Xu Zhongguang treated a child with smallpox. On the fifth day, the pustules were evenly distributed, clear, and protruding, but their color was tender and bright red, indicating weak spleen and stomach qi (Spleen and Stomach Qi). By the time the pustules were full of pus, they still lacked a tough and aged appearance, so one must still guard against complications such as diarrhea and pustule collapse with itching.
He prescribed Baoyuan Tang (Preserve the Origin Decoction) decocted in loess water, combined with Shenling Baizhu San (Ginseng, Poria, and Atractylodes Macrocephala Powder) to support the spleen. Solidifying the earth element can prevent the sinking of middle qi (Middle Qi Sinking). However, ordinary physicians, because it was just the beginning of the pus-filling stage, feared that the damp-draining properties of Ginseng and Poria would be harmful, and only used Baoyuan Tang for regulation. By the eighth day, the child had diarrhea four or five times, followed by inverted scabs (Inverted Scab), irritability, and died on the twenty-sixth day. In my experience, this type of smallpox often leads to death by the twelfth day, and sometimes by the fourteenth day.
Another child presented with similar symptoms. Following this treatment method, by the ninth day, the pus was clear and scabs formed, resembling bran. Fortunately, he could eat and his bowels were regular; he later developed an abscess/toxin but recovered.
Another child, five days into smallpox, had tender and bright red pustules. Some said this was "Huang Dou" (Shifting Smallpox), which cannot become tough and form scabs. Upon examination, the skin was white and tender, thin-skinned and bright red, suggesting no major danger. He was treated with Baoyuan Tang plus Guigui (Cinnamon Twig) and Baizhu (Atractylodes Macrocephala Rhizome) and recovered.
A one-year-old girl suffered from smallpox with tender redness. Initially, she vomited and had loose stools. Her body showed even and bright pustules, except around the mouth where they were densely clustered. On the first day, Qian Hu (Peucedani Radix), Jie Geng (Platycodonis Radix), Jiang Can (Bombyx Batryticatus), Chen Pi (Citri Reticulatae Pericarpium), Gan Cao (Glycyrrhizae Radix et Rhizoma), Chuan Xiong (Ligustici Chuanxiong Rhizoma), Mu Tong (Akebiae Caulis), Shan Zha (Crataegi Fructus), and Qiang Huo (Notopterygii Rhizoma et Radix) were used. On the second day, Qiang Huo and Chen Pi were removed from the previous formula, and Fang Feng (Saposhnikoviae Radix), Hong Hua (Carthami Flos), Lian Qiao (Forsythiae Fructus), and Gan Ge (Puerariae Lobatae Radix) were added. On the third day, Jie Geng, Chuan Xiong, Jiang Can, Dang Gui (Angelicae Sinensis Radix), Chen Pi, Gan Cao, Shan Zha, Qian Hu, Hong Hua, Dan Pi (Moutan Cortex), Lian Qiao, and Sheng Di (Rehmanniae Radix) were used. On the fourth day, Shan Yao (Dioscoreae Rhizoma), Fu Ling (Poria), Gui Shen (Angelicae Sinensis Radix), Chuan Xiong, Gan Cao, Bai Zhi (Angelicae Dahuricae Radix), Feng Fang (Vespa), Jiang Can, Jie Geng, Chen Pi, and 4 fen of Ren Shen (Ginseng Radix et Rhizoma) were used. On the fifth day, the same formula was used, but Ren Shen was increased to 6 fen. On the sixth day, Huang Qi (Astragali Radix), Dang Gui, Fu Ling, Bai Zhu, Bai Zhi, Chuan Xiong, Feng Fang, Jie Geng, Gan Cao, and 6 fen of Ren Shen were used, with Sang Chong Wine taken at night. On the seventh day, 9 fen of Ren Shen, Huang Qi, Bai Zhi, Dang Gui, Chuan Xiong, Gan Cao, Shan Jia (Manis Squama), and 2 fen of Guigui were used. At night, diarrhea occurred, eyes opened with choking and vomiting, initially expelling white foam, with panting and abdominal distension alternating. The previous formula was then modified with 1 qian of Ren Shen, 5 fen of Rou Dou Kou (Myristicae Semen), 3 grains of Sha Ren (Amomi Fructus Rotundus), 3 fen of Fu Zi (Aconiti Lateralis Radix Praeparata), and 1 qian each of Bai Zhu and Huang Qi, decocted and administered. The child became lethargic with phlegm rattle, experienced convulsions for a long time, and after about two to three shichen (hours), profuse sweating occurred all over the body, and voice returned. Milk was then given, the pustules became bright, flat areas suddenly rose, and pus formation began. On the eighth day, Ren Shen, Huang Qi, Bai Zhi, Bai Zhu, Hong Hua, Jiang Can, Dang Gui, Chuan Xiong, Chen Pi, Gan Cao, Guigui, and 2 fen of Rou Dou Kou were used; diarrhea stopped once pus was sufficient. On the ninth day, 0.2 qian of Ren Shen, Bai Zhi, Bai Zhu, Shan Yao, Guizhi (Cinnamomi Ramulus), Jiang Can, Gan Cao, Rou Dou Kou, and 2 fen of Bai Fu Zi (Typhonii Rhizoma) were used. There was distension in the head, face, chest, and abdomen, with only half of the calves having bloody scabs. On the tenth day, Ren Shen, Bai Zhi, Dang Gui, Bai Shao (Paeoniae Radix Alba), Chen Pi, Gan Cao, Wu Wei Zi (Schisandrae Chinensis Fructus), Yi Yi Ren (Coicis Semen), Shan Yao, and Fu Ling were used. Subsequently, a mild tonifying agent was used for regulation, leading to recovery.
Wan Mizhai treated the son of the Hu family, whose cheeks were red and dry before the eruption of smallpox. He stated: "The Shanghan Lun (Treatise on Cold Damage Disorders) says that continuous facial redness indicates Yangming heat (Yangming Heat Syndrome). If not resolved in advance, when the smallpox erupts, this area will be very dense and red, making it difficult to heal. He prescribed Shengma Gegen Tang (Kudzu Root and Cimicifuga Decoction) plus Fang Feng, Niu Bang Zi (Arctii Fructus), and Lian Qiao. After three doses, the redness disappeared completely, and the smallpox eruption was sparse."
The son of the Wang family, five years old, had dense and bright red smallpox. This is a critical condition, but since qi is substantial and blood is hot, it is treatable. He used Dang Gui Shao (Tip of Angelicae Sinensis Radix), Chi Shao (Paeoniae Radix Rubra), Sheng Di, Jing Jie (Schizonepetae Spica), Fang Feng, Gan Cao, Jie Geng, Lian Qiao, Niu Bang Zi, Qing Pi (Citri Reticulatae Pericarpium Viride), Jie Geng, and Zha Rou (Crataegi Fructus). After fifteen days of regulation, scabs formed.
Fei Jianzhong treated the son of the Zang family. The smallpox was very dense and plump, the color was not dry or stagnant, but merely bright red, and the body was too hot. Two doses of Qingre Jiedu Tang (Clear Heat and Detoxify Decoction) were given. Bowel movement occurred once every five days, appetite was absent, but by the next day the pus matured, and the red halo turned crimson. Three days later, there was no bowel movement, raising concern for Huo He Zheng (Fire-Brown Syndrome, characterized by dark, dry pustules). Urgent moistening was required to loosen the toxin. Sheng Di and Shi Gao (Talcum) were added to the previous formula, along with Huang Qin (Scutellariae Radix). Before the medicine was fully prepared, the child became dry and itchy, so it was administered immediately. Old feces, extremely foul and hard, were expelled, followed by refreshment and deep sleep. The red patches gradually faded and receded, and appetite greatly improved. Recovery was achieved using Rendong Jiedu Tang (Honeysuckle Detoxification Decoction).
