(The preceding content) was not recorded in full. Only Miao advocated using Baihu Tang (White Tiger Decoction) to treat Summerheat syndromes, yet mixed herbs of the Yin aspect such as Achyranthis Bidentatae Radix (Niuxi, 牛膝) and Polygoni Multiflori Radix (Shouwu, 首乌) into the treatment from the onset of the disease. These practices cannot serve as standards. The term "Fetal Malaria" (Tai Nue, 胎疟) is even more of a vulgar notion; that even Wei, a virtuous scholar, was confused by it shows how shallow this view is.)
Pei Zhaoqi treated a patient who had suffered from malaria for a long time without resolution. Upon attack, the patient would invariably experience dryness and thirst, drinking copiously, followed by vomiting, hiccups, and abdominal distension, with a sallow and emaciated complexion. This is a sign of malaria caused by excessive Dampness. The physician failed to examine the condition carefully and prescribed Buzhong Yiqi Tang (Tonify the Middle and Augment the Qi Decoction) supplemented with Anemarrhenae Rhizoma (Zhimu, 知母), Ophiopogonis Radix (Maidong, 麦冬), Mume Fructus (Wumei, 乌梅), and Pseudostellariae Radix (Huafen, 花粉) daily, which only exacerbated the condition. Pei prescribed pills made of Atractylodis Rhizoma (Cangzhu, 苍术) three taels, and Pinelliae Rhizoma Preparatum (Banxia, 半夏), Poria (Fuling, 茯苓), Alismatis Rhizoma (Zexie, 泽泻), Magnoliae Officinalis Cortex (Houpo, 浓朴), Citri Reticulatae Pericarpium (Chenpi, 陈皮), Amomi Fructus (Sharen, 砂仁), and Coptidis Rhizoma (Huanglian, 黄连) each five qian. Each dose was two qian, taken twice daily. After more than ten days, the patient recovered. Why does thirst increase when taking moistening agents, but cease when taking drying agents? It is because draining Dampness achieves the effect of moistening. In cases of chronic malaria, there are often yellow phlegm and foul fluids accumulating in the Middle Jiao (Central Palace/Spleen-Stomach). When Dampness in the Middle Jiao is severe, Heat is generated; when Heat is severe, Dryness arises, leading to thirst and excessive drinking. By draining the Dampness, Heat is not generated; once Heat is absent, Dryness naturally resolves. The ancients did not deceive us.
Huang Jinfang treated a woman surnamed Luo for malaria. The attacks occurred every night when it was quiet, with more heat than chills. Her diet remained normal, she felt thirsty but did not vomit, and there was no sweat upon the subsidence of fever. During attacks, she spoke deliriously, and her pulse was extremely withered and choppy. He determined that the pathogenic factor had entered the Blood aspect. If the pathogen were not drawn out from the Yin, it would inevitably lead to the exhaustion of Yin fluids, resulting in an incurable state. He used Angelicae Sinensis Radix (Danggui, 当归) two qian, Chuanxiong Rhizoma (Chuanxiong, 川芎) five fen, Prepared Rehmanniae Radix (Shudi, 熟地) two qian, Anemarrhenae Rhizoma (Zhimu, 知母) one qian (stir-fried with wine), Carthami Flos (Honghua, 红花) five fen, and Cimicifugae Rhizoma (Shengma, 升麻) three fen. This was decocted and administered before the onset of the attack. After four or five doses, she was cured.
Lu Liuxi was robust and vigorous, having never been ill. Due to worry and overwork, he contracted malaria. His food intake decreased, muscles wasted away, his body felt heavy and fatigued, and he frequently belched. His symptoms fluctuated, being mild one day and severe the next, lingering for three months, causing him great suffering. This was caused by irregular diet and improper rest/exertion, injuring the Yangming Stomach Meridian. Decreased appetite and hard, difficult bowel movements indicate that stomach disease has slowed down the mechanism of transportation and transformation. Muscle wasting indicates that the Stomach governs the muscles. Bodily fatigue indicates that stomach disease has impaired the mechanisms that constrain the body. Frequent belching indicates disharmony in the Stomach, manifesting as a sensation of obstruction. As for the fluctuation between mild and severe days, this is also a manifestation of the Yangming Stomach Meridian. The Classics state: "Yangming disease involves aversion to people and fire, and hearing the sound of wood causes sudden fright." It also states: "Yangming disease likes fire, and likes sunlight and moonlight." This reflects the subtle meaning of alternating excess and deficiency, consistent with the principle of being milder during the day and heavier at night. Initially, when the disease began, the pathogenic factor was in excess; thus, aversion to people, fire, and wooden sounds stemmed from their perceived ability to aid the pathogen. Later, when the disease had lasted long, the pathogen had receded but the healthy Qi was also deficient; thus, liking fire and sunlight stemmed from their perceived ability to support the healthy Qi. Jia, Bing, Wu, Geng, Ren are the Yang days of heaven and earth; Yi, Ding, Ji, Xin, Gui are the Yin days. With prolonged malaria and reduced eating, the healthy Qi in the Stomach is already deficient, while the pathogen is not yet fully cleared. Therefore, on Yang days, the healthy Qi is assisted by heavenly Yang, so the pathogen cannot overcome it, resulting in milder symptoms; on Yin days, the pathogen is assisted by heavenly Yin, so the healthy Qi cannot overcome it, resulting in heavier symptoms. The current critical point lies entirely in cultivating the Middle Qi. Although there is only one Stomach, it is divided into the Upper, Middle, and Lower Guan (orifices/sections). The Upper Guan corresponds to Heaven, containing mostly clear Qi; the Lower Guan corresponds to Earth, containing mostly turbid Qi; and the ability to ascend the clear and descend the turbid relies entirely on the Middle Guan for transportation and transformation. In this patient, the turbid Qi of the Lower Guan should normally descend, but transmission into the intestines is difficult. It should not ascend, yet it easily rises to the chest because the Middle Guan has historically been injured by dietary factors and cannot block the turbid Qi of the Lower Guan from invading the Clear pathways. Therefore, if the Qi of the Middle Guan is prosperous, the clear Qi of water and grain ascends to the Lung and irrigates all vessels. The turbid Qi of water and grain descends to the Large Intestine and is eliminated through urination and defecation. How could there be any sensation of chest obstruction? The previously used Liuwei Dihuang Wan (Six-Ingredient Pill with Rehmannia) consists of cloying and stagnant medicines, which are highly unsuitable for stomach disease. I have now revised the prescription to Lizhong Tang (Regulate the Middle Decoction), which correctly ascends the clear and descends the turbid.
