For all types of headache unresponsive to medication: Use Gancao (1.5 liang), boil in one bowl of water until reduced to 0.7 parts, remove dregs, and take warm. If ineffective, increase to 2 liang. (From *Weisheng Yijian Fang*)
[Dujie Zhufang]
Zhang Sanxi frequently observed severe headaches unresponsive to myriad prescriptions. Upon inquiry, he learned the patient had previously suffered from syphilitic sores. He used Tufuling (Smilax Glabrae Rhizoma, 4 liang), Baixianpi (Dictamni Cortex, 3 qian), Kushen (Sophorae Flavescentis Radix, 3 qian), Huangbai (Phellodendri Cortex, 1 qian), Zaojiaozizi (Gleditsiae Fructus Seed, 30 grains), Yiyiren (Coicis Semen, 2 qian), Mutong (Akebiae Caulis, 2 qian), Fangfeng (Saposhnikoviae Radix, 2 qian). For Qi deficiency, add Renshen and Huangqi; for Blood deficiency, add Siwu Tang (Four Substances Decoction). This yielded remarkable results, also alleviating body pain. (From *Yixue Liuyao*)
(Note: The former Instructor stated: "The so-called Yuduo Tang is precisely this formula.")
[Toufeng Shenfang]
Transmitted by Vice Minister Shen Guanyi from a Daoist priest. My servant's wife suffered severely from this pain, attempting suicide. After two doses, she did not relapse for years.
Tufuling (avoid iron, 4 liang), Jinyinhua (Lonicerae Japonicae Flos, 3 liang), Manjingzi (Vittariae Spora, 1 qian), Xuanshen (Scrophulariae Radix, 0.8 fen), Fangfeng (Saposhnikoviae Radix, 1 qian), Tianma (Gastrodiae Rhizoma, 1 qian), Xinyihua (Magnoliae Flos, 0.5 fen), Chuanxiong (Chuanxiong Rhizoma, 0.5 fen), Heidou (Sojae Semen Nigrum, 49 grains), Xincaogen (Juncus Effusus, 20 roots), Yacha (Sprouted Tea, 5 qian).
Boil with 1.5 zhong each of river water and well water down to one zhong, and take. (From *Guangbiji*) *Yitong* Shanniu Tang: For syphilitic headache persisting, remove Manjingzi and Xincaogen from this formula.
[Chubi Zhufang (Moxibustion Method Attached)]
*Boji Fang* Zhi Ling San for treating migraines.
Xionghuang (Realgar), Xixin (Asari Radix et Rhizoma) in equal parts, ground fine. Use less than one zi per application. For left-sided pain, sniff into the right nostril; for right-sided pain, sniff into the left nostril. (From *Bencao*)
Migraine Formula: Transmitted from Emperor Yuling via Wang Anshi, said to be a secret palace recipe. Use raw Luoban juice (Radish Juice) in the amount of one shell. Lie supine and instill into the nose: left pain into right nostril, right pain into left nostril, or both. Decades-long cases were cured in one application. Wang Anshi told his servants that several people were cured. (From *Su Shen Liangfang*)
For Wind-headache and migraine, use Chuibi San (Snuff Powder):
Gouti (Trichosanthis Fructus, 1 qian), Dilong (Pheretima, 1 qian), Ku Hu (Bitter Bottle Gourd, 1 qian), Xiaoshi (Natrii Sulfas, 1 qian), Shexiang (Moschus, 1 qian).
Grind all powders uniformly. First fill the mouth with water, then insufflate half a zi of powder deep into the nose. Expelling foul matter yields miraculous effects. (From *Taiping Shenghui Fang*) (Note: There is also a single-ingredient formula using Ku Hulu Zi.)
For lateral/corrective wind-headache, Jiannaofeng (Brain-clamping Wind), and all wind-headaches, regardless of duration, effective on the day, named Touting San:
Xixin (three long white stems), Gouti (seven pieces), Dingxiang (Caryophylli Flos, three grains), Nuomi (Glutinous Rice, seven grains), Naonao (Borneol, size of one bean), Shexiang (Moschus, size of one black soybean).
First grind Borneol and Moschus finely in a mortar. Then grind the other four ingredients into powder, mix with the Borneol and Moschus in the mortar. Store in a clay bottle, tightly sealed. Patient insufflates an amount the size of a large soybean into either nostril. Depending on duration, expelling about one sheng of cold saliva brings relief. (From *Benshi Fujiji*)
After washing hair and neck, if water-dampness enters the brain causing pain like traction or locking, use Gouti ground fine. Patient fills mouth with water, then insufflates Gouti powder into the nose to drain foul water; pain ceases immediately. (From *Xu Yijian Houji*)
[Biyun San]
Treats toxic accumulation entering the vertex, causing headache as if breaking. Snuff into nose.
Ebusicao (Centipedae Herba, 1 liang), Chuanxiong (Chuanxiong Rhizoma, 1 liang), Qingdai (Indigo Naturalis, 1 qian).
Grind fine. Patient holds cool water in mouth. Use a reed tube to blow powder into the painful side's nostrils. Sneezing indicates efficacy. (From *Waike Zhengzong*)
Regarding snuff formulas: *Benshi Fang* uses only acrid-warm herbs; Dongyuan and Hejian use acrid-cool. Long-standing wind often transforms into heat, but stagnant Qi requires warmth to unblock. One must adjust based on the disease; this is the responsibility of the wise. (From *Jingui Yi*) (Note: *Yitong* records Cheng Wenbin's verification,辨析 of snuff methods, which is deemed inaccurate and thus omitted.)
For lateral/corrective headache and Jiannaofeng: Use Dilong (earthworm) dried and roasted, 1 fen; Xuxiang (Frankincense), 0.5 liang, separately ground fine. Use 0.5 qian per application. Spread powder on a paper strip, roll into a paper cigarette, do not dip in oil, burn over an incense burner, and smoke into the nose. (From *Suosuilu*)
[External Application Methods]
For pain triggered by cloudy/rainy weather: Use Guimo (Cinnamon Twig Bark powder), 1 liang, mixed with wine to paste consistency. Apply to vertex and Taiyang acupoints. (From *Yonglei Qianfang*)
Toufeng Bingzi (Headwind Cake): Some use Wuweizi (Schisandrae Fructus), Quanxie (Scorpio), Tudougou (Steleophaga Plancyi Bolkovii), seven each, mixed with vinegar into cakes. Some use Nanxing (Arisaematis Rhizoma) and Chuanxiong in equal parts, pounded with scallion whites into cakes. Some use Bamazi (Ricini Semen) and Xuxiang. Some use Dahuang (Rhei Radix et Rhizoma) and Mangxiao (Natrii Sulfas) mixed with mud from the bottom of a well for pasting. However, external treatments should generally use acrid-warm dispersants regardless of cold/heat pathogenic factors. Using purely bitter-cold herbs increases stagnation. Unless heat is extreme, they should not be used lightly. (From *Jingui Yi*)
[Biansheng Fa (Bloodletting Method)]
I once treated an elderly woman with chronic headache. Observing purple-black blood vessels on her hands and feet, I used a Sanleng needle (Tripterygium hypoglaucum lancet) to prick them, draining cups of ink-like blood. Later, I moxaed and needled the affected meridians, leading to recovery. This aligns with the Classic saying: "Great Bi manifests as evil," and "Chronic headache and Bi not leaving the body: observe blood vessels and exhaust the blood." (From *Bencao Gangmu*)
For chronic lateral/corrective wind-headache unresponsive to treatment, caused internally by Phlegm-Saliva, Wind-Fire, stagnating collaterals, and blocking Qi-Blood, potentially leading to blurred vision, tight pupils, and constipation/urinary difficulty: It is appropriate to bleed to open stagnation and release the exterior. (From *Neike Yaoyuan*)
[Moxibustion Method]
To moxa for headwind, start with Xinhui, Baihui, and Qding acupoints. For headwind with eye pain, moxa Shangxing, Shentong, and Houding acupoints. I have verified this on myself and others. (From *Zisheng Jing*)
A scholar suffered brain-heat headache, so severe he threw himself off the bed, hitting his head on the floor. Cold water provided slight relief but pain persisted despite medications. Someone advised moxaing Xinhui, which cured him. If heat-pain can be moxaed, how much more so cold-pain? For Brain Pain, Brain Vertigo, and Brain Diarrhea, first moxa Xinhui; Jiangjian and other acupoints are secondary. (Same source)
[Appendix: Meilinggu Pain (See Preceding Sections for Syndrome Differentiation)]
Eyebrow/orbital pain belongs to Wind-Heat and Phlegm; treat as Wind-Phlegm, similar to gout. (From *Danxi Xinfa*)
Orbital bone pain has two syndromes: Eyes belong to Liver. Liver deficiency causes pain, worsening upon exposure to light; use Shengshu Dihuang Wan (Rehmannia Pills). Ingredients: Dihuang (Rehmanniae Radix), Shihu (Dendrobii Caulis), Zhike (Aurantii Fructus), Qianghuo (Notopterygii Rhizoma et Radix), Fangfeng (Saposhnikoviae Radix), Niuxi (Achyranthis Bidentatae Radix), Ganju (Chrysanthemi Flos), Xingren (Armeniacae Semen Amarum). Another syndrome is Fluid Retention in the Liver Meridian: attacks cause eyebrow bone pain, inability to open eyes, quiet during day, worse at night; use Daotan Yin (Phlegm-Dissolving Decoction), or Xiongxin Tang removing tea buds, or Erchen Tang swallowing Qingchuan Baiwan Wan. Select medicines from the Phlegm-Fluid section. (From *Yaojue*)
Meibone is where the Eye System passes, reaching up to the brain, because eyes belong to the brain. If Yang meridians carry external pathogens,郁 transforming into Wind-Heat-Toxin, attacking the brain upwards and eyes downwards, pain merges along the Eye System through the meibone. If Heart/Liver Heat attacks the eyes, pain connects Eye System and meibone. Similarly for Chest/Diaphragm Wind-Phlegm attacking upwards. (From *Zhunseng*)
This syndrome mostly belongs to Yangming Wind-Heat, with Deficiency and Excess paths. Deficiency pain triggers upon seeing light; use Xuanqi Tang plus Danggui and Baishao. Excess presents with inability to open eyes, quiet day/worse night; use Xuanqi Tang plus Congbai and Chidou, plus Gegen (Kudzu Root) for strong Wind, plus Shigypsum (Gypsum Fibrosum) for strong Fire. Generally, clear fire and dissipate pain; if ineffective, nourish Yin. Overusing Wind-dispersing herbs causes Fire-Heat to rise, worsening pain. (From *Yitong*)
Contraindications and indications for Meibone Pain are the same as for Yin-Deficiency Headache. (From *Bencao Jingshu*)
[Bingyu Tang]
Treats unbearable Meibone pain due to Phlegm-Jue. (From *Yangshi*) (i.e., Xiao Banxia Tang)
[Xuanqi Tang]
Treats unbearable Meibone pain.
Qianghuo, Fangfeng (each 3 qian), Gancao (honey-fried 3 qian), Huangqin (wine-processed 1 qian; omit in winter. If eating causes heat-pain, add Huangqin).
Crush. Take 3 qian per dose, add two zhan of water, boil to one zhan, remove dregs, take slightly warm, frequently after meals. (From *Shixiao Fang*) *Tongzhi* Qufeng Qing Shang San: For Wind-Heat attacking upper body and Meibone pain, add Baizhi, Chaihu, Chuanxiong, and Jingjie to this formula.
[Miantong (Facial Pain)]
The face is the Yangming region. Yangwei Mai originates from the meeting of all Yang meridians, all located on the face. Thus, facial pain is caused by Fire, differing in Deficiency and Excess. Sudden pain is mostly Excess; chronic pain is mostly Deficiency. High pathogenic factors suppress them; stagnant ones disperse them; Blood-Heat cools them; Qi-Deficiency tonifies them. Do not solely rely on bitter-cold fire-draining. Scholar Xu treated nasal pain with Xijiao Shengma Tang. An elderly man suffered fatigue-induced facial pain when hungry; treated with Buzhong Yiqi Tang plus Huangqin, Zhizi, Lianqiao, Niubangzi, and Heishen. Facial pain due to depression accumulating into Stomach Heat was treated with Yueju Wan plus Zhizi, Lianqiao, Beimu, and Juhong. (From *Yitong*) (Note: This is an excerpt from *Zhunseng*.)
[Xijiao Shengma Tang]
Wang Jianzheng Xigao suffered pain between nose and forehead, or numbness, for years. Suddenly, lips, jaw, and hairline became painful, unable to open mouth, interfering with speech and eating. Left forehead and cheek felt tight like paste, painful to touch. I diagnosed Foot-Yangming Meridian receiving Wind-Toxin, transmitting to collaterals, causing blood stasis. Others tried Paifeng, Xiao Xuming, or Toubing Dan without effect. I prepared this decoction for him, curing him in days.
Top-grade Xijiao (Rhinocerotis Cornu, shaved 1.1 liang), Zhen Chuan Shengma (Cimicifugae Rhizoma, 1 liang), Fangfeng (remove forked root), Qianghuo (remove rhizome, each 0.3 fen), Baizhi (avoid roasting), Huangqin (peeled), Chuanxiong (washed), Baifuzi (fried, each 0.5 liang), Gancao (honey-fried 0.1 fen).
Coarse powder. Take 4 qian per dose, add 1.5 zhan of water, boil to 0.8 zhan, remove dregs, drink thoroughly. After meals and before sleep, 3-4 times daily. Foot-Yangming is Stomach. Stomach accepts all flavors; toxins from rotting food accumulate there. Hence, Xijiao is chief to resolve dietary toxins, assisted by Shengma; remaining herbs clear Wind-Heat. (From *Benshi Fang*) (Foot-Yangming... paragraph referenced from *Yitong* excerpt.)
[Jingxiangtong (Neck and Occiput Pain)]
Neck pain radiating from headache should be sought among headache syndromes. If pain is isolated to the neck, it is not Wind-Evil, but Qi-stagnation or stiff neck from sleeping posture. All suit Heqi Yin, taken after meals. (From *Yaojue*) (Heqi Yin formula seen in *Heji Ju Fang*.)
For Kidney-Qi attacking upward, causing inability to turn neck/back, use Jiaofu San:
Da Fuzi (Aconiti Lateralis Radix Praeparata, one piece >6 qian, fried, skin/umbilicus removed, powdered).
Take 2 large qian of powder, 20 good Sichuan peppercorns (Zanthoxyli Pericarpium) filled with white flour. Boil with 1.5 zhan of water and seven slices of ginger to 0.7 parts. Remove peppercorns, add salt, take on empty stomach. A relative suffered neck tendon pain radiating to back, unable to turn, unresponsive to Wind medicines. I recalled *Qianjin Suifang* regarding Kidney-Qi attacking back/neck stiffness. I prescribed this, cured in two doses. Verified since. Kidney-Qi ascends from waist along spine to Caoxi Acupoint, then enters Ninian Palace. Caoxi point requires mastery of internal circulation to penetrate. Now reversing here causes blockage. Peppercorn guides Qi back to meridian, bringing peace. Caoxi Acupoint is Fengfu Acupoint. (From *Benshi Fang*)
Neck stiffness/pain often involves Phlegm making turning difficult; Phlegm residing in Taiyang. Erchen Tang plus Jiu Huangqin, Qianghuo, Honghua. (From *Yitong*)
[Huitou San]
Treats head/neck stiffness, tendon pain, or inability to turn head. Wu Yao Shunqi San (see Stroke section) plus Qianghuo, Duhuo, Mugua. (From *Jingui Yi*)
[Tianbaiyi]
Treats buzzing sound inside head like insect chewing, named Tianbaiyi. Use fine Camellia seed powder snuffed into nose. (From *Zhunseng*)
Buzzing inside head, named Tianbaiyi, mostly belongs to Fire, sometimes Phlegm-Dampness above. Danxi said: Thin people belong to Fire; use Bohe, Zhizi, Fuling, Gancao, Xixin, Chuanxiong, Huangqin, Shigypsum, Yacha. Fat people belong to Wet-Phlegm; use Banxia, Fuling, Zhishi, Huanglian, Tianma, Dansheng, Cangzhu, Huangbai, Yacha. Dai Fu'an said: Head ringing has Deficiency and Excess. Excess: use Liangge San or Mengshi Wan to purge downward. Deficiency: Dushen, Baoyuan, Liuwei, Bawei, Rongzhu Dan, Lurong Wan may fail. Alchemical formula: Camellia seed powder snuffed into nose. Sound belongs to Fire; Camellia seed is light/clear, travels clear pathways, disperses suppressed Fire. Tea is always used as guide in Headwind medicines; understand this. (From *Yitong*) (Note: *Lantai Guifan* says: This is a strange disease, must know.)
