructus), and Shan Yao (Dioscoreae Rhizoma) should be used. If there is Spleen and Stomach Qi deficiency, Buzhong Yiqi Tang added with Shan Zhu Yu and Shan Yao should be used. If overthinking damages the Spleen Qi, Guipi Tang (Restore the Spleen Decoction) added with Shan Zhu Yu and Shan Yao should be used concurrently. If there is Liver and Kidney depletion, Liuwei Wan should be used. If True Yang is deficient and failing, Bawei Wan should be used. If Heart and Kidney fail to interact, Bilixi Qingyin should be used. If there is Heart Qi deficiency with Heat, Qingxin Lianzi Yin (Clear Heart Lotus Seed Drink) should be used.
Lou Quanshan recorded in Bencao Gangmu (Compendium of Materia Medica): A young man suffered from nocturnal emission and white turbidity; after taking astringent herbs for essence retention, his condition worsened. Recognizing it as stagnation, he switched to Daochi San (Guide Out Redness Powder), administered in large doses, which stopped both emission and turbidity. Another middle-aged man suffered from nocturnal emission; astringent herbs were ineffective. He was treated with Shenxiong Wan (Shenxiong Pill) to purge, followed by Zhuling Wan (Polyporus Pill) after purgation, resulting in recovery.
Xu Donggao stated: Nocturnal emission occurs due to Fire in the Heart meridian and unsettled Spirit/thoughts, causing ejaculation during dreams of sexual intercourse. Treatment should employ formulas that clear the Heart, calm the Spirit, and warm the Gallbladder, adding Huang Lian (Coptidis Rhizoma), Sheng Di (Rehmanniae Radix), Ren Shen (Ginseng Radix et Rhizoma), Yuan Zhi (Polygalae Radix), Fu Shen (Poria with Hostwood), Suan Zao Ren (Ziziphi Spinosae Semen), Ling Yang Jiao (Antelopi Cornu), and similar herbs. For spontaneous emission, this is due to Qi and Blood deficiency leading to downward collapse; for those caused by Heat leading to uncontrolled flow, one must differentiate between deficiency and excess, using Bawu Tang (Eight Substance Decoction) added with Long Gu (Fossilia Ossis Mastodi) and Mu Li (Ostreae Concha), Chu Gen Pi (Stemonae Rhizomatis Cortex), and similar herbs. For cases where essence flows uncontrollably after urination, or flows spontaneously without urination, or flows from the urethra accompanied by itching and pain with a constant sensation of needing to urinate, one should first take Chensha Miaoxiang San (Cinnabar Wonderful Fragrance Powder), or Weixi Wan (Weixi Pill), or Liqing Yin (Liqing Drink). Additionally, Long Gu wrapped in cotton should be decocted together, or Wubeizi (Galla Chinensis), Mu Li, Bai Fu Ling (Poria), and Wu Wei Zi (Schisandrae Fructus) may be added to the decoction for administration.
Wang Yutai stated: In cases of uncontrolled seminal leakage accompanied by spontaneous sweating, dizziness, extreme weakness, alternating chills and fever, where tonifying and astringent herbs are ineffective, and the pulse is floating, soft, and scattered, this is not merely a deficiency syndrome, nor is it caused by excessive sexual activity. The cause lies in the mind having witnessed something, thereby developing affection; the intent has desires, and when arousal arises but remains unsatisfied, this disease results. Since medicinal tonification and consolidation have failed, how else can it be treated? The reason is that when the Heart loves, the Spirit does not return; when the Intent thinks, the Will is not peaceful. One must first harmonize Ying and Wei (Nutritive and Defensive levels); when Ying and Wei are harmonized, the Heart becomes calm. Next, regulate the Spleen; when Spleen Qi is harmonized, the residence of the Will stabilizes. When Heart and Kidney interact and Essence and Spirit remain within, the disease heals naturally. The method involves using 3 qian of Ren Shen, 1 qian of Dang Gui, cleaned and dried, ground into powder, divided into three doses, taken with Rice Decoction (Nuomi Yin). After completion, sweating will occur naturally, and chills and fever will subside. If dizziness persists, use 3 qian of Chuan Xiong (Chuanxiong Rhizoma) and 1 qian of Ren Shen, dried and ground into powder, divided into three doses, taken with boiling water. If dizziness resolves but seminal control remains poor, use 0.5 liang of Shao Yao (Paeoniae Radix Alba), 3 qian of Ding Xiang (Caryophylli Flos), and 3 qian of Mu Xiang (Aucklandiae Radix), crushed into a powder. Each dose should be decocted with five slices of Sheng Jiang (Zingiberis Rhizoma Recens) and two dates (Da Zao), taken on an empty stomach. This will calm the Heart, stabilize the Spirit, secure the essence, and bring joy to the Shen.
