Shen Douyuan stated: Bi Ding (Nasal Carbuncle) is formed due to excessive melancholy and severe physical exhaustion, causing toxic qi from the Zang-Fu organs to transmit into the meridians. Within two or three days, the patient experiences mental fatigue and soreness in the bones and muscles; by four or five days, alternating chills and fever occur as the toxic qi attacks the Heart, leading to swelling of the head and face; by eight or nine days, symptoms include vomiting, coma, rising phlegm, and rapid breathing, where only one in ten patients can be saved. Physicians must not fail to examine this carefully. For initial treatment, one should apply the methods from the Ding Chuang (Carbuncle and Ulcer) section. (From *Qi Xuan*)
Chen Shigong stated: Bi Ding arises inside the nasal cavity, with pain radiating to the Naohu point, causing difficulty in breath regulation and nasal congestion. In severe cases, the lips and cheeks swell. Initially, acupuncture should be performed, followed by treatment according to the methods for Ding Chuang. (From *Zheng Zong*)
Gu Shicheng stated: Bi Ding occurs within the nasal aperture, resulting from melancholy injuring the Lung, or kidney transmission due to sexual excess, where Fire invades the Metal position, causing burning and scorching. Treatment should strictly follow the methods for Ding Chuang, without need for a separate principal prescription.
Master Qi stated: Nasal swelling is caused by excessive Fire in the Lung Meridian. Blowing powder made from Gleditsia Sinensis Fruit (Zao Jiao) into the nose induces dozens of clear sneezes, which leads to recovery. Since the nose is blocked by Qi stagnation, inducing sneezes disperses the obstructed Qi entirely; thus, there is no need to use Gan Jie Tang (Licorice and Platycodon Decoction) to clear the Lung, as the nasal swelling will subside naturally. (From *Mi Lu*)
He also stated: There are cases where the nose swells to the size of a fist with excruciating pain, caused by Fire in the Lung Meridian and heat stagnation in the nose that cannot be vented. The method should be to clear the pathogenic factors from the Lung and remove the Fire from the nose. The primary formula is Jie Yong Tang (Resolve Stagnation Decoction). This formula works because all its ingredients enter the Lung Meridian to eliminate Fire pathogens, ensuring the resolution of stagnation and swelling.
Gu Shicheng stated: The nose is the orifice of the Lung. If there is accumulated Heat in the Lung, or if stomach Heat steams the Lung due to consuming strong wine or roasted foods, or if Lung Fire is extremely hyperactive, sores may arise in the nasal aperture. These present with dryness, cracking, pain, and often red papules. One should use Shihu Maidong Tang (Dendrobium and Ophiopogon Decoction) to clear Heat, or Liuwei Dihuang Tang (Six-Ingredient Rehmannia Pill) added with Maidong (Ophiopogon Tuber).
Chen Shigong stated: Bi Chuang (Nasal Ulcer) is generated by Excess Fire and stagnant Heat in the Lung Meridian. (From *Zheng Zong*)
