(Zhen, pronounced as 'zhen' or 'cheng') This condition arises from food stagnation. The pain is stabbing in the hypochondrium and chest/epigastrium, accompanied by mental restlessness, inability to eat, regurgitation, and nausea. If not treated for a long time, it gradually forms a mass. This is also called "Food Jia" (食瘕 - Food Mass).
(Xia/Jia, pronounced as 'xia' or 'jia') This condition arises from blood injury. There is oppression in the chest and diaphragm, with pain radiating to the lower abdomen. Occasionally, there is attacking and pounding sensation moving upward to invade the chest and heart region. Although eating is not obstructed, muscles do not grow, and the body wastes away. If not treated for a long time, it gradually forms a Jia knot. This is also called "Blood Knot" (血结 - Blood Stasis/Mass).
(Pi, pronounced as 'fu', 'bi', or 'bi') This condition arises from Qi injury. There is distension of the heart and abdomen, abdominal enlargement, fullness in the hypochondrium, and stabbing pain that moves back and forth, affecting the left hypochondrium. The complexion is yellow, the skin and flesh are emaciated, and there is fatigue and weakness. If not treated for a long time, it gradually forms a Pi mass. This is also called "Qi Mass" (气块 - Qi Lump).
(Pi, pronounced as 'pi') This condition arises from accumulation injury. Its symptoms resemble those of intestinal dysentery (Chang Pi), with frequent loose stools that are slippery and unformed, resembling dysentery but not quite, resembling parasites but not quite. The abdomen is dry and painful, with an attacking sensation moving upward, causing chest and heart fullness and oppression. If not treated for a long time, the stubborn knot does not dissipate, and masses re-form, resembling the appearance of Pi.
The above four conditions all present with alternating chills and fever, similar to malaria symptoms. They are all caused by damage to the five Zang organs. However, only Pi and Pi masses located in the left hypochondrium are due to Qi aggregation, occurring in both men and women on the left side. The general principles are the same, with minor differences. For treating chills and fever, first administer Li Jiang Yin (Pear Syrup Decoction) for one or two doses to resolve the chills and fever. After the chills and fever have subsided, wait three to five days before administering Mo Pi Wan (Pi-Grinding Pill), to be given frequently every day. For Jia diseases, only administer San Leng Wan (Curcuma Longa Pill) to eliminate accumulated toxins, aiming for free flow as the endpoint.
To treat children suffering from Pi and Pi masses who, after consuming arsenic pills, develop thirst, irritability, edema of the head and face, tight abdominal distension, panting, inability to sit or lie comfortably, emaciation, exhaustion, and persistent chills and fever, it is appropriate to use Ren Shen Luo Bo Yin (Ginseng and Radish Decoction).
Bai Zhu (Atractylodis Macrocephalae Rhizoma), Ku Jing (Platycodonis Radix), Gan Cao (Licorice Root, honey-fried), Ren Shen (Ginseng Root), Mai Men Dong Zi (Ophiopogonis Seed, heart removed).
These are processed into equal parts and ground into powder. Each dose consists of one large Qian Bi (measure), taken with half a cup of fresh radish juice, boiled down to half volume, cooled, and administered.
To treat pediatric Pi Qi that has not resolved, where repeated purgative treatments have led to deficiency and exhaustion, abdominal pain, loss of appetite, puffy face and eyes, and vomiting when forced to eat, it is appropriate to use this formula: Dou Kou Cao Guo Yin Zi (Cardamom and Tsaoko Decoction).
Rou Dou Kou (Nutmeg, one roasted), Cao Guo (Tsaoko Fruit), Bing Lang (Areca Seed, each 1 Qian), Mian Huang Qi (Astragalus Root, pounded and honey-fried), Bai Fu Ling (Poria), Bai Shao Yao (White Peony Root), Bai Zhu (Atractylodis Macrocephalae Rhizoma), Gan Cao (Licorice Root), Chen Pi (Citrus Reticulatae Pericarpium, each 1 Qian), Ban Xia Qu (Pinellia Fermented Paste, 1 Qian).
The herbs are coarsely crushed. Each dose consists of 2 Qian Bi, combined with three small slices of fresh ginger, half a dark plum (Wu Mei), and one jujube. These are wrapped in wet rattan paper and roasted until fragrant. Remove the paper, add a small cup of water, boil down to half volume, strain, and administer orally on an empty stomach before meals. The dregs should be re-boiled and taken together. Additionally, take Xiao Chen Xiang Jian Wan (Small Agarwood Decoction Pills) concurrently for enhanced efficacy.
