When infants present with exanthems (rash), if the rash has receded due to wind exposure and has not cleared smoothly, a harmonizing method should be used to promote eruption, utilizing the Xiaodu Yin (Disinfection Decoction). If the rash has fully erupted, it is appropriate to use Qingjin San (Clear Metal Powder) to disperse wind-heat and drain heart-fire, ensuring that Lung Metal is not overcontrolled by Heart Fire. Otherwise, the mother of the Lung (Lung itself in this context referring to the organ system or potentially Spleen as Earth generating Metal, but text says "Lung's mother", likely implying the protective layer or the organ's vulnerability) may be closed by cold pathogens, inevitably leading to phlegm-wheezing, coughing, hoarseness, fever, sore throat, loss of appetite, followed by sores and scabies, gradually developing into malnutrition (Gan syndrome) and consumption diseases. If the pathogen transmits downward into the Large Intestine, symptoms such as diarrhea and spontaneous dysentery will occur; this is because the Heart Fire has not dissipated, or there may be hemoptysis, hematemesis, epistaxis, or damage to the eyes. If the pathogen transmits downward into the Small Intestine, symptoms such as red and scanty urine and bloody dysentery will occur. All the above conditions should be treated with descending methods.
Runfei Xiaodu Yin (Moisten Lung and Disinfect Decoction):
To moisten the lung and disinfect, use Ziqin (Scutellariae Radix), Zhike (Aurantii Fructus), Huafen (Trichosanthis Radix), and Gualouren (Trichosanthis Semen);
Sangye (Mori Folium), Xingren (Armeniacae Semen Amarum), Chenpi (Citri Reticulatae Pericarpium), Suzi (Perillae Fructus), and Shanzhi (Gardeniae Fructus);
Shigao (Gypsum Fibrosum) and Laifuzi (Raphani Semen) descend qi and clear heat;
If wheezing occurs, add Mahuang (Ephedrae Herba) and Danzhuli (Bambusa Textilis Caulis Juice); if constipation occurs, add Dahuang (Rhei Radix et Rhizoma) and Zhen Heliu (Tamarilli Ramulus).
This decoction is essentially Qingjin Jianghuo Tang (Clear Metal and Descend Fire Decoction). If the rash has reached the fingertips and the Tianting area (forehead), this formula can be used without replacement. If the rash has not fully emerged, do not judge based on complete eruption; if the person is calm and normal, administration can be delayed. Comprehensive discussion on symptom observation and prescription modification: Use Shigao and Laifuzi to descend qi and stabilize wheezing; use heavy doses of Mahuang to open the lung orifices; use raw and cooked Dahuang to unblock obstructions; use Zhuru (Bambusa Textilis Caulis) to resolve stomach stagnation; use honey-fried Pipaye (Loquat Leaf) to stop coughing; use Lüdou (Phaseoli Semen) and Zhima (Sesami Fructus) to detoxify and moisten the intestines; add Zhuli (Bamboo Juice) to eliminate phlegm, moisten dryness, and descend fire; Xi Heliu (Tamarilli Ramulus) helps expel rash toxins outward and treats post-rash dysentery; Dilì (Dioscorea esculenta / Water Chestnut) treats dysphagia, opens the stomach, aids digestion, and clears excess heat from the chest; Baijiezi (Sinapis Semen) resolves phlegm, regulates qi, and induces sweating; Shuilugen (Phragmitis Rhizoma) descends fire and stops vomiting; Baimaogen (Imperatae Rhizoma) controls erratic blood flow, stops hiccups, treats lung-heat wheezing, and stops strangury; Mutong (Akebiae Caulis) promotes urination and guides fire downward; add Doujian (Diospyros Fructus Peduncle) to stop hiccups and descend qi; Congbai (Allii Fistulosi Bulbus) induces sweating and releases the muscle layer.
For the backup exterior-releasing formula: Shengma (Cimicifugae Rhizoma), Qianghuo (Notopterygii Rhizoma et Radix), Fangfeng (Saposhnikoviae Radix), combined with Juhua (Chrysanthemi Flos), Chanyi (Bombyx Batryticatus), Dalì (likely Niu Bang Zi / Arctii Fructus in context, though literally "Great Strength"), Chuanxiong (Ligustici Chuanxiong Rhizoma), Jiegeng (Platycodonis Radix), Jingjiesui (Schizonepetae Spica), Qianhu (Peucedani Radix), Gegen (Puerariae Lobatae Radix), and Gouteng (Uncariae Ramulus cum Uncis).
For the half-exterior half-interior descending formula: Zili (likely Zisu/Radix Perillae or similar, text says Zili), Lì (Dalì/Niu Bang Zi), Qin (Qin/Coptidis Rhizoma or Scutellariae Radix - text says Zi Li Qin, likely Ziqin/Scutellariae Radix), Suzi (Perillae Fructus), Gouteng (Uncariae Ramulus cum Uncis), Shanzhiren (Gardeniae Fructus), Shigao (Gypsum Fibrosum), Gancao (Glycyrrhizae Radix et Rhizoma), Mutong (Akebiae Caulis), Sangye (Mori Folium), Xingren (Armeniacae Semen Amarum); if constipation, add Dahuang (Rhei Radix et Rhizoma).
Xiaotan Huadu Tang (Eliminate Phlegm and Resolve Toxin Decoction):
To eliminate phlegm and resolve toxin, use Fu Chanyi (likely Chanyi/Bombyx Batryticatus), Bohe (Menthae Haplocalycis Herba), Gouteng (Uncariae Ramulus cum Uncis), Baishangpi (Mori Cortex), Zisu (Perillae Folium), Xingren (Armeniacae Semen Amarum), Gancao (Glycyrrhizae Radix et Rhizoma), Jiegeng (Platycodonis Radix), Dalì (Niu Bang Zi / Arctii Fructus), and Jiangcan (Bombyx Batryticatus) are suitable.
After taking this decoction, if the patient is fatigued, breathes heavily, has phlegm-cough and hoarseness, or suffers from wind-cold invasion, taking this can detoxify, resolve phlegm, restore voice, and disperse pathogens.
