Depending on the severity, acupuncture techniques must not be scattered or indiscriminate. One should promptly employ methods to regulate Qi and disperse wind, dry dampness and relax sinews, strengthen the Spleen and nourish Blood. When Qi and Blood are harmonized, Wind-Dampness will be eliminated.
If a patient considers their initial mild symptoms insignificant and neglects them, while the physician fails to recognize the violent and fierce nature of the disease, leading to careless administration of tonifying or purgative treatments, the toxic pathogenic factors of Wind-Dampness will flow throughout the body. This results in everted eyelids and lips, loss of eyebrows and hair, flaccid paralysis of the limbs, detachment or fracture of the ten fingers, and ulceration of muscles. At this stage, even if Huangdi (Yellow Emperor) and Qibo were reborn, or Luque and Bianque returned to life, what medical skill could they possibly apply? Is this not heartbreaking!
When the disease manifests at age fourteen or fifteen, the Primordial Yang (Yuan Yang) is not yet fully developed, and Tian Gui (the substance derived from Kidney essence that promotes sexual maturity) has not arrived. How can the Kidney Yuan be injured? This is not a condition of adulthood but stems from congenital insufficiency, with both Qi and Blood being deficient. The skin and interstices (Cou Li) are loose and sparse. Parents, unaware of the root cause, fail in proper care and nurturing, allowing Cold-Damp pathogens to invade the sinews and bones, compounded by the attack of Deficient Wind, forming the disease. Its rapid onset and easy spread cannot be compared to adult-onset cases; out of every hundred patients, only four or five may survive. Why? Because their sinews and bones are not yet firm, and their muscles are tender and fragile, making them susceptible to toxins.
When the disease manifests at age four or five, it is even more severe than the above. Generally, humans inherit Father's Essence and Mother's Blood to form a point of True Yuan (Primordial Origin). Then two Kidneys are formed; from Kidneys comes Liver, from Liver comes Heart, from Heart comes Spleen, from Spleen comes Lung. Once the Five Zang organs are complete, the Six Fu organs follow; once the skeletal framework is ready, skin and flesh grow. Only when the physical form is complete does reproduction become possible. This condition arises because before conception, the parents already had Dampness pathology; how could the conceived fetus be free of this disease? After birth, day by day and month by month, the toxin spreads throughout the body. Upon contracting Wind pathogens, Dampness transforms into Heat upon encountering Wind. This steams and burns the blood vessels and muscles, causing the disease to erupt. Thus, it spreads immediately upon onset, quickly becoming an incurable degenerative state. Out of every hundred, none can be saved. All is due to parental causes. However, previous discussions on Leprosy (Ma Feng) only mentioned Wind, omitting Dampness, failing to clarify the root origin of the disease; thus, these are not definitive theories.
Wind corresponds to Wood and the Liver; the Liver governs the sinews. Without Dampness accompanying it, severe cases lead to paralysis or hemiplegia, while mild cases cause sinus spasms, numbness, contracture, bi-syndrome, and insensitivity. In Leprosy, however, Wind and Dampness knot together in the bone marrow, transforming into Heat over time, with the momentum increasing daily. Wind drives this process, and Damp-Heat obstructs between the muscles and the layer of skin and hair. Initially, there is numbness, which is a Wind pathology. Later, skin perforates, flesh rots, sinews break, and bones fracture. If not for the harm caused by Dampness, how could it reach such a state? Furthermore, prolonged Dampness generates parasites (worms), which consume internal Zang organs and external muscles, damaging sinews and bones. Over time, it spreads extensively without limit.
However, toxicity manifesting in the Five Zang organs has observable signs:
- Toxicity excess in the Heart: Mood swings (excessive joy or anger), reddish complexion, slurred speech, growth of polyps in the eyes, frequent vesicles, and irregular pain, itching, or numbness.
- Toxicity excess in the Liver: Irritability, bluish-green complexion, dry throat, frequent resentment, upward gaze, small vesicles on the face with edema.
- Toxicity excess in the Lungs: Profuse sweating, aversion to wind, frequent coughing, shortness of breath, generalized itching, quiet during the day but worse at night, and eventually loss of eyebrows and hair.
- Toxicity excess in the Spleen: Lethargy, inability to control the limbs, yellowish and weak complexion, red tinea corporis all over the body, or urticaria.
- Toxicity excess in the Kidneys: Edema of the face and eyes, lower back and spine pain, inability to stand upright, pale and dull complexion, with perforation starting on the soles of the feet.
There is a theory of "Five Deaths and Five Incurables" regarding Leprosy:
1. Skin Death: Numbness and insensitivity.
2. Flesh Death: Insensitivity to cutting.
3. Blood Death: Ulceration of muscles.
4. Sinew Death: Shedding of fingernails.
5. Bone Death: Collapse of the nasal bridge, severed eyelids, and everted lips.
Once these five death signs appear, it is said to be difficult to treat.
The five incurable conditions are:
1. Inability to observe dietary and behavioral taboos.
2. Failure to abstain from alcohol and sexual indulgence.
3. Intolerance of temperament and excessive anger.
4. Excessive worry and overthinking.
5. Obstruction of urination and defecation.
It is also said: Male patients with onset on the left foot are difficult to treat; female patients with onset on the right foot are difficult to treat. These are empirical lessons for practitioners and must not be ignored. (From *Medical Essentials*)

**(Heart Meridian)**
**Big Ma Feng (Leprosy):** Caused by Kidney deficiency leading to Dampness retention, and Liver deficiency stirring Wind. Wind and Dampness interact, obstructing the blood vessels, muscles, skin, and interstices. Initially, one feels numbness in a spot on the head, face, hands, or feet, with no sensation of pain or itch. If not treated promptly, it gradually spreads over the whole body. Long-term effects include loss of eyebrows, everted lips, detachment of ten fingers, and muscle ulceration, rendering it untreatable. (Suitable for: Ma Feng Wan, Hui Sheng Wan, Hai Zhen Dan, Bai Hua She Wan, Qing Shen Yu Feng Tang.)
**Snake Skin Feng (Involved Kidney Meridian):** Begins between the buttocks and thighs. The skin cracks open, resembling the pattern of a snake's belly. It may be painful or itchy, spreading rapidly over the body. If not treated urgently, it is incurable. Suitable for: Huo Long Dan combined with major tonification. Avoid puncturing, needling, or fumigation; violation leads to immediate death. (Suitable for: Huo Long Dan, Li Feng Dan, Qi Xiao Wan.)
**Tuo Gen Feng (Common in Kidney Meridian):** Initially appears on the heels and under the groin as blisters. These may rupture, form small sores, or develop into swollen calluses. They may be painful or itchy. Over time, they perforate and rot, extending to the soles of the feet. Commonly known as "Straw Sandal Feng" or "Fish Cheek Feng," belonging to the same category. (Suitable for: Qi Xiao Wan, Huo Long Dan, Da Xiao Feng San.)
**Fish Scale Feng (Involved Lung Meridian):** Initially presents as dry, white patches all over the body with floating itch and numbness. Small sores gradually form, turning into plum-blossom-like scales, resembling skin scraped off a fish. Scratching causes pain. Yellow fluid may exude. The patient fears both cold and heat. (Suitable for: Yi Li Jin Dan, Li Feng Dan, Huo Long Dan, Da Xiao Feng San.)
**Xie Mei Feng:** Initially manifests as sudden weeping, drunken-like rambling, fear mixed with laughter or silence, frequent nightmares, or hallucinations of intercourse with spirits. Alternating chills and fever, chest fullness, anorexia due to smell sensitivity. Ancient texts call this "Qi Zhu." (Suitable for: Zhen Bao San Sheng Dan, Shu Feng San.)
**Blood Feng (Involved Liver, Spleen, and Kidney Meridians):** Etiology includes sexual activity after heavy drinking or eating, excessive lust, fighting, bathing naked in water, angry eating, or enduring hunger while exerting force. Wind-Dampness invades internally. Over time, evil toxins attack. Initially, muscles swell and turn red all over the body, resembling beatings. Blood blisters appear everywhere. If pathogens affect Yang meridians, symptoms worsen in the morning and improve in the evening; if affecting Yin meridians, they worsen in the evening and improve in the morning. Hematemesis, epistaxis, vomiting blood, hiccups, belching, acid regurgitation, mood swings, red patches, numbness, ulceration of swollen areas with continuous fluid leakage, hematochezia, facial edema, headache feeling like splitting, and limb contracture/paralysis occur. All result from Wind-Dampness accumulating and transforming into Heat. Wind exacerbates Damp-Heat, and Damp-Heat empowers Wind. They attack and overflow everywhere. Attack on Heart/Lungs causes bleeding; attack on muscles/skin causes swelling, numbness, perforation, and ulceration, leading to incurable states. Can one not pity this? At the first sign, urgently use Shu Feng San and Zheng Yang Dan for treatment to ensure survival.
**(Liver Meridian)**
**Goose Palm Feng (Involved Lung Meridian):** Begins on the palms and ankle joints. Purple or white tinea develops, causing numbness, itchiness, and thickened skin. Scratching produces white flakes. Pain and itch vary. May appear on the dorsum of the foot, called "Shoe Strap Feng." Over time, it perforates and spreads. (Suitable for: Da Xiao Feng San, Er Ba Ji Yang Dan, Xiao Zao Dan, Hei Guang Tang.)
**Drumstick Feng (Kidney Meridian involving Liver):** Initially, soreness and pain behind the knees, aversion to cold, dampness, and wind. Difficulty walking. Commonly known as "Cold-Damp Hong Kong Foot." Over time, calves weaken, joint pain intensifies, muscle mass decreases, knees and ankles enlarge, toes become sore and numb. Shape resembles a drumstick. (Suitable for: Shen Xian Huan Gu Dan, Zao Ling Dan, Du Sheng San, Bathing Prescription.)
**Blood Bi Feng (Primarily Kidney):** Due to bodily weakness and excessive labor, Cou Li are loose, easily inviting Wind-Dampness. Pathogens enter the Blood level. Accumulating over time, they transform into Heat and generate worms that consume muscles. Initially, fatigue and sweating occur. Sleep is restless. Body shakes as if blown by wind. Over time, purple patches appear. Extreme pain turns to itch, extreme itch to pain. Eventually, nasal collapse, contracture, and limping lead to death. (Suitable for: Zheng Yang Dan, Ru Xiang Sou Feng Wan.)
**Gao Feng (Involved Spleen and Kidney Meridians):** Caused by excessive alcohol and sex, failure to avoid Wind-Dampness. The Three Yin Meridians of the Foot are affected. Qi and Blood stagnate. Over time, Heat and toxins form. Intense Heat surges and overflows. Initially appears on the brow ridge or cheeks as nodules. Later becomes ulcers with foul, fishy odor. Pus flows and adheres, causing further ulceration. Head and feet become distorted, eyes sink, nose collapses, fingers break, lips evert, leading to death. Early treatment with Ru Xiang Sou Feng Wan, Ku Shen Wan, Da Xiao Feng San, along with major Qi and Blood tonification, and medicinal baths, can ensure safety.
**Tong Feng:** Initially, spasmodic pain in limb joints, unable to flex or extend day or night. Difficulty turning over in bed. Eventually leads to paralysis and inability to rise. (Suitable for: Ru Xiang Sou Feng Wan, Da Ding Feng Wan, Ma Huang Yin, Yu Feng Tang, Xiong Zhu San.)
**Dian Feng (Involved Spleen and Kidney Meridians):** Anger injures the Liver; desire injures the Kidneys; irregular diet injures the Spleen. Excessive anger stirs the Liver, generating Wind. Excessive desire depletes Kidney Yang, leading to Yin-Cold excess. Irregular diet damages Spleen transport, causing Dampness accumulation. Without prompt treatment, Qi and Blood become disordered, Cou Li become insecure, and Six Excess pathogens invade and combine with the primary disease, forming a malignant illness. Initially, alternating chills and fever, vomiting, coughing, swelling of skin and flesh. If pathogens are not expelled and Righteous Qi supported, it leads to madness, shouting, sinus contracture, hand tremors, limb traction, singing, dancing, laughing, scolding, hair loss, poor complexion, and various complications leading to death. At the first sign, suitable for: Duo Ming Hai Zhen Dan, San Sheng Dan, Si Mo Dan, Qu Xie An Shen Wan.
**(Spleen Meridian)**
**Ban Zhi Feng (Half-Limb Feng) (Involved Kidney Meridian):** Due to congenital insufficiency and Cold-Damp invasion, or sexual activity after heavy drinking, allowing Wine-Damp-Heat to enter sinews and bones via deficiency. External Wind contraction occurs. Wood (Liver) becomes excessive while Earth (Spleen) is deficient, failing to generate Metal (Lung). Lung and Kidney lack roots. Evil pathogens dominate. This forms Pian Feng (Hemiplegia), affecting either left or right side. Joint spasms, unbearable pain, inability to flex/extend limbs. Ying Blood stagnates, unable to flow through collaterals. Sinews contract while collaterals slacken, leading to paralysis. Crying out in pain day and night until death. Also called Pian Feng or Wei Bi; the syndrome is the same. (Suitable for: Ru Xiang Sou Feng Wan, Bao Zhen Dan, Zheng Yang Dan, Xiong Zhu San, Qu Xie An Shen Wan, Jiu Ku Zai Sheng Dan.)
**Ruan Tan Feng (Soft Paralysis Feng):** Due to dual deficiency of Qi and Blood, loose muscles, and insecure Cou Li. Wind pathogens invade bones via deficiency. Over time, Heat forms, obstructing collaterals. Qi and Blood cannot circulate. Sinews lack Blood nourishment. Limbs ache day and night, limbs are weak, contracture prevents extension, laxity prevents flexion. Body soft as cotton. Joints make sounds when moving. Leads to paralysis. (Suitable for: Sou Feng Shun Qi Wan, Bao Zhen Dan, Zheng Yang Dan, Shen Xian Huan Gu Dan, Xiong Zhu San.)
**Zi Yun Feng (Purple Cloud Feng):** Constitutionally deficient in Qi and Blood with excess Fire. Movement causes profuse sweating. Qi deficiency makes Cou Li loose, allowing moisture from clothes/bedding to invade skin. Wind pathogens contract subsequently. Wind and Dampness struggle, stagnating in the skin. Over time, worms generate, causing purple-black spots spreading over the body, resembling clouds or sunset glow. Not tinea nor scabies. Intermittent pain and itch. Over time, high swelling, gradual numbness, leading to death in 40-50% of cases. (Suitable for: Qing Feng Yin, Bathing Prescription.)
**Gan Feng (Dry Feng) (Involved Liver and Kidney Meridians):** Lung Qi deficiency invites Wind. Metal deficiency fails to generate Water. Kidney source dries up, causing skin dryness. Wind lingers in skin, transforming into Heat and Dampness. No specific painful areas. Gradual appearance of black and white spots. Heat in palms, soles, and bottoms of feet like branding. Generalized itching. Incurable with standard treatment. (Suitable for: Fu Yuan Dan, medicinal baths, avoiding wind to induce sweating.)
**Ci Feng (Thorn Feng):** Ying deficiency with excess Heat. Heat makes Cou Li porous, facilitating sweating. Wind enters via deficiency. Excessive sweating further depletes Blood. Wind's dynamic nature causes Heat to surge horizontally through collaterals. When Righteous Qi struggles against pathogens, Qi reverses and stagnates, causing stabbing pain like knife or awl pricks. Pain moves around as pathogens and Righteous Qi separate. Like bandits in late Ming Dynasty: looting rich areas, then disappearing when troops arrive. Burning, aching pain in joints. (Suitable for: Yi Li Jin Dan, Xiao Zao Dan.)
**Yang Feng (Itch Feng) (Involved Lung Meridian):** Wei Qi is inherently deficient, Cou Li insecure. Wind enters easily, floating in the skin. Whole body incessant dry itch. (Suitable for: Zheng Yang Dan, Da Xiao Feng San.)
**(Lung Meridian)**
**Bai Dian Feng (White Spot Feng) (Involved Spleen Meridian):** Spleen accumulates Heat, failing to generate Metal. Lung deficiency invites Wind, drying fluids. Blood relies on Spleen to move. Spleen trapped by evil Heat cannot control Blood. Lung invaded by Wind, obstruction in skin/hair. Qi and Blood disharmonious, circulation irregular. Areas obstructed by Wind gradually turn white. Limbs are root of Spleen; skin/hair match Lung. Hence, mostly starts on hands and feet. (Suitable for: Zao Ling Dan, Shen Xiao Zhui Feng Wan, Yu Shu Dan.)
**Mao Feng (Hair Feng) (Involved Heart and Kidney Meridians):** Entering water while sweating, or Kidney deficiency stepping on frost/ice, inviting Cold-Damp. Over time, transforms into Fire and Toxin. Intense attack stirs Heart Fire rising to overcome Lung. Damp-Heat toxin flows into collaterals. Occasional scratching causes extreme pain, like being stung by hairs. Spreads over body, movement feels like knife/needle piercing. Unbearable pain. Once spread, incurable. (Suitable for: Da Xiao Feng San, Ru Xiang Sou Feng Wan, Da Ding Feng Wan, Li Feng Dan, Jiu Ku Hui Sheng Dan, Qu Xie An Shen Wan.)
**Li Jie Feng (Joint Feng) (Involved Liver and Kidney Meridians):** Indulgence injures Kidneys; rage transmits to Liver. Failure to avoid Dampness below and Wind outside allows Wind-Damp to invade sinews/bones via deficiency. Unclear treatment leads to accumulation. Over time, transforms into Heat and Toxin, hindering Ying-Wei circulation. Soreness, numbness, spasms in elbows, knees, shoulders. Sinew contracture, enlarged joints. Difficulty moving limbs. Leads to paralysis. (Suitable for: Qu Xie An Shen Wan, Jiu Ku Hui Sheng Dan, Da Ding Feng Wan.)
**Bi Ni Feng (Wall Mud Feng) (Involved Liver and Spleen Meridians):** Liver Wind excessive, overcoming Spleen Earth. Preference for raw/cold/damp-producing foods damages Spleen transport. Dampness stagnates internally, transforming into Heat over time. Wind and Damp-Heat toxin spread through muscles. Qi and Blood stagnation means muscles do not receive nourishment. Skin turns gray-white, like old wall mud. Heavy Damp, light Wind, so no pain/itch. If spreading over whole body, incurable. (Suitable for: Ru Xiang Sou Feng Wan, Xiong Zhu San.)
**Zhen Feng (Rash Feng) (Involved Heart and Spleen Meridians):** Spleen accumulates Dampness, Earth declines, failing to generate Metal. Lung deficiency invites Wind. Accumulation transforms into Damp-Heat toxin, steaming muscles. Worms generate. Initially, rashes like millet beans or scabies. Itchy but not painful, or painful but not itchy. Over time, coalescing ulcers, incurable. (Suitable for: Xiao Zao Dan, Yu Shu Dan.)
**Ya Feng (Aphonia Feng) (Involved Liver and Spleen Meridians):** Anger injures Liver; excess Qi becomes Fire. Wood moves, generating Wind. Spleen Earth injured by Wood, struggling to transport, retaining Dampness. Spleen decline fails to send essence to Lung; Lung also declines. Liver Wood evil, taking advantage of declining organs, carries Spleen Dampness to attack Lung. Lung governs voice; Metal hollow resonates. Now obstructed by Liver/Spleen Damp-Heat, orifices blocked. Initially hoarse, later aphonic. (Suitable for: Bao Zhen Dan, Duo Ming Hai Zhen Dan.)
**(Kidney Meridian)**
**Leng Feng (Cold Feng) (Involved Spleen Meridian):** Kidney deficiency invites Wind-Dampness. Kidney governs bones; deficiency means marrow insufficient. Wind-Damp invades joints via deficiency. Pathogens intensify over time, overflowing into muscles. Initially numbness, sometimes cold pain, joint soreness. In winter, bedridden. Over time, Qi/Blood stagnation, blistering, perforation, death. (Suitable for: Qi Xiao Dan, Zheng Yang Dan, Hai Zhen Dan, Ru Xiang Sou Feng Wan, Leng Feng Fang.)
**Lou Ti Feng (Leaking Hoof Feng):** Excessive sexual desire exhausts Kidney Meridian. Failure to avoid frost/snow exposes soles to Cold-Damp. Accumulation transforms into Heat and Wind. Qi/Blood stagnation. Initially numbness/itch at Yongquan (KI1) acupoint. Gradually forms water pockets. Perforation releases yellow fluid. Spreads over body, generating decay and foul odor. Incurable. (Suitable for: Qi Xiao Dan, Huo Long Dan, Ru Xiang Sou Feng Wan.)
**Ha Ma Feng (Toad Feng) (Involved Liver Meridian):** Lung deficiency invites Wind-Dampness. Accumulates in skin, lingering long, transforming into Heat and Toxin. Ying-Wei stagnation. Body develops lumps resembling toads. Rupture causes ulcers, pus, blood, foul odor. (Suitable for: Hai Zhen Dan, Zheng Yang Dan, Shen Xiao Zhui Feng Wan.)
**Tao He Feng (Walnut Feng) (Involved Spleen Meridian):** Spleen has chronic Damp-Phlegm. Earth declines, failing to generate Metal. Qi deficiency makes exterior insecure, inviting Wind. Wind and Phlegm interact. Chronic Damp-Phlegm scatters in muscles. Initially, lumps all over body. Large ones like chestnuts, linked in groups of 3-5. Small ones like beans, in rows. Color/shape resembles walnuts. (Suitable for: Huo Long Dan, Zao Ling Dan.)
Another type: Contraction of fingers, or fine black small patches between ankles, thighs, abdomen, ribs, neck, cheeks, or head. Erupts during rainy weather, disappears in clear weather. Or erupts during Yin periods, disappears during Yang periods. Or bright color in clear weather, turns purple-black and painful in rain. Named **Du Chi Chuang**. Due to Wind-Damp stagnating long, entering Zang-Fu. Must dispel Wind, nourish Blood, regulate Qi, clear Yang, dry Dampness. If not treated urgently, becomes Big Feng, difficult to cure.
**Re Feng (Heat Feng) (Involved Lung and Spleen Meridians):** Yin deficiency invites Dampness. Lung invites Wind. Wind and Damp stir Heat. Pathogens surge upward, obstructing Lung orifices. Constant flow of foul, yellow, turbid nasal discharge. Coughing up thick saliva. Eyes belong to Yang; now due to Qi deficiency, pathogens obstruct Yang pathways...
