After childbirth, the body is fatigued and exhausted, and both the Spleen and Stomach are injured. Therefore, after new delivery, one should avoid rich and greasy foods, stay away from strong flavors, consume congee and vegetables, and use these as methods of regulation. Those who do not understand only worry about postpartum deficiency, considering rich flavors to be nourishing. Furthermore, there are cases where food is forced upon someone who has no appetite; although the Stomach may accept a small amount, the Spleen finds it difficult to transform and transport, easily leading to food stagnation and epigastric fullness/stuffiness, and even fever due to food injury. Do not mistakenly identify this as Yin Deficiency. In cases of abdominal pain caused by food stagnation, do not wrongly suspect blood stasis. To treat such conditions, one must carefully ascertain the reality (excess/deficiency) of the syndrome. If there is fever but dietary intake is normal, then blood tonification can be performed. However, if there is aversion to food and sour belching, these are signs of food stagnation; one should immediately focus on supporting the Yuan Qi (Original Qi), tonifying Qi and Blood, strengthening the Spleen and assisting the Stomach. Determine what food was injured and supplement with appropriate herbs for digestion and guiding out stagnation. Thus, when Spleen Qi recovers and transformation becomes easy, stagnant matter moves, and the Stomach desires grain. With such combined treatment of tonification and purgation/digestion, there will be no instability. Therefore, skilled practitioners distinguish treatment methods by priority and urgency, differentiate syndromes based on the presence or absence of mass-pain, and consider the duration since delivery. They can emphasize the root of postpartum deficiency while simultaneously eliminating the branch of accumulated food. If attacking excess and tonifying deficiency are done according to method, recovery is assured. Hence, there are distinct treatments using Shenghua Xiaoshi Tang (Life-Generating Food-Dissolving Decoction) and Jianpi Xiaoshi Tang (Spleen-Strengthening Food-Dissolving Decoction). If harsh medicines are mistakenly taken, causing excessive digestion and leading to loss of appetite, only Huoming Dan (Life-Saving Pill) can save the patient. I often see practitioners treating postpartum conditions who only know how to quickly eliminate food accumulation, thereby damaging Zheng Qi (Upright Qi) and increasing bloating and stuffiness. When one dose is ineffective, harsher drugs are added; when one physician fails, another is consulted. Methods vary sequentially, mild conditions become severe, leading to death. The family attributes it to fate, and the physician believes they have exhausted their skills. Is this not lamentable?
Shenghua Xiaoshi Tang (Life-Generating Food-Dissolving Decoction): Treats postpartum abdominal mass-pain that has not ceased, and food stagnation causing epigastric fullness/stuffiness.
Chuanxiong Rhizome (Chuanxiong Radix) (2 qian) Danggui Radix (Danggui, 5 qian, stir-fried with wine) Dried Ginger (Ganjiang, charred black) Prepared Licorice Root (Zhigancao, 4 fen each) Peach Kernel (Taoren, 10 grains, skin and tip removed) Medicated Leaven (Shenqu, stir-fried) Malt (Maiya, stir-fried, 6 fen each)
Decoct in water and take orally. For meat injury, add Hawthorn Fruit (Shanzha, 5 fen) and Amomum Fruit (Sharen, 5 fen). For cold-damaged food, add Evodia Fruit (Wuzhuyu, 1 qian) and Cinnamon Bark (Rougui, 5 fen). For deficient persons, add Ginseng Root (Renshen, 1 qian, or 2 qian).
Jianpi Xiaoshi Tang (Spleen-Strengthening Food-Dissolving Decoction): Treats postpartum abdominal mass-pain that has ceased, and food stagnation causing epigastric fullness/stuffiness.
Ginseng Root (Renshen, 2 qian) Danggui Radix (Danggui, 3 qian, washed with wine) Chuanxiong Rhizome (Chuanxiong) Medicated Leaven (Shenqu, stir-fried, 1 qian each) Atractylodes Macrocephala Rhizome (Baizhu, 1.5 qian, stir-fried with soil) Hawthorn Fruit (Shanzha) Amomum Fruit (Sharen, 6 fen each) Malt (Maiya) Prepared Licorice Root (Zhigancao, 5 fen each)
Decoct in water and take orally. If cold stagnation has persisted for a long time and the Spleen and Stomach are extremely weak, even medication may not immediately facilitate transformation; manual rubbing and pressing should be used. Alternatively, applying stir-fried bran leaven externally with heat is also excellent.
Changsheng Huoming Dan (Long-Life Life-Saving Pill): Treats loss of appetite due to overuse of digestive/purgative herbs, or cases where a person who originally desired food instead experiences total anorexia.
Ginseng Root (Renshen, 3 qian)
Use a new jar or silver pot, half zhong of water, decoct until seven-tenths remain. Then, grind the scorched rice from the bottom of a rice cooker into powder. First, take a small amount, mixed with the Ginseng Decoction, administered as two or three tea spoons. After the appetite is stimulated, gradually increase the dosage, gradually adding more Ginseng Decoction, and administer successively. Do not use medicinal jars to decoct Ginseng, lest the smell of medicine induce vomiting. This formula has saved dozens of lives and is miraculously effective.
