Lochia fails to discharge, infiltrates the large intestine, causing severe diarrhea that is incontinent, with excretion of green, white, or black stool. Other causes include food stagnation, cold-dampness, weak Spleen and Stomach, and deficient Lung Qi, all of which can lead to diarrhea. If it is dysentery, it is also caused by lochial blood infiltrating the large intestine and accumulating as stagnation, termed "Postpartum Dysentery" (Chan Zi Li), a condition that is extremely critical. Furthermore, prolonged dysentery leading to emaciation is termed "Deficient Emaciated Dysentery" (Xu Lei Li). Dysentery caused by qi stagnation is termed "Qi Dysentery" (Qi Li). Additionally, there are cases caused by external contraction of the Six Evils (Liu Yin) and internal injury from diet. Treatment must always address underlying Qi and Blood deficiency; one must strictly avoid using standard methods for treating ordinary dysentery, lest it lead to untreatable consequences.
If diarrhea occurs while lochia remains, use Shenghua Tang (Transforming the Remains Decoction) with added Poria (Fuling). Wait until the lochial blood is completely discharged, then treat according to the specific condition. (From *He*, Vol. 24). If the constitution is too weak to sustain itself, add Ginseng (Renshen).
For undigested food in the stool (Wan Gu Bu Hua), use Shenghua Tang with added Poria (Fuling), Chebulae Pericarpium (Hezi Pi), Lotus Seed (Lianzi), and Fresh Ginger (Shengjiang); if not improved, add Ginseng (Renshen). (From *He*, Vol. 24).
Dry mouth and thirst due to deficient Stomach Qi failing to generate fluids. Use Shenghua Tang combined with Shengmai San (Generate the Pulse Powder). (From *He*, Vol. 24 & 23).
Food stagnation with frequent belching. Use Shenghua Tang with added Poria (Fuling), Medicated Leaven (Shenqu), Hawthorn Fruit (Shanzha), and Amomi Fructus (Sharen). (From *Re*, Vol. 8).
Distension in the hypochondrium and abdominal pain with migratory location. Within one month postpartum, Wind Evil invades due to deficiency. The pathogen remains in the Mingmen (Blind Membrane) and disperses into the abdomen and hypochondrium, causing paroxysmal abdominal pain, indigestion of food and water, abdominal borborygmus, and diarrhea. Suitable treatment is Tiaozhong Tang (Regulate the Middle Decoction). (From *Re*, Vol. 8).
For weak Spleen and Stomach with unresolved blood stasis, first administer Shenghua Tang with added Costus Root (Muxiang), Amomi Fructus (Sharen), and Alpiniae Oxyphyllae Fructus (Yizhiren); once the blood stasis is resolved, administer Liu Junzi Tang (Six Gentlemen Decoction) with added Myristica Seed (Rouguo), Alpiniae Oxyphyllae Fructus (Yizhiren), plus Cimicifugae Rhizoma (Shengma) and Bupleuri Radix (Chaihu) to raise the clear Yang of the Stomach.
For residual blood infiltrating the large intestine causing incontinent severe diarrhea, take Fuqi San: Burn four to five spikes of large Schizonepeta (Jingjie) in a cup until charred, ensuring no oil or fire contamination, mix with Musk (Shexiang), grind into powder, and take mixed with clear water.
For chronic Spleen diarrhea persisting for years, take Shenling Lianzi Yin (Ginseng, Poria, and Lotus Seed Drink). (From *Bu*, Vol. 57).
Red and white dysentery occurring within seven days postpartum, accompanied by frequent tenesmus, is extremely difficult to treat. One wishes to regulate Qi and move Blood to expel the dysentery pathogen, yet fears damaging the deficient Postpartum Yuan Qi; one wishes to nourish Ying and tonify Qi to strongly support postpartum weakness, yet fears exacerbating the dysentery pathogen. To achieve moving without damaging the Yuan and tonifying without aiding the pathogen, only Shenghua Tang with Blackened Ginger (Heijiang) removed, and added Poria (Fuling) and Costus Root (Muxiang), is suitable. This effectively eliminates lochia while simultaneously resolving dysentery stagnation, treating both without conflict.
Cold extremities, restlessness, and inability to lie down are extremely difficult to save. Use Ginseng (Renshen), Angelica Sinensis (Danggui), and Schizonepeta (Jingjie).
Abdominal pain due to stagnation: Ginseng (Renshen), Angelica Sinensis (Danggui), White Peony Root (Baishao), Red Yeast Rice (Hongqu), Vinegar-processed Cimicifugae Rhizoma (Shengma), Licorice (Gancao), Motherwort (Yimucao). If lochia is not yet finished, add Frankincense (Ruxiang), Myrrh (Moyao), Amomi Fructus (Sharen), and Donkey-hide Gelatin (Ejiao).
Thirst without desire for cold drinks indicates deficient Stomach Qi failing to generate fluids. Take Qiwei Baizhu San (Seven-Ingredient Atractylodes Macrocephala Powder). (From *Bu*, Vol. 56). Fever at night indicates deficient Kidney Water failing to moisten. Take Liuwei Wan (Six-Ingredient Pill). (From *Bu*, Vol. 2).
Dysentery with vomiting: Liu Junzi Tang (Six Gentlemen Decoction) with added Patchouli (Huoxiang). (From *Bu*, Vol. 1). If Stomach Fire is excessive, apply crushed living river snail mixed with Musk to the umbilicus. Subsequently use formulas to nourish Yin and clear Stomach Heat.
Metrorrhagia combined with intractable dysentery: Jia Shen Siwu Tang (Ginseng-Enhanced Four Substance Decoction). (From *Bu*, Vol. 7).
Rectal prolapse due to dual deficiency of Qi and Blood: Jia Shen Shenghua Tang (Ginseng-Enhanced Transforming the Remains Decoction). (From *Bu*, Vol. 58).
