1. Acute pain in the umbilical and abdominal region; 2. Heavy bearing-down sensation in the lower back; 3. Sensation of fire emerging from the eyes; 4. Urgency and tightness at the anus; 5. Swelling and fullness of the vaginal orifice (Chanhu); 6. Twitching of the tendons and vessels in the middle finger of the hand; 7. Simultaneous discharge of amniotic fluid or blood. Only when these seven signs are present is it the time for the fetus to exit the uterus (Baogong), and only then can one exert effort for delivery. If these symptoms are not yet complete, even if abdominal pain persists for one or two days, one must absolutely not strain or push forcefully. In cases where amniotic fluid has already discharged and the fetal head has reached the birth canal but remains undelivered after three or four days, it is due to the mother's Qi being deficient beforehand. In such instances, Renshen (Ginseng Radix) is specifically effective for those with weakness; a large dose of Bazhen Tang (Eight-Treasure Decoction) should be used to deliver two pills of Baifeng Wan (White Phoenix Pill) to tonify Yuan Qi (Primal Qi), regulate diet, and allow natural delivery when the time is ripe.
When getting into bed postpartum, the pillow should be high and there should be back support; do not lie flat. Commentary: After delivery, one should sit upright and lean for about two Shichen (four hours) before sleeping with a high pillow, which can prevent Blood Dizziness (Xueyun). The knees should be bent upward, and a box may be placed under the heels to prevent the legs from extending straight. Immediately drink a cup of hot Tongbian (Child Urine). One should keep the mouth closed and rest quietly. An extra person should attend closely, speaking gently to the parturient occasionally, ensuring she does not fall into deep sleep. Extreme fatigue followed by deep sleep may cause Qi and Blood to congest upwards, leading to dizziness. Loud shouting should be avoided to prevent fright. The walls around should be shielded from wind. If there are no other ailments, only Huasheng Tang (Transformation of Stasis Decoction) should be taken; no other medicines are necessary.
Postpartum, an iron scale weight, or nails/stones heated red-hot and quenched in vinegar, should be used so that the vinegar vapor enters the nose. This prevents Blood Dizziness, helps to consolidate Shen (Spirit), and resolves foul odors. Apply this method three to four times daily for three days, then stop.
Within seven days postpartum, if Huasheng Tang has not been taken and uterine lochia pain (Kuai Tong) persists, Huasheng Tang should be supplemented to clear the source. If within three days, two or three doses of Huasheng Tang have been taken but Kuai Tong remains, additional doses may still be administered until the pain subsides naturally.
Within seven days postpartum, Ren Shen (Ginseng Radix), Baizhu (Atractylodis Macrocephalae Rhizoma), and Huangqi (Astragali Radix) should not be used lightly, lest they cause lochia stasis pain to persist. However, if there is Qi and Blood collapse, profuse sweating, syncope, panting, or a faint pulse, these are critical conditions requiring emergency intervention. In such cases, one must adapt and use larger amounts of Ren Shen and Fuzi (Aconiti Lateralis Radix Praeparata) combined with Huasheng Tang to rescue the patient. If the condition improves after medication, reduce the dosage of Ren Shen and Fuzi, continuing only with Huasheng Tang to resolve stasis. Once Kuai Tong ceases, resume using Ren Shen, Baizhu, and Huangqi. Keep the abdomen warm by heating small garments and applying them to the belly; even in summer, a single blanket should cover the lower body. In winter, use a stove to heat the bedding thoroughly; otherwise, cold abdomen will lead to blood stasis and pain.
For seven days postpartum, do not wash the perineum, do not comb hair, do not engage in strenuous labor, do not rise early to avoid wind-cold invasion, and do not walk to avoid disturbing sinews and bones. After the eighth day, warm water may be used for sitting baths of the perineum, but care must be taken to prevent wind from entering the vaginal orifice.
Within one month, avoid excessive talking and labor. Female workers should not use cold water to wash hands and feet; even warm water should be used sparingly. Avoid fright and anger. During the double-full-month period (two months), abstain from sexual intercourse, desire/fire, and excessive mental labor. Violating these prohibitions renders treatment ineffective. If abstinence is maintained for 120 days, illness will be rare throughout life. Dietary restrictions are even more critical than during pregnancy. Avoid heavy, turbid foods because the Spleen and Stomach are greatly deficient postpartum and cannot digest them easily, risking blockage of meridians and stagnation of Qi and Blood. Avoid overly hot foods to prevent disturbance of new blood. Avoid cold foods to prevent blood stasis. Avoid digestive and dispersing agents that damage True Qi. Avoid excessive alcohol and salt intake, which can dry up breast milk. Regarding the gender of the baby, it is determined by the husband's destiny and irrelevant to the woman; one should offer comfort and never sigh nearby, causing the mother distress and potential harm. Furthermore, cruel individuals who drown female infants act against humanity and reason, and will surely face severe retribution.
