Commentary: The *Shennong Bencao Jing* records five types of Gan syndromes, named according to the five Zang organs affected. Here, we discuss the five manifestations of Kidney Gan, which are the most critical and urgent, and cannot be treated as ordinary diseases. This disease presents with five manifestations and spreads rapidly, which is terrifying. Thus, the name "Zou Ma Gan" (Running Horse Gan) is not unfounded. At the initial stage, the infant has bad breath rising to the stomach, with foul-smelling respiration. Gradually, it damages the tendons and gums, causing sores on the flesh, either swelling or ulcerating. The teeth become scorched and black. Progressively, sores erupt from the gum tissue, breaking down and discharging pus. Further on, heat forces its way into the vessels, causing intermittent bleeding. Prolonged heat accumulation causes gum decay, widening of the alveolar bone, and tooth loss. If a six- or seven-year-old child loses all their teeth, they will never grow back; how can this be cured? Now, providing excellent prescriptions, treatment should be administered promptly, adjusting according to the progression of the disease, rather than waiting for the illness to manifest severely before medicating.
For infants and young children with weak spleen and stomach, inability to consolidate waste, persistent diarrhea and dysentery (red or white), imbalance of cold and heat, frequent episodes day and night, recurring after remission, the Li Chang Dou Kou San (Intestine Cardamom Powder) is suitable:
Rou Dou Kou (Myristicae Semen, one nut, baked), Hu Fen (Lead Subcarbonate, stir-fried, 2 liang), Long Gu (Fossilia Ossis Mastodi, raw, 1 liang), Bai Fan (Alumen, calcined, 1 liang).
Grind the above four herbs into powder. Take one tablespoonful each time, mixed with warm rice gruel, at any time. Alternatively, make thin paste pills the size of hemp seeds, taking 50–60 pills each time.
Commentary: Hu Fen is true lead powder, manufactured using lead methods. Produced in Shaozhou, it is called Shao Fen; produced in Dingzhou, it is called Ding Fen; collectively known as Guang Fen. Its nature is astringent and cold, thus used to secure the intestines and prevent deficiency-slipperiness. Dou Kou is a herb that warms the viscera and harmonizes the spleen and stomach; Long Gu and Bai Fan astringe the intestines and stop dysentery. For patients suffering from intestinal deficiency, slippery discharge, and severe dysentery occurring day and night without control, taking this remedy leads to timely recovery. All meat, fish, and pickled foods must be strictly prohibited. This formula is also highly effective for treating autumnal white dysentery.
