This condition arises from qi stagnation and rebellion, not from deficiency leading to collapse. Pressing the chest causes the patient to frown; pressing the abdomen causes the body to stiffen in pain. Inquiry reveals that due to a busy day with no time to eat, the illness erupted after entertaining guests late at night. The cause is extreme hunger followed by overeating, resulting in food retention in the middle jiao. Confusion and inability to speak, along with a hidden pulse (Fuchi Mai), are caused by dietary accumulation blocking the clear orifices. Inability to move the limbs corresponds to the classic description of excessive Earth qi. Initially, one emesis would have sufficed; now that three days have passed, the upper, middle, and lower jiao are all affected. One should employ simultaneous emesis and purgation with digestive agents to divide and reduce the pathogenic force. Thus, ginger light salt water was used to induce vomiting, expelling several bowls of phlegm and fluid. Shortly after, mental clarity slightly improved. Pulse diagnosis revealed rapid and forceful pulses at the Cun (inch) and Guan (bar) positions. A decoction of Ezhu (Curcuma zedoaria Rhizome), E Zhu (Curcuma phaeocaulis Rhizome), Binglang (Areca Seed), Zhishi (Immature Bitter Orange), Juhong (Aged Tangerine Peel), Shenqu (Medicated Leaven), Muxiang (Costus Root), Baiddoukou Ren (White Cardamom Seed), and Laifuzi (Radish Seed) was prepared to deliver five qian of Runzi Wan. After three or four bowel movements, the condition significantly subsided. Re-examination showed Guan and Chi (cubit) pulses present, deep and forceful, though tenderness remained upon abdominal palpation. Two qian of Runzi Wan were administered again with the previous decoction. Four days later, thin rice gruel was introduced, and Erchen Tang (Two-Cured Decoction) was prescribed with minor additions of Danggui (Angelica Sinensis Radix) and Shaoyao (Peony Root) to nourish Ying blood, Renshen (Ginseng Radix) and Baizhu (Atractylodes Macrocephala Rhizome) to support Stomach Qi, and Muxiang and Doukou Ren to resolve residual fullness. Peace was restored within ten days.
Lu Xiaoyu treated the wife of Pan Biquan, nearly fifty years old. Following severe anger, she suddenly collapsed with clenched jaw. Pulse diagnosis revealed slippery and large pulses at both Cun and Guan positions, while Chi pulses were deep and absent. Xixian San (Thin Saliva Powder) mixed with water was administered via forced ingestion after prying open the teeth, expelling approximately one basin of phlegm. She revived shortly after, though consciousness remained unclear. Re-palpation showed slightly normalized Cun/Guan pulses and rising Chi pulses. Erchen Tang (Two-Cured Decoction) added with Beimu (Fritillaria Bulb), Huanglian (Coptis Rhizome), and Xiangfu (Cyperus Rhizome) was given for several doses, resulting in recovery.
Lu Zuyu treated Yu Zhongkai. After mental exertion compounded by anger, he suddenly fainted while combing his hair in the morning. Symptoms included limb coldness, open eyes and mouth, sawing-like throat sounds, and urinary/fecal retention. Pulse diagnosis: left three positions were wiry, slippery, and rapid; right three positions were deep, solid, and forceful. This is Phlegm Jue (Phlegm Cold Extremities). Niuhuang Wan (Calculus Bovis Pill) dissolved in ginger juice, with an additional 1 fen of Niuhuang (Calculus Bovis), was administered continuously (4-5 pills). Subsequently, Chenpi (Aged Tangerine Peel), Beimu (Fritillaria Bulb), Huafen (Trichosanthes Root), Danning (Arisaema Cum Bile), Huangqin (Scutellaria Root), Huanglian (Coptis Rhizome), Zhishi (Immature Bitter Orange), Gualou (Trichosanthes Fruit), Qianhu (Peucedanum Root), Jiegeng (Platycodon Root), Zhaojia (Soap Pod), Jiangzhi (Ginger Juice), and Zhuli (Bamboo Sap) were taken as a single dose to induce vomiting of thick phlegm (2-3 bowls). Soap pod and Chenpi were removed from the formula, adding Qingpi (Green Tangerine Peel). After two doses and two bowel movements, old phlegm was expelled via stool, and symptoms markedly decreased. Thereafter, formulas for strengthening Spleen, clearing fire, nourishing blood, and resolving phlegm were used for regulation.
Zheng Xianfu, over sixty, fainted and became immobile following severe anger. I reasoned that anger generates fire in the Liver, causing closure of Qi flow in the Foot Jueyin and Foot Shaoyin meridians, leading to unconsciousness. Severe anger damages sinews, causing flaccidity and loss of motor function. Urgently, Huanglian (Coptis Rhizome) and Huangbai (Phellodendron Bark) were used to drain rebellious fire, and Xiangfu (Cyperus Rhizome) to subdue Liver Qi. Consciousness gradually cleared within one to two days. Subsequently, tonics harmonizing Qi and Blood and strengthening sinews and bones were administered, leading to recovery in a few days. (From *Yaoyao Huowen*, *Yishuo Xubian*.)
Zhu Danxi treated Lv Zongxin, who had abdominal masses and cold feet extending to the knees. Da Chengqi Tang (Major Order the Qi Decoction) modified was used for purgation, curing both the masses and the cold extremities. (Masses are easily identified; foot coldness indicates Heat Jue, often misunderstood. Filed under Jue category.)
Wu Yang treated a fellow townsman returning from Zhejiang with cold extremities, imperceptible pulses, lying motionless for over ten days without intake of fluids or food. Physicians deemed him dead. Wu Yang diagnosed Heat Jue (Heat Cold Extremities). He placed a basin of fresh water in a bathhouse, set a board over it, and laid the patient on top. Using a blue cloth, he wiped the limbs and splashed water on them, also wetting the patient's lips. Thirsty for water, the patient drank the entire amount and recovered. (From *Taihan Ji*.)
Passing through Zhangqi, Wu Qiao was stopped by someone reporting on Wang Yiyang, over fifty, who developed fever after hematuria. Various physicians treated him for Taiyang disease (with diaphoretic agents) or post-dysentery deficiency (with Spleen-tonifying agents). Prolonged treatment led to delirium, coma, and cold extremities after ten days of fasting. Desperate family sought Wu Qiao's opinion. Wu Qiao stated this was Heat Jue and could be cured. Shigao Huanglian Tang (Gypsum and Coptis Decoction) was given; the patient revived after one dose and recovered after five doses over subsequent days. (Same source.)
Wei Yuheng stated: The sister of Bao Luyin suffered from Jue, unconscious, eyes closed, nasal flaring, cyanosis around the nose and mouth, and intermittent limb convulsions from midnight until Si hour (9-11 AM), unable to take fluids. Pulses were irregular. I diagnosed severe Lung Metal deficiency with rebellious Liver Fire, where extreme heat mimics wind. Only Renshen Tang (Ginseng Decoction) could cure this; other medicines were useless. Although Ginseng was boiled, others dissuaded its use. Another physician prescribed Shichangpu (Acorus Rhizome), Yuanzhi (Polygala Root) to open the orifices, Fushen (Poria with Hostwood), Zaoren (Ziziphus Seed) to calm the spirit, Maidong (Ophiopogon Tuber), Beimu (Fritillaria Bulb) to clear phlegm, and Chensha (Cinnabar), Iron Rust Water to anchor. However, medicine poured into the left mouth corner flowed out the right, never swallowing. Helpless, we waited until dusk. Upon re-examination, the forehead turned black. Urgent Renshen Tang administration was still viable. Administered via teaspoon, after 6-7 teaspoons, swallowing sounds were heard. As the jaw relaxed, rice water mixed with Ginseng Tang was given. Eyes opened, body moved, and cyanosis vanished. Thin porridge was slowly consumed, and the patient sat up and recovered. Later recurrences were mild, cured by Dihuang (Rehmannia Root), Shashen (Glehnia Root), Maidong (Ophiopogon Tuber), and Gouqi (Goji Berry).
The daughter of the Gu family, sixteen, with a history of sores and Yin-deficiency constitution, developed hypochondriac pain and cough with sputum due to labor at year-end. One physician prescribed Guanghuoxiang (Patchouli), Niuxi (Achyranthes Root), Jingjie (Schizonepeta Spike), Fangfeng (Saposhnikovia Root), Xiangfu (Cyperus Rhizome), Sharen (Amomum Fruit), Zhike (Bitter Orange), Jiegeng (Platycodon Root) for three to four doses. She felt a mass like a fist pressing upward in the Zhongwan area, preventing food intake. An older physician attributed this to excessive phlegm requiring removal. The formula was similar but added Cangzhu (Atractylodes Lancea Rhizome), Houpo (Magnolia Bark), Zhuli (Bamboo Sap), and Jiangzhi (Ginger Juice). After two doses, the condition worsened. Treatment was sought on the second day of the new year. By arrival, the patient had died at dawn, presenting with closed eyes, open mouth, rigid posture, unresponsiveness, and inability to swallow. Abdomen was still warm. Pulse was absent, complexion pale-green, gums bleeding purple clotted blood. Limbs were soft. Diagnosis: Original Qi deficiency exacerbated by labor, mistreated with drying/dispersing herbs damaging Lung Metal, causing Liver Wood to carry phlegm/food upward. Harsh drugs depleted Yin and dried Wood, leading to Yin collapse and Yang floating. Unlike Phlegm-Food Jue which responds to emesis, this Yin-collapse Jue requires restoring Soul and Spirit. Vitality remained. Immediate prescription: Shudi (Prepared Rehmannia Root) 2 liang, Gouqi (Goji Berry) 1 liang, Shashen (Glehnia Root) and Maidong (Ophiopogon Tuber) 5 qian each. Rapidly boiled and slowly administered. Fear of non-swallowing was mitigated by using a hairpin to assist ingestion. Swallowing sounds confirmed success. Next day, patient could sit up. Formula halved, plus Gualou Ren (Trichosanthes Seed) 2 qian, eight doses resulted in full recovery. Years later, upon marriage, conflict with mother-in-law caused hypochondriac pain. A physician recommended Muxiang (Costus Root) powder. Mother-in-law forbade it. Patient died from a sudden Jue episode.
Huang Jinfang treated Li Mou with cold extremities, shivering chills, and icy skin. Though facial expression was bleak, internal sensation was irritability and fullness; pulse was deep/hidden but Liver pulse was forceful. Diagnosed as Heat Jue. Prescription: Huangqin (Scutellaria Root) 1 qian, Huanglian (Coptis Rhizome) 0.5 qian, Chaihu (Bupleurum Root) 0.8 qian, Zhike (Bitter Orange) 0.8 qian, Houpo (Magnolia Bark) 1 qian, Dahuang (Rhubarb Root) 2 qian, Wumei (Mume Fruit) 1 piece, Qingpi (Green Tangerine Peel) 0.5 qian, Binglang (Areca Seed) 0.8 qian, Xixin (Asarum Root) 0.2 qian. After administration, cold extremities reversed, whole body became hot, and mild medications completed the cure.
