In the treatment of *Gu Zheng* (Bone-steaming, a type of consumptive disease characterized by deep-seated heat in the bones), when the pathogenic heat is deeply rooted within the interior, all medicines with light and ascending properties are strictly contraindicated. If such ascending and dispersing agents are used, they will instead guide the heat outward, exacerbating the intense burning sensation, depleting body fluids, and causing emaciation of muscles. Under such circumstances, it is impossible even to stop the internal heat, let alone expect the fever to subside and the disease to cease. In cases of Kidney Water deficiency and Blood dryness, leading to the boiling of Yin Fire (deficiency fire generated from Yin deficiency), *Jiajian Yiyin Jian* (Modified One-Yin Decoction) added with *Qinghao* (Artemisiae Annuae Herba) is indicated. If essence and blood remain stagnant in the meridians, obstructing Yang Qi and generating heat, while accumulating Yin Qi to form *Zhu* disease (a chronic wasting condition), prolonged steaming leads to dried blood; in such cases, stasis should be removed promptly, and *Maijian San* (Wheat Powder Decoction) is indicated. Furthermore, Wind pathogens attaching to the bones can also cause steaming heat and night sweats; *Qinjiao Biejia San* (Gentian and Turtle Shell Powder) or *Chaihu Meilian Wan* (Bupleurum, Plum, and Coptis Pill) are indicated. Additionally, when Yang pathogens enter the bones and cannot be vented, presenting with initial chills followed by fever, a long pulse, and sweating, *Shigao* (Gypsum Fibrosum) is indicated.
Consumptive diseases complicated by spermatorrhea or seminal emission are like vessels with holes at the bottom, making them extremely difficult to regulate and treat. This may be due to the agitation of Jun Fire (Heart Fire), the exuberance of Xiang Fire (Ministerial Fire), insecurity of the Yu Men (external genitalia), disharmony between Heart and Kidney, Qi failing to secure the essence leading to involuntary sliding and loss, or exhaustion of Yuan Yang (Original Yang) causing the closure mechanisms to fail and chaotic flow of fluids. Urgent introspection and internal nourishment are required, combined with herbal regulation. Only by completely eradicating the root cause can the primary disease be cured.
When Jun Fire is unclear, the Spirit agitates above and the Essence agitates below. In cases of severe Fire, *Er Yin Jian* (Two-Yin Decoction) or similar formulas should be used first to clear Heart Fire. In cases where Fire is not severe, *Baizi Yangxin Wan* (Seed-of-Arborvitae Pill for Nourishing the Heart) or *Tianwang Buxin Dan* (Emperor of Heaven’s Heart-Nourishing Pill) should be used to nourish and gather Heart Qi, followed by astringent medications to secure the essence. Xiang Fire is prone to movement, often associated with Heat in the Liver and Kidneys, leading to easy leakage; *Jingyan Zhudu Wan* (Experience Pig-Stomach Pill) is considered the most effective, or *Gu Jing Wan* (Sperm-Consolidating Pill). However, one must assess the severity of the Fire; if clearing is appropriate, the Fire must be cleared first. For insecurity of the Yu Men, *Lingzhu Tusisi Wan* (Poria, Atractylodes, and Dodder Seed Pill), *Shuilu Erxian Dan* (Land-and-Water Two-Immortal Pellet), *Jinsuo Zhengyuan Dan* (Golden Lock Preserving-Origin Pellet), or *Jinsuo Sixian Dan* (Golden Lock Thinking-Immortal Pellet) are indicated. For disharmony between Heart and Kidney, *Kanli Jiaoji Dan* (Kan-Li Mutual-Interaction Pellet) is indicated. For Qi failing to secure the essence, *Mi Yuan Jian* (Secret-Origin Decoction), *Juyuan Jian* (Raise-Origin Decoction), *Shoupi Jian* (Spleen-Sustaining Decoction), or *Shiquan Dabu Tang* (Complete Ten-Tonifying Decoction) are indicated. For insufficiency of Yuan Yang, *Yougui Wan* (Right-Restoring Pill), *Bawei Wan* (Eight-Ingredient Pill), *Jiajiuzi Wan* (Chinese Chive Seed Pill), or *Gushen Wan* (Kidney-Fortifying Pill) are indicated.
