The pathogenic factor exits from the lower jiao, the dysentery stops, but the residual heat instead travels to the various collaterals and overflows upward. Otherwise, how could the properties of Paojiang (Processed Zingiberis Rhizoma) and Wuweizi (Schisandrae Fructus) be slightly relieved? This epistaxis (nosebleed) is also similar to the heat pathogen in the Yangming channel of Shanghan (Cold Damage), which resolves through "red sweat" (referring to nosebleed).
You Zaijing said: The *Ping Re Bing Lun* (Treatise on Evaluating Heat Diseases) states that Laofeng (Labor Wind) typically occurs below the lungs. Its symptoms include stiffness and rigidity of the neck, dim vision, expectoration of viscous sputum resembling mucus, aversion to wind, and chills. How should it be treated? It is answered: One must rescue the difficulty in flexion and extension (breathing difficulties). If the kidney essence is sufficient to guide the Jing (essence) upward, recovery occurs in three days; for middle-aged individuals, in five days; for those with deficient essence, in seven days. During this time, clear or yellow nasal discharge is coughed up, resembling pus, shaped like a pellet, exiting from the mouth or nose. If not discharged, the lungs will be injured, and injury to the lungs leads to death. Reading this allows one to understand why unresolved wind-cold can evolve into consumptive disease (Lao). "Laofeng" refers to contracting wind pathogen while already fatigued. Fatigue causes fire to flare upwards, and wind takes advantage of this, causing wind and fire to contend, with Qi gathering in the upper part; thus, it is said that the pathology lies below the lungs. The Lung governs Qi and respiration. When wind-heat invades the Lung, body fluids inevitably congeal, and Qi inevitably becomes obstructed, making both flexion and extension difficult; hence, it is said that one must rescue the difficulty in flexion and extension. Rescuing flexion and extension means benefiting Lung Qi and dispersing pathogenic factors. However, whether pathogenic factors disperse depends on the strength of Zhengqi (Upright Qi). If Yang Qi is vigorous and Jing essence guides effectively, after three days, then five days, and finally seven days, green-yellow nasal discharge will be coughed out. Once discharged, wind and heat exit together, and the Lung remains unharmed. If not discharged, wind and fire remain accumulated in the Lung, injuring it. Lung injury results in wheezing, coughing, and hoarseness, gradually affecting the five Zang organs, thereby forming consumptive disease. Nowadays, people treat consumptive disease by daily use of nourishing tonics, yet see little benefit. Is this not because the pathogenic factors have not been expelled? For pathogens lingering for a long time, tonification is certainly useless, and clearing heat does not resolve them either; this is why consumptive disease is difficult to treat.
Xu Lingtai said: When ordinary people occasionally contract wind-cold, presenting with headache, fever, cough, and runny nose, it is colloquially called Shangfeng (Wind Cold). This is not the Shangfeng described in the *Shanghan Lun* (Treatise on Cold Damage), but rather a miscellaneous contraction during seasonal epidemics. People often ignore it, unaware that this is a very difficult-to-treat disease concerning life and death. Generally, Shangfeng enters the Lung via the skin and hair. The Lung is a delicate organ, unsuitable for both cold and heat. Too much cold causes pathogenic factors to stagnate without exiting; too much heat scorches Metal (Lung) and moves blood; too much moisture generates phlegm-fluids; too much dryness consumes essence and fluids; excessive dispersion causes sweating leading to Yang deficiency; excessive astringency causes Qi closure and pathogenic binding. Moreover, some regard it as a minor ailment, failing to avoid wind and cold or regulate diet, allowing the pathological mechanism to deepen year after year. Some develop hemorrhagic diseases, some develop Feiwei (Lung Atrophy), some develop asthma, some become frail, all of which are common. The harm of misdiagnosis and mistreatment is countless. The proverb says: "Unresolved wind-cold becomes consumption." This is a profound truth. So, how should it be treated? First, expel wind, using herbs like Suye (Perillae Folium) and Jingjie (Schizonepetae Herba); second, eliminate phlegm, using Banxia (Pinelliae Rhizoma) and Xiangbei (Fritillariae Thunbergii Bulbus); third, direct Qi downward, using Suzi (Perillae Fructus) and Qianhu (Peucedani Radix); fourth, harmonize Ying and Wei (Nutritive and Defensive levels), using Guizhi (Cinnamomi Ramulus) and Baishao (Paeoniae Radix Alba); fifth, moisten fluids, using Louren (Trichosanthis Semen) and Yuan Shen (Scrophulariae Radix); sixth, nourish blood, using Danggui (Angelicae Sinensis Radix) and Ejiao (Asini Corii Colla); seventh, clear fire, using Huangqin (Scutellariae Radix) and Shanzhi (Gardeniae Fructus); eighth, regulate Lung Qi, using Sangpi (Mori Cortex) and Niulizi (Arctii Fructus). These eight methods are added or subtracted according to the severity of symptoms. Additionally, avoiding wind and cold, and abstaining from pungent and sour foods, will likely lead to gradual recovery; otherwise, serious illness will surely develop. If one further adds lifting and pungent-drying agents like Jiegeng (Platycodonis Radix) or Ganjiang (Zingiberis Rhizoma), and if ineffective, adds astringents like Wuweizi (Schisandrae Fructus), bleeding will definitely occur. Once bleeding occurs, if Shudi (Rehmanniae Radix Preparata) and Maidong (Ophiopogonis Radix) are immediately used to fill the Lung, consumptive disease will result in death. In forty years, I have seen thousands of such cases. How tragic.
Zhang Luyu treated the eldest daughter-in-law of Jiang Runong, who suffered from wind-heat cough, absence of sputum, burning sensation, spontaneous sweating, and was seven months pregnant. She had previously taken twenty-plus doses of harmonizing and lung-clearing medicines, but her cough worsened, and the fetus became increasingly unstable. Pulse diagnosis revealed all six pulses were soggy, large, and powerless, with the right Cun and Guan positions particularly full and choppy. Zhang said: This is heat injuring Lung Qi. Using harmonizing medicines forced sweating, causing Lung Qi to become even drier and the cough more severe. Reluctantly using bitter-cold medicines to suppress the fire only caused the fire to become depressed and trapped inside without dissipating. Therefore, he prescribed a large dose of Weirui (Polygonati Odorati Rhizoma), along with Chuanxiong (Ligustici Chuanxiong Rhizoma), Xingren (Armeniacae Semen Amarum), Baiwei (Anemarrhenae Rhizoma), and Gancao (Glycyrrhizae Radix et Rhizoma), taking half of the Weirui Tang (Jade Decoction). He additionally added Danggui (Angelicae Sinensis Radix), Jiegeng (Platycodonis Radix), Wuweizi (Schisandrae Fructus), and Huangqi (Astragali Radix) to benefit Qi and generate fluids, thereby astringing Lung Qi. After two doses, sweating stopped, cough decreased, and the fetus stabilized. Adding raw Hezi Pi (Terminalia Chebulae Pericarpium), she recovered after one dose.
Yu Jinyan contracted wind-cold with spontaneous sweating after laborious work, experiencing chest fullness and phlegm accumulation, coughing up green or yellow nasal discharge shaped like pellets. This corresponds to the Laofeng pathology located below the lungs mentioned in the *Neijing*. He prescribed Fuling Guizhi Baizhu Gancao Tang (Poria, Cinnamon Twig, Atractylodes Macrocephala, and Licorice Decoction), adding Ginger Juice and Bamboo Sap. After two doses, the condition stabilized. He also treated Song Xiang's daughter, who was prone to depression, suffered from dry cough, and caught wind in spring, worsening her cough. He used Xiaojianzhong Tang (Minor Construct the Middle Decoction) prepared with honey-fried ginger, adding honey-fried Chenpi (Citri Reticulatae Pericarpium). The external pathogen resolved, but the cough persisted. Next, he used Xiaoyao San (Free Wanderer Powder), still using honey-fried ginger and orange peel, followed by Yigong San (Extraordinary Merit Powder) using honey-processed Baizhu (Atractylodis Macrocephalae Rhizoma), ginger, and orange peel, which brought stability.
Wu Peiyu's daughter suffered from wind-cold cough. Self-medicating with wind-expelling and lung-moistening drugs aggravated vomiting, thirst, and sore throat. Pulse diagnosis showed floating and slippery pulses responding to pressure. He prescribed Banxia San (Pinellia Powder). After three administrations, the illness vanished as if lost. Someone asked: Cough, sore throat, and thirst usually contraindicate drying agents in the world; why did Banxia prove effective? He replied: Treating disease requires seeking the root. This case involved acute pressure from wind-cold pathogen carrying fluid retention attacking upward. Thus, Banxia and Guizhi were used to open meridians and rapidly sweep away phlegm-saliva, combined with Gancao to harmonize the Spleen and Stomach and rescue fluids. Once wind-phlegm dispersed and Ying and Wei circulated freely, sore throat and dry thirst would naturally subside. If cooling and moistening agents were used, they would nourish phlegm-dampness, further obstructing meridians and condensing fluids, making dry throat and thirst even more persistent. Modern practitioners treating wind-cold cough, although using exterior-releasing drugs, invariably combine them with Sangbai (Mori Cortex), Huangqin (Scutellariae Radix), and Huafen (Trichosanthis Radix), or even Zhimu (Anemarrhenae Rhizoma) and Baipi (Phellodendri Cortex). Young people subjected to this treatment inevitably plant the roots of hemoptysis and deficiency damage. Those past middle age often develop phlegm-fire wheezing and coughing. Initially, when Yuanqi (Original Qi) is not yet depleted and pathogenic heat temporarily latent, it may seem manageable. Over time, true Qi is gradually damaged, and the more medicine taken, the worse the condition becomes. When hemoptysis appears, regardless of the meridian or Zang-Fu involved, whether due to fire or injury, whether blood is scattered or clotted, whether complexion is dull or bright, whether stasis exists, physicians indiscriminately administer cool-cold hemostatic agents like Xijiao Dihuang Tang (Rhinoceros Horn and Rehmannia Decoction), causing blood to accumulate and become entrenched. Later, attacks may occur every month or two; despite daily intake of previous medications, there is no response. I once saw a person with neck swelling and fever. A Shanghan specialist diagnosed it as Datou Shanghan (Big Head Cold) and used exterior-releasing drugs, causing head swelling as well (undeniably so). Big Head Cold seemed certain. However, seeing the treatment worsen the condition, the family consulted a miscellaneous disease specialist, who diagnosed damp-heat and phlegm-fire, using interior-purging drugs. The head and anterior left neck subsided, but posterior right neck remained swollen, proving it was not Big Head Cold but a miscellaneous disease. Suspecting an abscess due to the lateral swelling, the family consulted a surgeon, who diagnosed a lateral Duikou Carbuncle, treating it with internal fortification and external applications. This caused further Qi and Blood stagnation, preventing heat release, leading to郁遏 (depression/obstruction) and eventually ulceration. The head ailment was originally the same, but treatments varied drastically, and symptoms shifted repeatedly. This confirms that diseases change according to the drugs administered.
