Official Wu fell from a horse and injured his head. Due to excessive blood loss, he experienced fever, irritability, muscle twitching, and tendon tremors. Some suggested administering medicines for tetanus (Po Shang Feng). I said: This is caused by blood deficiency leading to the stirring of deficient fire. The best approach is to vigorously tonify the blood. Thus, Shengyu Tang (Saint's Life Decoction) was used. After two doses, the condition stabilized, and subsequent nourishment of Qi and Blood led to recovery.
A man suffered an arm injury with excessive blood loss, followed by erroneous purgation. This resulted in incessant irritability and heat, and non-healing, necrotic flesh. Shengyu Tang was administered; after four doses, there was slight relief. Then, Bazhen Tang (Eight Treasures Decoction) with Schisandrae Fructus (Wuweizi) and Ophiopogonis Radix (Maidong) brought stability. Subsequently, Liujunzi Tang (Six Gentlemen Decoction) with Chuanxiong Rhizoma (Chuanxiong), Angelicae Sinensis Radix (Danggui), and Astragali Radix (Huangqi) was used. After four doses, the ulcer opened and drained; after more than twenty doses, it closed and healed. Generally, such cases require distinguishing the severity of the illness, the presence or absence of blood stasis, and the deficiency or excess of Yuan Qi (Original Qi). One must not indiscriminately use purgatives, lest damage to Qi and Blood make opening and closing of ulcers difficult. Often, treatment begins with drinking child's urine mixed with wine, or adding Carthami Flos (Honghua) and Sappan Lignum (Sumu), which yields very rapid results. If aggressive draining agents are used indiscriminately, errors are frequent. The speed of ulcer healing depends on the deficiency or excess of Qi and Blood.
Academician Zhang fell from a horse in late autumn, suffering excessive blood loss, sweating, and irritability. The next day, his sweating stopped spontaneously, but the heat and restlessness worsened, accompanied by trismus (lockjaw) and hand tremors. This indicates Yin Blood deficiency, allowing Yang Fire to invade, causing sweating. However, cold air constricts the Couli (pores/skin texture), preventing sweat release and trapping Fire internally, causing severe stagnation and exacerbating other symptoms. (In all diseases where abundant Fire causes sweating, abruptly stopping the sweat will only increase other symptoms.) Therefore, Siwu Tang (Four Substances Decoction) with Bupleuri Radix (Chaihu), Scutellariae Radix (Huangqin), and Gardeniae Fructus (Shanzhi) was used. After four doses, symptoms subsided slightly. Then, Siwu Tang with Ginseng Radix et Rhizoma (Renshen), Atractylodis Macrocephalae Rhizoma (Baizhu), soft Bupleuri Radix (Ruan Chaihu), Schisandrae Fructus (Wuweizi), and Ophiopogonis Radix (Maidong) was used, leading to complete recovery.
A woman injured her foot in early winter, resulting in blood loss, sweating on the head, internal heat with thirst, shortness of breath, irritability, and periodic mental confusion. Her pulse was surging and large but weak upon deep pressure. This signifies Yin Blood deficiency below and solitary Yang flaring above, causing Jue (cold extremities/fainting) and head sweating. Siwu Tang combined with Xiaochaihu Tang (Minor Bupleurum Decoction) was administered; after one dose, sweating stopped. Then, Siwu Tang without Chuanxiong Rhizoma, plus Ginseng Radix et Rhizoma (Renshen), Atractylodis Macrocephalae Rhizoma (Baizhu), Ophiopogonis Radix (Maidong), Schisandrae Fructus (Wuweizi), and Glycyrrhizae Radix et Rhizoma Preparata (Zhigancao), with a small amount of Cinnamomi Cortex (Rougui), was used. After four doses, all symptoms disappeared. After more than thirty doses, Qi and Blood were restored, and she recovered.
A man fractured his leg in early summer, suffering excessive blood loss. Initially, he experienced dizziness and blurred vision, later progressing to mental confusion. This was due to damaged Yin Blood and blazing Yang Fire. The Lung Metal failed to control Liver Wood, causing Liver Wood to flourish and generate Wind. Child's urine was urgently administered. Additionally, Ginseng Radix et Rhizoma (Renshen) and Angelicae Sinensis Radix (Danggui) each 5 qian, Schizonepetae Spica (Jingjie), Chuanxiong Rhizoma (Chuanxiong), Bupleuri Radix (Chaihu), Paeoniae Radix Alba (Baishao), and Atractylodis Macrocephalae Rhizoma (Baizhu) each 2 qian, Gardeniae Fructus (Shanzhi), Scutellariae Radix (Huangqin), and Platycodonis Radix (Jiegeng) each 1 qian, and Glycyrrhizae Radix et Rhizoma (Gancao) 0.5 qian were given. He felt refreshed immediately thereafter. Then, Siwu Tang with Ginseng Radix et Rhizoma (Renshen) and Atractylodis Macrocephalae Rhizoma (Baizhu) each 3 qian, Rehmanniae Radix Preparata (Shengdi), and Bupleuri Radix (Chaihu) each 1 qian were used. After four doses, irritability completely resolved.
