Sha poison rushes to the heart, causing dizziness and stifling sensation, leading to collapse. The presentation resembles heatstroke or wind-stroke (Zhong Feng), with loss of consciousness and sudden death; this is an acute and severe case of Sha syndrome. If there is a slight sign of recovery, lift the patient and perform bloodletting (Fang Sha); if not cured, carefully examine the pulse and differentiate symptoms before administering medication. If dizziness occurs without regaining consciousness and the patient cannot be lifted, it is essential to accurately diagnose the pulse and differentiate symptoms to determine the cause. First, administer several doses of medicine via gavage to restore consciousness, then lift the patient for bloodletting, followed by gradual regulation and treatment.
Case Record 1:
The wife of Mr. Wang Junmei fell ill in June with dizziness and coma. Upon consultation, her Cun pulses on both sides were hollow (Kou) and scattered, while other pulses were normal. However, upon deep pressure, intermittent pauses (Xie Zhi) were occasionally observed. This was Sha syndrome caused by summer-heat, turbid qi, and foul pathogens invading the Heart Meridian. The patient could not be supported upright. Initially, Baohua San (Precious Flower Powder) was administered cold with Bohe Tang (Mint Decoction) and Huoxiang Tang (Agastache Decoction). After slightly regaining consciousness, she was supported upright, and three needles were used to prick and release toxic blood. The condition did not improve. Chenxiangawei Wan (Aucklandia and Asafoetida Pill) was then given with slightly chilled Mint Decoction, leading to gradual stabilization. Subsequently, Siwu Tang (Four Substance Decoction) was used for regulation, resulting in recovery.
Sudden coma without awakening, or incessant phlegm-wheezing, with eyes rolling upward, resembling the pediatric condition known as "Luo Gong" (falling bow). This is hidden Sha syndrome, which is difficult to identify. It requires careful pulse examination and symptom differentiation to confirm Sha poison. Observe the body's temperature (coolness or heat) and the moisture/dryness of the lips and tongue before treatment.
Case Record 2:
The son of Sheng Honglie presented in September with fever, thirst, stupor, and inability to wake up, along with upward deviation of the eyes. Upon consultation, all six pulses were faint, fine, and concealed (Fu). Baohua San plus Sharen Tang (Amomum Decoction) was administered cold, restoring consciousness. Twelve needle pricks were performed for bloodletting after supporting the patient, releasing dark purple-black toxic blood. Jiuku Dan (Pain-Relieving Pellet) and Xixin Dahuang Wan (Asarum and Rhubarb Pill) plus Sharen Tang were administered at a slightly cool temperature. Finally, Fangfeng San Sha Tang (Saposhnikovia Detoxifying Sha Decoction) added with Jinyinhua (Lonicerae Japonicae Flos), Danshen (Salviae Miltiorrhizae Radix et Rhizoma), Shanzha (Crataegi Fructus), and Buluobozi (Raphani Semen) led to recovery.
Chen Gongyu presented in August with frequent low-grade fever and mild oral thirst. After drinking boiling tea water, he suddenly felt heavy and fell into a coma. Upon consultation, the Chi pulse on the left side was deep and fine, with irregular rhythm. The Cun pulse on the right side was floating and hollow. This indicated Kidney deficiency complicated by Sha invasion. Sha of the Kidney Water pathogen reversely interfering with the Mother organ, Lung Metal, causes excessive phlegm and qi stagnation leading to dizziness. Duohuo Honghua Tang (Angelica Pubescens and Carthamus Decoction) was decocted with Beimu (Fritillariae Bulbus) and Niuxi (Achyranthis Bidentatae Radix), mixed with Tongbian (Child Urine) for ingestion. Yuanhong San (Round Red Powder) was advanced to restore consciousness. Then, bloodletting was performed after supporting the patient. Two days later, after the Sha qi cleared, Buzhong Yiqi Tang (Middle-Warding Qi-Supplementing Decoction) and Liuwei Dihuang Wan (Six-Ingredient Rehmannia Pill) were used for regulation, resulting in recovery.
