The fetus resides in the uterus, consuming the mother's blood. The residual portion coagulates into a mass. During delivery, if this mass descends with the infant, there is no harm; if it does not descend, it stagnates and causes pain, known as "Infant Pillow Pain" (Erzhen Tong). Heishen San (Attack 10) is indicated.
If the condition persists for a long time without resolution, the blood transforms into pus. Additionally, after childbirth, cold pathogenic qi may enter the abdomen, causing hardness and a sensation of being struck, along with stabbing pain below the umbilicus, which is termed Cold Hernia (Hanshan). Fried Evodiae Fructus (Wuzhuyu) should be used; do not treat incorrectly.
If blood transforms into pus and flows into the joints, it forms Bone Abscess (Gusu). A surging and rapid pulse is its sign. If neglected, it often becomes a critical condition (Bai Zheng) and requires urgent treatment. If abdominal distension occurs, or if water sounds are heard when turning the body, or if pus discharges from the umbilicus or with stool, tonification should be used to expel the toxin, followed by Shenxian Huinong San (Attack 11).
After the child leaves the uterus, the Sea of Blood (Xuehai) suddenly becomes deficient, causing pain. Injury to the uterine gate (Baomen) inevitably leads to congestion and swelling. Before the swelling subsides, palpation may feel like a mass, but it is not actually a mass; it must not be treated as Infant Pillow Pain. Dianbao Jian (Tonify 42) or Wuwu Jian (Tonify 43) are indicated.
Intestinal wind (Changfeng) due to anger, depression, and blood deficiency, as well as ingestion of toxic substances from various fish, can all cause intestinal ulcers (Changyang). Salt fried hot can be applied as a compress to the umbilicus. Alternatively, an arrowhead can be heated red-hot and quenched in wine; drinking this wine can cure the condition. Take Chuanxiong Rhizoma (Chuanxiong), Angelicae Sinensis Radix (Danggui), Glycyrrhizae Radix et Rhizoma (Gancao), Achyranthis Bidentatae Radix (Huainiuxi), and Ligustici Rhizoma et Radix (Ben), grind into powder, and take with wine.
The waist is the external manifestation of the Kidney. The conception vessel is connected to the Kidney. Childbirth damages Kidney Qi and injures the uterine collaterals. When deficiency has not yet recovered, wind-cold pathogens invade, causing lower back pain. Sansheng San (Disperse 25) is indicated.
If lochia has stopped, but the lower back feels heavy and painful with radiation downward, and the thighs feel as if pierced by awls into the bone, this indicates blood stagnation in the collaterals, which will likely develop into carbuncle or abscess. Siwu Tang plus Corydalis Rhizoma (Yanhusuo) is indicated (Tonify 7).
When true Qi is deficient and pathogens take advantage, Danggui Huangqi Tang plus Taxilli Herba (Sangjisheng) is indicated (Tonify 44).
For chronic pain that does not resolve, frequent consumption of pig or sheep kidneys is recommended.
Abdominal distension with frequent belching, and vomiting upon taking various medicines, indicate Spleen deficiency. Scorched Atractylodis Macrocephalae Rhizoma (Jiaozhu) decocted in rice wash water is indicated.
If the Bladder has retained water, and lochia flows out incompletely, the interaction between Qi-blood and water causes distending pain in the hypochondrium.
If pain is accompanied by vomiting, Disheng Tang (Attack 12) is indicated. Without the pungency of Ginger and Cinnamon Twig to promote the action of the medicine, the distension would instead be exacerbated.
For Liver Channel blood stasis, Siwu Tang plus Cinnamomi Cortex (Rougui), Corydalis Rhizoma (Yanhusuo), Lygodii Spora (Puhuang), and Carthami Flos (Honghua) is indicated (Tonify 7).
For Liver Channel Qi deficiency, Sizijunzi Tang plus Angelicae Sinensis Radix (Danggui) and Paeoniae Radix Alba (Baishao) is indicated (Tonify 4).
For Liver Channel blood deficiency, Siwu Tang plus Ginseng Radix (Renshen) and Atractylodis Macrocephalae Rhizoma (Baizhu) is indicated (Tonify 7).
When Kidney Water fails to nourish Liver Wood, Liuwei Wan is indicated (Tonify 2).
When Lung Metal is excessive and overacts on Liver Wood, Xiebai San is indicated (Disperse 26).
For body heat and sweating, Shenghua Tang plus Ginseng is indicated (Harmonize 24).
For dual deficiency of Qi and Blood, Shiquan Dabu Tang is indicated (Tonify 39).
Anger injuring the Liver causes distending pain in both hypochondria, hematemesis, and alternating chills and fever. Bazhen Tang plus Bupleuri Radix (Chaihu) and Moutan Cortex (Danpi) is indicated (Tonify 5).
