(Attached: Anal Itching and Pain)
Rectal prolapse is a syndrome of Yuan Qi (Primordial Qi) sinking, caused solely by Qi deficiency failing to secure and hold. Therefore, it commonly occurs in postpartum women, those with chronic dysentery, those who exert excessive effort, the elderly with declining health, and infants with insufficient Qi and Blood. Use a large dose of Buzhong Yiqi Tang (Tonify the Middle and Augment the Qi Decoction), where Sheng Ma (Cimicifugae Rhizoma) should be stir-fried with vinegar. *Yimen* states: The Lung governs the Po Men (Anal Orifice); Lung Heat causes anal closure, while Lung Cold causes anal prolapse. One must warm and tonify Lung Qi, adding He Zi (Chebulae Fructus) and Chu Gen Pi (Buckthorn Bark) to the aforementioned formula. This is because the Lung and Large Intestine are exterior-interior related. If Spleen and Stomach deficiency-cold leads to diarrhea/dysentery with sliding prolapse, use Weiguan Jian (Stomach Gate Decoction) added with Wu Mei (Mume Fruit) and Wu Wei Zi (Schisandrae Fructus). If Spleen Qi deficiency causes sinking and prolapse, use Buzhong Yiqi Tang (Tonify the Middle and Augment the Qi Decoction). If Liver and Kidney Yin deficiency causes sinking and prolapse, use Buyin Yiqi Jian (Nourish Yin and Augment Qi Decoction). If deficiency is complicated by Fire, or there is redness, swelling, heat, and pain, use Buzhong Yiqi Tang (Tonify the Middle and Augment the Qi Decoction) added with Huang Qin (Scutellariae Radix), Huang Lian (Coptidis Rhizoma), and Huai Hua (Sophorae Flos). For postpartum rectal prolapse, use Liuwu Jian (Six-Ingredient Decoction) added with Sheng Ma (Cimicifugae Rhizoma), or Dianbao Jian (Placenta Chamber Decoction) added with Ren Shen (Ginseng). Still, wash with warm water and reduce the prolapse manually. For cases with Damp-Heat sinking down causing painful prolapse, use Chouxin Yin (Draw Down the Fuel Infusion) or Dali Qingyin (Great Purification Infusion). For intestinal wind with bleeding leading to prolapse, use Renshen Weifeng Tang (Ginseng Wind-Protecting Decoction). For prolapse caused by constipation and straining, use Renshen Guben Wan (Ginseng Root-Solidifying Pill) added with Huai Jiao (Sophorae Immaturus Fructus). For cases complicated by hemorrhoids and pain, use Siwu Tang (Four Substances Decoction) added with Huai Hua (Sophorae Flos), Huang Lian (Coptidis Rhizoma), and Sheng Ma (Cimicifugae Rhizoma). External method: decoct for washing/fumigation. For Cold patterns, fumigate and wash with Jingjie (Schizonepetae Herba) and Hu Cong (Allium Fistulosum), then apply a paste made from Fu Long Gan (Uterus Dragon's Liver/Calcined Lodestone), Die Tou Gu Hui (Ash of Turtle Head), and Bai Yao Jian (Fermented Tea Brick) mixed with oil. For Heat patterns, wash with Xiao Ming (Mirabilite) and Bai Fan (Alumen) decoction, then sprinkle Huang Bo (Phellodendri Cortex) powder and Mu Li Fen (Oyster Shell Powder). For redness, swelling, and heat/pain, remove the shell of Tian Luo (Apple Snail), add Bing Pian (Borneolum Syntheticum), dissolve in water, and apply. For pediatric rectal prolapse, apply ash from roasted turtle head. For anal itching, fumigate and wash with Xiao Ming (Mirabilite) decoction.
*Neijing* states: "What has sunk, lift it up." Xu Zhicai said: "Astringents can treat prolapse." These are the methods for treating rectal prolapse. Ancients mostly used herbs like Ren Shen (Ginseng), Huang Qi (Astragali Radix), Dang Gui (Angelica Sinensis), Bai Zhu (Atractylodes Macrocephala), Chuan Xiong (Ligusticum), Gan Cao (Licorice), and Sheng Ma (Cimicifugae Rhizoma) to lift and tonify, or combined them with Wu Mei (Mume Fruit) and Wu Wei Zi (Schisandrae Fructus) to secure and astringe. Externally, using fumigation and washing agents with astringent properties ensures cure without exception.
[Anal Itching and Pain] This is a disease involving Wind, Dampness, and Fire simultaneously. When Large Intestine receives Dampness, it flows to the anus, causing itching; use Qin Jiao Qiang Huo Tang (Gentian and Notopterygium Decoction). If affected by Wind and Damp-Heat, parasites may generate causing itching; use Shenying Heiyu Dan (Divine Response Black Jade Pill) or Pian Xu Tang (Polygoni Avicularis Decoction), externally wash with Ku Lian Gen (Meliae Cortex) decoction. If worms erode the anus, sores appear on the upper lip; use Huachong Wan (Worm-Dissolving Pill). If Large Intestine has Fire, the anus becomes painful; use Qisheng Wan (Seven-Sages Pill) or Qin Jiao Bai Zhu Wan (Gentian and Atractylodes Pill). In severe cases with dry stool, rectal protrusion, and bleeding, use Danggui Yuliren Tang (Angelica and Prunus Decoction).
Danxi stated: Whenever one engages in sexual intercourse after heavy drinking and eating, or suppresses the emission of anterior yin Qi, which then returns to the Large Intestine, Wood overacts on Fire and insults Dry Metal, thus Fire follows Dryness, resulting in inevitable constipation. For severe cases, one must use Bitter-Cold to drain Fire, Pungent-Warm to harmonize Blood, Moisten Dryness, Dispel Wind, and relieve pain; this is the treatment principle. Suitable formulas include Qin Jiao Bai Zhu Wan (Gentian and Atractylodes Pill), Kuchang Wan (Intestine-Widening Pill), and Danggui Yuliren Tang (Angelica and Prunus Decoction).
