Zi Hezi treated a woman suffering from dry and constipated stool, dysuric and astringent urine, and infertility for half her life. She frequently took purgative herbs to promote bowel movement; when she did so, her stool was unobstructed, but upon cessation, constipation returned. She suddenly became pregnant. By the fourth month, physicians prohibited the use of purgatives, yet her stool remained difficult. During defecation, excessive straining caused miscarriage; this occurred three times. In another instance, at three or four months of pregnancy, preceded by previous episodes of constipation and astringency, she assumed the fetus would be lost. Zi Hezi palpated her pulse, finding it slippery in both hands, but dared not abruptly apply purgative methods. Instead, he employed dietary therapy: boiling spinach and okra (with "Jian" likely being an error, retained as per *Rumen Shiqin*), using shepherd's purse seedlings as vegetables, and mixing pig and sheep blood into a soup. Half a year later, she successfully delivered a child, and the dryness disease also healed. He frequently observed pregnant women passing pus and blood with severe tenesmus damaging the fetus; those cured by the aforementioned method were countless.
Xue Lizhai treated the concubine of Li Puting, who during pregnancy suffered from difficulty in urination and defecation. Others used formulas to clear heat and regulate qi, which instead depleted her primordial qi. Her liver pulse was wiry and urgent, and her spleen pulse was slow and sluggish. Observing her face was greenish-yellow and lusterless, Xue said, "This is caused by depression and anger." He used Jiawei Guipi Tang (Modified Spleen-Restoring Decoction) as the main formula, assisted by Jiawei Xiaoyao San (Modified Free Wanderer Powder), and the condition stabilized.
The wife of Wang Tiancheng, a Chief Magistrate, suffered from gestational dysentery. After recovery, she experienced obstruction of both urination and defecation. Her family, consisting of generations of physicians, self-administered heat-clearing agents without effect. Palpating her pulse, it was floating, large, and choppy, indicating deficiency of qi and blood. In the morning, he prescribed Bazhen Tang (Eight-Treasure Decoction) with added Tao Ren (Peach Seed) and Xing Ren (Apricot Seed); in the evening, Jiawei Xiaoyao San with Che Qian Zi (Plantago Seed). She recovered.
Lu Yangyu treated a woman at nine months of pregnancy who had no passage of stool or urine for three days. Suddenly, fetal gas rushed upward to the heart, causing several fainting spells. Palpation revealed a flooding and full pulse. Lu stated that purgation was necessary and administered Dachengqi Tang (Major Order the Qi Decoction) for one dose, with slight additions of Mu Xiang (Aucklandia Root) and Dou Ren (Cardamom Seed). A village physician, seeing the use of approximately one liang of Da Huang (Rhubarb Root and Rhizome), shook his head and stuck out his tongue in astonishment. The husband looked troubled. Lu said, "I will sit in your house until she survives before I leave." They then安心ly decocted and administered the medicine. After about one to two hours, both urination and defecation occurred, expelling a large amount of black feces, and the fetus remained unharmed. Lu left two doses of Tiaoqi Yangrong Tang (Regulate Qi and Nourish Blood Decoction), which were not taken. Several days later, due to difficulty in urination, they decocted and took it, leading to recovery. Over a month later, she delivered a male infant.
Chen Sansan treated a woman at five months of pregnancy with obstruction of stool and urine, chest and abdominal distension, and unbearable stabbing pain in the legs, feet, and abdomen. He used Xiong (Ligusticum Chuanxiong Rhizome), Gui (Angelica Sinensis Root), Chi Shao (Red Peony Root), Zhi Ke (Bitter Orange Fruit), Bing Lang (Areca Seed), Mu Tong (Akebia Stem), Hua Shi (Talcum), Xing Ren (Apricot Seed), Cong Bai (Scallion White), along with one zhong of Tongbian (Child's Urine) and water, decocted to eight-tenths volume. Two qian of Da Huang powder and two qian of Che Qian Zi powder were added, boiled again, mixed with four or five spoons of honey, and taken warm. Both urination and defecation became smooth, and she was relieved.
Nie Jiuwu said: A physician came asking, "I treated a woman at eight or nine months of pregnancy who suddenly had no passage of stool or urine and severe abdominal distension. I used Chengqi Tang to purge, but there was still no passage. The situation is now critical." This cannot be resolved by ordinary decoctions. I instructed him to use Qianni Dahuang Wan (Pharbitis and Rhubarb Pill). After taking approximately one liang, both urination and defecation resumed. The next day, her husband came to thank me and mentioned, "All ailments are removed, but during urination, someone must press the lower abdomen tightly with their hand; otherwise, urination is impossible." I reasoned that qi was still obstructed and prescribed Qingpi (Green Tangerine Peel) and Xiangfu (Cyperus Rhizome) among other qi-regulating herbs. One dose led to recovery. Over a month later, a boy was born, and mother and child were safe. (Formula see Vomiting Section.)
Shen Yaofeng said: Previously, Zhu Danxi treated a pregnant woman with urinary retention. He instructed a woman to apply sesame oil to her hands, insert them into the birth canal, and lift the fetus. Urine flowed out like a stream. Subsequently, he administered a large dose of Ginseng, Astragalus, and Cimicifuga decoction. He also treated a woman with cystoptosis (retroverted bladder causing urinary retention) using Ginseng and Angelica decoction, curing her through induced vomiting. Wang Ren'an recorded this formula as Shen Zhu Yin (Ginseng and Atractylodes Drink), consisting of Dang Gui (Angelica Sinensis Root), Shu Di (Prepared Rehmannia Root), Chuan Xiong (Ligusticum Chuanxiong Rhizome), Shao Yao (Peony Root), Ren Shen (Ginseng Root), Bai Zhu (Atractylodes Macrocephala Rhizome), Chen Pi (Citrus Reticulata Peel), Ban Xia (Pinellia Tuber), Zhi Gan Cao (Honey-fried Licorice Root), added with fresh ginger, decocted and taken on an empty stomach. Zhu Danxi discussed that cystoptosis occurs frequently in women with weak constitution, those prone to worry and depression, those with irritable temperaments, and those consuming rich flavors. Ancient formulas using slippery/lubricating herbs rarely showed efficacy. He reasoned that the bladder does not rotate on its own but is compressed by the fetus; if the bladder is lifted, the water pathway opens naturally. Recently, a favored concubine in the Wu household suffered from this. Her pulse was slightly choppy, becoming wiry under heavy pressure. I said, "This is caused by anxiety and worry." Choppy indicates blood deficiency and excess qi; wiry indicates fluid retention. Blood deficiency makes the fetus weak and unable to be supported; excess qi with fluid retention causes instability in the middle jiao and overflow, leading the fetus to descend. Thus, I gave the aforementioned medicine and decoction, followed by induced vomiting. After the qi settled, I administered more medicine, and she stabilized. This may have been accidental success, but subsequent treatments of several patients were all effective.
