The *Huangdi Neijing* (Yellow Emperor's Inner Canon) states: "Wind is the chief of all diseases; it penetrates easily and changes rapidly." When wind pathogen invades the human body, there are different types such as involvement of the Fu organs (六腑), involvement of the Zang organs (五脏), true stroke (Zhen Zhong Feng), and pseudo-stroke (Lei Zhong Feng). Later physicians differed in their treatment approaches, some focusing on phlegm (Tan), some on fire (Huo), some on Qi (Qi), and some on deficiency (Xu). Tracing the origin of these conditions reveals that none occur without a deficiency of Middle Qi (Spleen/Stomach Qi), which allows external pathogens to take advantage of the weakness. True stroke often occurs in the northwest regions where the wind is harsh.
If wind lodges in the meridians, it becomes severe wind (Li Feng); if it lodges in the Shu points of the Zang and Fu organs, it becomes hemiplegic wind (Pian Feng); if wind qi ascends along the Wind Mansion (Fengfu, GV16), it becomes brain wind (Nao Feng); if it combines with the Taiyang meridian from the Brain Door (Naohu, GV17), it becomes eye wind (Mu Feng); if one drinks alcohol, sweats, and is exposed to wind, it becomes leaking wind (Lou Feng); if one engages in sexual intercourse, sweats, and is exposed to wind, it becomes internal wind (Nei Feng); if wind enters the intestines and stomach, it becomes intestinal wind (Chang Feng); if wind lodges externally in the interstices (Cou Li), it becomes draining wind (Xie Feng). Although the names differ, the treatments vary.
"Pseudo-stroke" resembles stroke but lacks pain and chills/fever; instead, the limbs and joints suddenly become useless, and mental spirit and speech suddenly become abnormal. This is not caused by external wind but is transformed from Liver Wood evil (Gan Xie), which damages the Earth element (Middle Jiao/Spleen). Thus, one suddenly falls into unconsciousness, with deviation of the mouth and eyes, and upward surging of saliva and phlegm. In severe cases, five signs of organ failure appear: open mouth indicates Heart failure (Xin Jue); limp hands indicate Spleen failure (Pi Jue); closed eyes indicate Liver failure (Gan Jue); urinary incontinence indicates Kidney failure (Shen Jue); snoring sound indicates Lung failure (Fei Jue). If all five signs are present, the condition is fatal and untreatable. Other signs such as foaming at the mouth, fixed gaze, complexion like makeup (flushed red), muscle wasting, and tendon/bone pain also indicate untreatability. Unless prevention is undertaken during health, attempting to achieve results after the disease has struck means that even if survival is fortunate, the suffering will be immense.
For Qi obstruction, phlegm surging, critical coma, and loss of consciousness: Moxibustion on Baihui (GV20), Fengchi (GB20), Dazhui (GV14), Jianjing (GB21), Jianshi (PC5), Quchi (LI11), Zusanli (ST36), Jianyu (LI15), Huantiao (GB30), Jinggu (GB39).
For limb spasms and numbness, chaotic heart-mind, and prodromal signs of impending stroke, whether due to wind or Qi stagnation: Moxibustion on Hegu (LI4), Fengshi (GB31), Kunlun (BL60), Shousanli (LI10), Guanyuan (CV4), Dantian (CV12 or CV6 depending on context, here likely referring to lower Dantian area).
For sudden stroke: Moxibustion on Shenque (CV8). For any sudden stroke, this point is best. Luo Tianyi stated that for stroke, herbal medicine can only support the body, but to achieve complete recovery, moxibustion is superior. Not only does it disperse wind pathogen and unblock blood vessels, but its merit in restoring Yang and benefiting Qi is beyond description.
For Wind Epilepsy (Feng Xian): Moxibustion on Qianshencong (EX-HN18), 5 fen anterior to Qianding (GV20); from Shenting (GV24) to this point is 4 cun, moxa 3 zhuang. Moxibustion on Houshencong (EX-HN19), 1 cun posterior to Baihui (GV20), moxa 3 zhuang.
For trismus (lockjaw): Moxibustion on Jiguan (EX-HN14), located 8 fen below the ear, near the front. The *Qianjin Yifang* states that for stroke-induced lockjaw, moxa 5 zhuang on these two points for immediate cure. Another method suggests Jiache (ST6) and Chengjiang (CV24).
For Hemiplegia (Pian Feng): Treat left-sided paralysis by moxaing right-side points, and right-sided paralysis by moxaing left-side points. Points: Jianyu (LI15), Jianjing (GB21), Baihui (GV20), Kerenzhuren (ST7), Chengjiang (CV24), Dicang (ST4), Zusanli (ST36), Sanjian (LI3), Erjian (LI2), Yanglingquan (GB34), Yangfu (GB38) [for facial deviation]; Lieque (LU7), Fengshi (GB31), Quchi (LI11), Huantiao (GB30), Zusanli (ST36), Jinggu (GB39), Kunlun (BL60).
For Hand/Foot Marrow Pores (referring to points for treating flaccidity/numbness): The *Qianjin Fang* states that Hand Marrow Point is in the depression behind the styloid process of the wrist, and Foot Marrow Point is 1 cun posterior to the lateral malleolus. Both treat flaccidity syndromes, dispel wind, and hemiplegia; moxa 100 zhuang each.
For Facial Deviation (Mouth and Eye Skew): Moxibustion on Jiache (ST6), Dicang (ST4), Shuigou (GV26), Chengjiang (CV24), Tinghui (GB2), Hegu (LI4). If the mouth deviates to the right, it indicates left-sided meridian stroke with laxity; moxa the left Xiangu (ST43) 14 zhuang (2x7). If the mouth deviates to the left, it indicates right-sided meridian stroke with laxity; moxa the right Xiangu (ST43) 14 zhuang. Moxa cone size like a wheat grain.
For Aphonia (Yin Ya): Moxibustion on Tiantu (CV22), Lingdao (HT4), YinGu (KI10), Fuliou (KI7), Fenglong (ST40), RanGu (KI2).
For Upward Gaze (Dai Yan): Moxibustion on Shenting (GV24); moxa 5-7 zhuang on the 3rd and 5th thoracic vertebrae (ignite simultaneously for immediate effect).
For Paralysis: Moxibustion on Jianyu (LI15), Hegu (LI4), Quchi (LI11), Huantiao (GB30), Fengshi (GB31), Zusanli (ST36), Jinggu (GB39), Yanglingquan (GB34), Kunlun (BL60), Jianjing (GB21), Zhongzhu (TE3), Yangfu (GB38).
For Opisthotonos (Jiao Gong Fan Zhang): Moxibustion on Baihui (GV20), Shenmen (HT7), Jianshi (PC5), Pusen (BL61), Mingmen (GV4).
For Wind Bi with Numbness (Feng Bi Bu Ren): Moxibustion on Tianjing (TE10), Chize (LU5), Shaohai (HT3), Yangfu (GB38), Zhongzhu (TE3), Huantiao (GB30), Taichong (LR3).
For Stroke Prevention: Moxibustion on Fengchi (GB20), Baihui (GV20), Quchi (LI11), Hegu (LI4), Jianyu (LI15), Fengshi (GB31), Zusanli (ST36), Jinggu (GB39), Huantiao (GB30).
