Zhu Danxi treated a patient with asthma (a disease characterized by rapid breathing and rattling phlegm in the throat), which occurred once every ten days. The disease was located in the Upper Jiao (the area above the diaphragm, including the chest and lungs) and could not be resolved through sweating. It was the tenth month, so he used Ephedrae Herba (Mahuang, 麻黄) and Scutellariae Radix (Huangqin, 黄芩), each two qian, decocted with ginger juice for oral administration. At bedtime, he administered Xiaowei Wan (Small Stomach Pill) thirty pills, after which the condition stabilized. Xiaowei Wan of Danxi consists of Genkwa Flos (Yuanhua, 芫花) stir-fried with vinegar, Kansui Radix (Gansui, 甘遂), Rhei Radix et Rhizoma (Dahuang, 大黄), Euphorbiae Pekinensis Radix (Daji, 大戟), and Phellodendri Cortex (Huangbai, 黄柏), ground into fine powder, and formed into pills using Atractylodis Macrocephalae Rhizoma paste (Baizhu Gao).
The *Shengji Zonglu* records the treatment of a person who choked on vinegar, causing incessant wheezing and asthma. Two liang of peeled Glycyrrhizae Radix et Rhizoma (Fen Gancao, 粉甘草) were cracked open and soaked in the juice of six or seven pig gallbladders for three days. After being dried by roasting and ground into powder, honey pills were made. Thirty to forty pills were taken with clear tea, leading to gradual recovery.
Wang Yutai treated a patient with salt-induced asthma (asthma triggered by excessive salty food intake). He used two qian of white flour, kneaded with rock sugar, shaped into cakes using ash lye from sugar cakes, dried them in a stove, cut them into four pieces, and sprinkled them with four qian of Arsenic Sulfide (Qingfen, 轻粉) separately stir-fried. The patient ate this and recovered by vomiting phlegm. (All the above cases are from *Da Huan*.)
Gong Zicai treated a patient suffering from asthma for over ten years. Upon onset, there was upper airway wheezing, coughing with expectoration, spontaneous sweating, cold limbs, and deep, fine pulses in all six positions. This indicated Qi deficiency and Spleen weakness. He prescribed Liujunzi Tang (Six Gentlemen Decoction) augmented with Astragali Radix (Huangqi, 黄芪), Schisandrae Fructus (Wuweizi, 五味子), Asparagi Radix (Tandong, 天冬), Ophiopogonis Radix (Mandong, 麦冬), Pruni Semen (Xingren, 杏仁), Zingiberis Rhizoma (Jiangsheng, 生姜), and Jujubae Fructus (Zao, 枣), decocted and administered, resulting in recovery.
Another patient had suffered from asthma since childhood, triggered by cold exposure. Onset involved rapid wheezing, coughing, and surging phlegm-saliva. The condition persisted without cure, becoming chronic. He was prescribed Licorice Root (Gancao, 甘草), Platycodon Root (Jiegeng, 桔梗), Scutellaria Root (Huangqin, 黄芩), Coptis Rhizome (Lian, 黄连), Trichosanthes Fruit (Gualou, 瓜蒌), Fritillaria Bulb (Beimu, 贝母), Asparagus Root (Tandong, 天冬), and Ophiopogon Root (Mandong, 麦冬) to clear Lung heat, combined with Liuwei Dihuang Wan (Six-Ingredient Pill with Rehmannia) to tonify the Kidney. The formula was named Qing Shang Bu Xia Wan (Clear Upper Tonify Lower Pill). One course led to complete recovery.
Sun Wenyuan treated Cha Shaochuan, who had a long-standing history of asthma. Each attack caused his whole body to feel hot as if burning, shortness of breath, and loud rattling sounds in the throat like boiling water. Recovery typically took seven days and nights, ending with sweating. (This aligns with the principle of "recovery after seven days.") He habitually indulged in alcohol and sexual activity, exposing himself to wind and cold. Someone advised him to take a large dose of one ounce each of Gypsum Fibrosum (Shigao, 石膏), Ephedra Herb (Mahuang, 麻黄), Apricot Seed (Xingren, 杏仁), Aurantium Fruit (Zhike, 枳壳), and strong tea, decocted together. This was called Wuhu Tang (Five Tiger Decoction). Whenever asthma struck, he would drink this to counteract it, stopping immediately after drinking. It worked rapidly and effectively upon repeated use. Over three years, he consumed countless doses of Wuhu Tang, resulting in abdominal distension as large as an inverted basket, legs swollen and shiny like pillars, internal and external sores, continuous discharge of clear fluid, and a pungent odor. He could not lie flat for five months. When consulted on the day after Winter Solstice, his breathing was labored and wheezy, his body hot and restless. He wore heavy cotton quilts, placed his feet in a fire box, surrounded himself with fire basins, wore a velvet hat and muffs, bound his head with a cloth, covered his nose with a velvet sleeve, and sat within heavy curtains, yet still shivered with fear of cold for ten days. His pulse was floating, large, and powerless; his complexion was white with hints of blue. The old disease stemmed from Qi deficiency and central fullness; the new disease resulted from Cold pathogen in the exterior. He was instructed to first release the exterior using Perilla Leaf (Zisu, 紫苏), Water Chestnut (Matou, 荸荠), Asarum Root (Xixin, 细辛), Licorice Root (Gancao, 甘草), Saposhnikovia Root (Fangfeng, 防风), White Cardamom Seed (Baokourén, 白蔻仁), Atractylodes Rhizome (Cangzhu, 苍术), Tangerine Peel (Chenpi, 陈皮), Ginseng Root (Renshen, 人参), Notopterygium Root (Qianghuo, 羌活), and Fresh Ginger (Shengjiang, 生姜). After one dose, he sweated slightly, and the fear of cold disappeared. However, his nose remained cold. He used Saposhnikovia Root (Fangfeng, 防风) and Astragalus Root (Huangqi, 黄芪), two liang each, decocted to steam his nose. This was done for the duration of a meal, three times a day, and cured the next day. Persistent nausea and vomiting were treated with Renshen Weni Tang (Ginseng Warming Spleen Decoction) augmented with Clove (Dingxiang, 丁香). One dose stopped it. Wanting to promote urination, he ate several jin of carp at night, causing severe abdominal distension. Pingwei San (Pingwei Powder) with one liang of Olive Flesh (Ganlanrou, 橄榄肉) (which can resolve fish toxicity) was given for two doses, stabilizing the condition. However, abdominal distension and difficulty urinating prevented him from lying down. Fuzi Lizhong Tang (Aconite Regulate Middle Decoction) augmented with Amomum Fruit (Sharen, 砂仁), Psoralea Fruit (Buguzhi, 补骨脂), Adzuki Bean (Chixiaodou, 赤小豆), and Cinnamon Bark (Guixin, 桂心) was administered for four consecutive doses, slightly improving urination. Subsequently, Zunzhong Wan (Respectful Pill) was taken three times daily, five pills per dose. After five days, urination became smooth, allowing him to lie down. Maintaining this regimen resolved the distension.
Li Shicai treated Gu Wenxue, who had suffered from asthma for ten years with no effect from various medicines. Palpating the two Cun positions, he found them rapid and choppy. He said: "A choppy pulse indicates that Phlegm-Fire, Wind-Cold have lodged deeply and firmly for a long time. It is necessary to tonify for over a month, then employ emetic and purgative methods." Within half a year, he underwent emesis and purgation ten times and took over a hundred types of tonifying formulas before recovering. He then took Buzhong Yiqi Wan (Tonify the Middle and Augment the Qi Pill) augmented with Chicken Egg (Jizi, 鸡子) and Halitum (Qiushi, 秋石), taking it for about a year, preventing recurrence permanently. (Buzhong Yiqi Tang is not perfectly suited to this disease; its success was due to the combined effect of attacking and tonifying. Wang Ken-tang's view is correct: patients with deficiency cannot easily undergo attacking therapies without prior tonification.)
Shi Peiran treated the wife of Ruan Erhua, who suffered from asthma exacerbated by excessive use of cooling agents. Phlegm obstructed upwards, causing yellowish swelling of the face and eyes. During fainting spells, she appeared motionless as if dead; upon recovery, snoring sounds were audible in the outer courtyard. Physicians fled upon seeing her. Palpating the six pulses, they were deep, slippery, weak, and tight. The illness originated from winter Cold injury. The Classics state: "Cold exposure and cold drinks injure the Lung." Ancients treated this disease using light, clear, acrid, and dispersing agents containing Ephedra Herb (Mahuang, 麻黄). If cold-cooling agents were used, the pathogen would become sealed and unable to exit, causing phlegm to bind increasingly and obstructing the Qi of the Upper Jiao. Thus, Tongguan San (Pass the Gate Powder) was used to induce vomiting of phlegm-saliva. After three inductions, the phlegm-Qi cleared, and wheezing stabilized. Later, San'ao Tang (Three Unblocking Decoction) combined with Daotan Tang (Phlegm-Guiding Decoction) was adjusted for regulation, leading to peace within a month. *Ju Fang* San'ao Tang: Ephedra Herb (Mahuang, 麻黄) with stems intact, Apricot Seed (Xingren, 杏仁), Licorice Root (Gancao, 甘草) in equal parts, plus five slices of Fresh Ginger (Shengjiang, 生姜), decocted together. *Ju Fang* Tongguan San: Ligusticum Rhizome (Chuanxiong, 川芎) one liang, Asarum Root (Xixin, 细辛) five qian, Licorice Root (Gancao, 甘草), Aconite Root (Chuanwu, 川乌), Angelica Dahurica Root (Baizhi, 白芷), Ligusticum Chuanxiong (Fuxiong, 抚芎) each two liang, Borneol (Longnao, 龙脑), and Mint Leaf (Boheye, 薄荷叶) half liang. All ground into fine powder. Take one qian per dose, mixed with scallion white and tea liquor; mint decoction is also acceptable.
Qian Guobin treated the mother of Zhao Yan, a scholar from Jinling, aged sixty. She suffered from a Phlegm disorder, roaring like a saw day and night, vomiting several bowls of phlegm. Initially able to walk, she gradually became bedridden. Fortunately, her Stomach function remained intact, and her diet was normal. Many physicians failed. Palpating the six positions, the pulses were floating and slippery, more pronounced on the right Cun and Guan positions. Floating indicates Lung Qi deficiency; slippery indicates accumulated Phlegm in the Spleen meridian. This is Phlegm Roaring Syndrome. Daotan Tang (Phlegm-Guiding Decoction) augmented with Apricot Seed (Xingren, 杏仁) and Ephedra Herb (Mahuang, 麻黄) was given for twenty doses, but the condition did not improve, so he left. Two other physicians worsened the condition, and she sought him out again. He said: "My skills are exhausted; let me think." Suddenly realizing that roaring phlegm belongs to the Taiyin Lung meridian, he noted that the Lung is a clear and empty organ, with six lobes and two ears, hanging down like a canopy. Now, sticky phlegm is fixed in the lung fissures, producing roaring sounds during respiration. Since the Stomach governs reception and the Spleen governs transformation, now the Stomach receives but the Spleen fails to transform, causing fluid retention and phlegm formation. This condition requires a drastic "plundering" agent. He presented the Sanbai Wan (Three Whites Pill) formula: using Arsenic Trioxide (Baibi, 白砒) three fen calcined until yellow, Fritillaria Bulb (Beimu, 贝母), and Platycodon Root (Jiegeng, 桔梗) each three fen, pounded with rice paste into pills the size of mustard seeds. Seven pills were taken with cold tea before sleep. After seven doses, the phlegm stopped and roaring ceased. She recovered after taking Spleen-regulating and Lung-clearing medicines. Generally, when a disease is critical to this extent, only drastic plundering agents can cure it. This demonstrates mastery of the Seven Formulas and Ten Dosages. Ancients had deep learning; modern practitioners have shallow learning. Only by considering the use of wild, wolf-like, tiger-like plundering agents can one reach true expertise. One must combine odd and standard approaches. How can simple paths exhaust the skills of Qibo and Huangdi? If the insight is true, plundering agents may be temporarily used. Fermented Soybean (Dansudou, 淡豆豉) one liang and Arsenic Trioxide (Baibi, 白砒) one qian ground into powder, mixed with three qian of pounded rice into pills the size of radish seeds. Take seven pills with warm water. Extremely effective for Cold Asthma. (Wentian notes: The formula attached by Mr. Wang can treat conditions with deeply rooted and stubborn phlegm-saliva obstruction, and I have seen its efficacy frequently. It should not be used unless absolutely necessary. Organs and viscera may be damaged by it, potentially leading to hemoptysis later. Caution is essential.)
