[Symptoms of Restlessness and Insomnia due to Deficiency] The exterior syndrome has completely resolved. Although there is thirst, the patient cannot consume water (or drinking does not relieve thirst). Urination and defecation are clear and unobstructed. The spirit is listless and timid. The patient frequently desires sleep but is often suddenly awakened from it. This constitutes the syndrome of restlessness and insomnia due to deficiency.

[Etiology of Restlessness and Insomnia due to Deficiency] It may result from excessive sweating, leading to the depletion of body fluids; or from erroneous purgation damaging the interior and injuring the Middle Qi (Spleen and Stomach energy); or from the reckless use of emetic methods, severely damaging the ethereal Qi of the Upper Jiao (Upper Burner). All these factors can lead to restlessness and insomnia due to deficiency.

[Pulse Diagnosis for Restlessness and Insomnia due to Deficiency] The pulse is mostly deficient and soft, or appears deficient and choppy. If the pulse is empty and large, it indicates extreme weakness of the Middle Qi. If the pulse is fine and rapid, it signifies that essence and blood have been exhausted.

[Treatment for Restfulness and Insomnia due to Deficiency] For an empty and large pulse, use Buzhong Yiqi Tang (Tonify the Middle and Augment the Qi Decoction) with added Huangbai (Phellodendron Bark) and Zhimu (Anemarrhena Rhizome). For a fine and rapid pulse, use Shengmai San (Generate the Pulse Powder) combined with Liangtiandi Jian (Cool Heaven and Earth Decoction). For cases of True Yang insufficiency where the Spirit lacks its abode, use Zaoren Yuanzhi Tang (Sour Jujube Seed and Polygala Decoction); in severe cases, use Bawei Shenqi Wan (Eight-Ingredient Kidney Qi Pill).

[Symptoms of Restlessness and Insomnia due to Liver Fire] The hypochondrium feels distended at times. Nighttime sleep is frequently disturbed by fright. There is thirst with excessive drinking. The abdomen is distended and large as if pregnant. Pulling pain occurs in the lower abdomen and costal margin, radiating to the genitalia. This is restlessness and insomnia due to Liver Fire.

[Etiology of Restlessness and Insomnia due to Liver Fire] It may be caused by anger injuring the Liver, causing Liver Qi stagnation; or by excessive mental exertion and planning, injuring Liver Blood. The Liver stores blood. If Yang fire disturbs the Blood Chamber (Uterus/Blood Reservoir), nighttime sleep becomes restless.

[Pulse Diagnosis for Restlessness and Insomnia due to Liver Fire] The Left Guan position pulse is exclusively large, or appears wiry and rapid, or wiry and slippery. The Cun and Guan positions are洪大 (full and large), indicating Wood and Fire illuminating each other. The Cun and Guan positions are deep and rapid, indicating dry Wood generating Fire. The Guan position is larger than the Chi position, indicating Dragon-Lei Fire rising.

[Treatment for Restlessness and Insomnia due to Liver Fire] For Liver injury due to anger with rebellious Liver Fire, use Shugan San (Soothe the Liver Powder). For Liver injury due to mental exertion, use Siwu Tang (Four Substances Decoction) with added Zhizi (Gardeniae Fructus) and Chuanlian (Coptidis Rhizoma). For dry Wood generating Fire, use Longdan Xiegan Tang (Gentian Liver-Draining Decoction). For a large Left Chi pulse, use Jiemi Ganshen Wan (Secret Formula Liver-Kidney Pill).
