【大论】产后血虚,腠理不密,故多汗,因遇风邪搏之则变痉,痉者口噤不开,背强而直,如发间状,摇头马鸣,身反折,须臾十发,气息如绝,宜速撬口灌小续命汤,稍缓即汗止,如两手摸控者,不可治也;
[General Treatise] Postpartum wind-stroke arises from the injury to Qi and Blood during childbirth, damaging the Zang-Fu organs. If one rises and labors too early before recovery, Qi deficiency occurs, allowing Wind pathogen to enter through the deficiency, thus causing wind-stroke. The Cold-Wind pathogen lodges in the skin and collaterals, manifesting as pain, bi-syndrome (obstruction syndrome), emaciation, weakness, inability to bear labor, and shortness of breath. Generally, if tendons and vessels are combined with Cold, they become spasmodic and deviated; if combined with Warmth, they become flaccid and weak. If the pathogen enters the Zang-Fu organs, it causes mental confusion, fright, and palpitations. Diseases arise according to the specific Zang-Fu or collaterals injured.
Guo Jizhong stated: What is postpartum wind-stroke? The answer is: Within five to seven days after childbirth, forcing oneself out of bed, or within a month engaging in sexual intercourse, or harboring sorrow and anger which disturbs the Chong and Ren harmonious Qi, or eating raw and hard foods that damage the Zang-Fu organs. At the onset, there is dry eyes, trismus (lockjaw), muscle twitching, gradually progressing to stiffness and rigidity of the waist and spine. This condition is incurable. It is caused by human error, not accidental wind-stroke.
[Xue Ji] If the aforementioned symptoms are due to external pathogens, it indicates insufficient Zheng Qi (Upright Qi) and excess Pathogenic Qi. Treatment should primarily tonify Yuan Qi (Source Qi), with slight adjuvants to treat the disease. If one labors excessively or engages in intercourse within a month, both Xing Qi (Form Qi) and Yuan Qi are insufficient; pure tonification of Yuan Qi is required, and revival is often achieved. Misusing Wind-dispelling drugs will accelerate death.
[Zhu Danxi] For postpartum wind-stroke with deviation of the mouth and eye, one must first greatly tonify Qi and Blood, then treat Phlegm. Treatment should be guided by distinguishing the abundance or scarcity of Qi and Blood via the pulses on the left and right hands. One must not treat it as ordinary wind-stroke using Xiao Xuming Tang (Minor Continue Life Decoction) or other exterior-releasing Wind-dispelling drugs.
Fangfeng Tang (Saposhnikovia Decoction): Treats postpartum wind-stroke with stiffness in the back and neck, chest fullness, and shortness of breath.

Preparation: Coarsely crush. Take 5 qian per dose, add 4 sheng water, 5 qian deer meat, and 5 slices of ginger. Decoct until 2 sheng remains, remove deer meat, decoct again until 1.5 sheng remains, dissolve Ejiao, strain, and take warm at any time, twice daily.
[Yun Qizi] Treats postpartum wind-stroke with hemiplegia, slurred speech, mental confusion, forgetfulness, and unsettled spirit.
Ingredients: Duhuo, Danggui, Shaoyao, Fangfeng, Chuanxiong, Xuanshen, Tianma (each 5 qian); Guixin (3 qian).
Preparation: Coarsely crush. Add 8 sheng water, boil down to 2.5 sheng, divide into three doses. Make another dose if effective. Can also be made into pills, 20 pills per dose. Add Gegen (5 qian) for heat; add Baizhu (5 qian) for cold. Add Shengjiang (1.5 liang) for Qi stagnation. Add Niuxi (1.5 qian), Biba (3 qian), Huangqi (4 qian) for limb paralysis. Add Danggui and Shaoyao (each 7.5 qian) for abdominal pain. Add Renshen (5 qian), Xuanshen (1 liang) for anorexia. If Cold invades the Three Yin meridians, use Xiao Xuming Tang with Shengjiang. If Summer-heat invades the Three Yang meridians, remove Fuzi from Xiao Xuming Tang, halve Guixin, and add Bohe.
Baoming Xueqi Tang (Life-Preserving Blood and Qi Decoction): Treats various postpartum wind syndromes, flaccidity, spasms, and weakness.
Ingredients: Qinjiao, Qianghuo, Fangfeng, Baizhi, Chuanxiong, Shaoyao, Danggui, Baizhu, Fuling, Shudihuang (equal parts).
Preparation: Grind into powder. Half make honey pills, half keep as powder. Take 50-70 pills with wine.
For postpartum wind-deficiency, Five Slacks and Six Urgencies, numbness and bi-syndrome in limbs, and disharmony of Qi and Blood:
Take 1 sheng soybeans, fry until cooked, throw into 3 sheng wine while hot, seal with honey, and drink.
Duanju Fang treats postpartum wind-stroke with slurred speech and limb spasms.
Qianghuo (3 liang) ground to powder. Take 5 qian per dose with half water and half wine (0.5 sheng each). Decoct, strain, and take warm.
Huatuo Yufeng San (Huatuo's Wind-Curing Powder): Treats postpartum wind-stroke with trismus, limb convulsions resembling opisthotonos, postpartum blood syncope with unconsciousness, limb rigidity, or vomiting/diarrhea nearing death.
Use Jingjie sui (lightly roasted and powdered). Take 3 qian with Doulinjiu (soybean-leavened wine) or child's urine. If trismus, pry teeth open to administer; if gum lock, insufflate into nose. Effect is miraculous. Generally, postpartum severe vertigo leads to sweating and loose pores, making one susceptible to wind-stroke.
Li Shizhen said: This formula is highly praised. Yaoengtang's Jiyian Fang calls it Rusheng San when taken with wine, claiming immediate effect. Chen's formula is Juqing Gubai San. Xiao Cunjing's formula uses ancient coins decocted in water, called Yinnianjin. Wang Kuang's Zhimi Fang adds Danggui etc., decocted in equal water. Xu Shumi's Benshi Fang states it has miraculous divine efficacy. A woman postpartum slept long, woke confused and unconscious. Doctors used this formula plus Jiaojiao San, advising sleep and scratching head with left hand afterwards, which proved true. Zan Yin's Chanbao states this disease often results from anger injuring Liver, sorrow depressing internally, or catching wind during delivery; urgent administration is needed. Dai Shi's Zhengzhi Yaojue calls it Duxing San. Jia Sidao's Yuesheng Suichao calls it Zaisheng Dan.
For postpartum wind-stroke, using Xuming Tang and Qianghuo dispersing drugs requires careful differentiation of Qi and Blood. Combining Siwu Tang and Sijunzi Tang in halves, adjusting dosage, and taking in divided doses is appropriate.
[Extreme Deficiency Generating Wind]
[General Treatise] What is extreme deficiency generating wind after excessive postpartum bleeding? Answer: Women rely on Ying Blood. Excessive blood loss leaves Qi without mastery, causing cyanotic lips, cold flesh, sweating, vertigo, and coma, with life hanging in the balance. This is extreme deficiency generating wind. Urgently administer Jiwei Shangdan. Using Wind-dispelling drugs here is a mistake.
[Xue Ji] If Heart and Spleen Qi and Blood are deficient, use Shiquan Dabu Tang. If ineffective, add Fuzi and Gouteng. If Liver channel Blood is deficient, use Xiaoyao San plus Gouteng. Classics state: Spleen's luster is in the lips; Heart's fluid is sweat. If Heart and Spleen are extremely deficient, urgently use Shenfu Tang. A woman with these symptoms took various tonics but developed cold limbs, spontaneous sweating, diarrhea, borborygmus, and abdominal pain. I diagnosed Yang Qi deficiency and Cold, using Liujunzi Tang with Jiang and Fuzi increased to 5 qian each. Ineffective, I used Renshen and Fuzi at 1 liang each, which worked. Later, borborygmus and pain returned. I moxaed Guanyuan point over 100 moxibustions and administered Shiquan Dabu Tang, finally effective.
Jiwei Shangdan

Danggui Jianzhong Tang (See Postpartum Abdominal Pain).
[Trismus]
[General Treatise] Postpartum wind-stroke with trismus is due to Qi and Blood deficiency allowing Wind to invade the tendons connecting the jaw and cheeks. Hand Triple Energizer tendons knot at the jaw. Childbirth damages Zang-Fu and tendons. Wind exploits this, pulling the Yang tendons asymmetrically. Tendons contracting from Cold-Wind cause trismus.
Gegen Tang: Treats postpartum wind-stroke with trismus, falling, vertigo, profuse phlegm, and急促 breathing, as well as various postpartum diseases.
Ingredients: Gegen, Shengjiang (each 6 liang); Duhuo (4 liang); Danggui (3 liang, de-gorged); Zhigancao (roasted); Guixin; Baifuling (peeled); Shigao; Renshen (de-gorged); Baizhu; Fangfeng (de-gorged); Chuanxiong (each 2 liang).

Ingredients: Fulonggan (1.5 liang); Ganjiang (0.5 liang, processed).
Preparation: Fine powder. Take 2 qian with warm wine at any time, twice daily.
Jiaojiao San: Treats hundreds of diseases before and after childbirth, including disharmony of Ying and Wei, irregular menstruation, cramping pain, Qi excess/Blood deficiency, mass formation, and postpartum wind-stroke.
Ingredients: Shengdihuang (1 sheng, pressed for juice); Shengjiang (12 liang, pressed for juice).
Preparation: Stir-fry Shengjiang dregs in Shengdihuang juice, and Shengdihuang dregs in Shengjiang juice until slightly dry. Roast to fine powder. Take 3 qian with warm wine. Suitable for ordinary abdominal pain, especially indispensable postpartum.
[Opisthotonos]
[General Treatise] Postpartum opisthotonos is due to body deficiency catching Wind, which enters the Yang meridians. Wind circulates through Yin-Yang collaterals. Entering Yang meridians causes back arching and spasm resembling a bowstring.
[Xue Ji] This syndrome results from Qi and Blood depletion, loose pores, and excessive sweating, indicating a deficiency pattern. Secure Qi and Blood primarily, assisting with the base formula. Danxi said: Postpartum, tonifying Qi and Blood is paramount; other symptoms are secondary. Chills and fever indicate extreme Qi/Blood deficiency. Use large doses of Renshen, Huangqi, Danggui, Baizhu, Rougui to nourish. If ineffective, urgently use Paofuzi. If still ineffective, use Renshen 1 liang, Paofuzi 2-3 qian, named Shenfu Tang. If no response, increase dosage as medicine may not have reached the site.
Yufeng San, Danggui Tang, Qianghuo Jiu (See above).
Lingyangjiao San: Treats postpartum wind-stroke with body arching.
Ingredients: Lingyangjiaoxie, Danggui (each 7.5 qian); Duhuo, Fangfeng (de-gorged); Mahuang (de-joint, 1 liang); Renshen (de-gorged); Chishaoyao, Xixin (de-tips); Guixin (each 0.5 liang).
Preparation: Coarsely crush. Take 8 qian per dose, add 1.5 sheng water and 5 slices Shengjiang. Decoct to 1 sheng, strain, take warm at any time.
Duanju Fang: For postpartum wind-stroke where patients cannot take other drugs, use Yi Wu Duhuo Tang or Yi Wu Baixian Tang, or decoct them together.
Chuan Duhuo (3 liang, finely cut).
Add 3 sheng water, boil to 1 sheng, divide doses. Those tolerating wine can use wine/water mix.
Yi Wu Baixian Tang: Use Baixianpi, following Duhuo method.
Postpartum wind-spasm with trismus, blue face, and rigid limbs:
Use Zhuli (bamboo sap) 2 sheng, divide into 5 doses, take warm frequently. Great effect.
Formula for postpartum wind-stroke, opisthotonos, trismus, and muteness:
Chuanwu (5 liang, chopped bean-sized).
Stir-fry with 0.5 sheng black beans until half-black. Add 3 sheng wine to pan, stir quickly, filter through silk. Take small cup warm to induce sweat. If mouth won't open, pry and administer. If ineffective, add 1 he chicken manure (fried), mix with wine. Continue until cured.
Qianjin Jishi Li: Treats postpartum wind-stroke, men's various winds, and postpartum hundred diseases. Also treats postpartum wind-trismus, severe illness, back rigidity, and frequent arching.
Wuji shi (3 sheng), Da dou (2 sheng).
Fry beans until sound stops, fry chicken manure until yellow. Pour 1 sheng wine over manure, collect juice, pour over beans. Take 1 sheng per dose. Severe cases 4-5 doses. Excellent.
Zhang Wenzhong treats postpartum wind-spasm, whole body cold/stiff, trismus, unconsciousness:
Baizhu (4 liang, finely cut). Decoct in 3 sheng wine to 1 sheng, take in one dose. Effective.
[Sweating Leading to Spasm]
[General Treatise] Postpartum Blood deficiency leads to loose pores and sweating. Meeting Wind pathogen transforms this into Spasm. Spasm features locked jaw, stiff straight back, head shaking, horse-like neighing, body arching, frequent attacks, near cessation of breath. Urgently pry mouth and administer Xiao Xuming Tang. If delayed, sweating stops. If hands feel around blindly, it is incurable.
[Xue Ji] Postpartum spasm results from excessive blood loss, extreme Yuan Qi deficiency, or external pathogen interaction, causing tight jaw, limb rigidity, back arching, and convulsions. With sweat but no aversion to cold is Rou Jing (Soft Spasm); without sweat and with aversion to cold is Gang Jing (Hard Spasm). Postpartum occurrence is truly due to excessive blood loss leaving tendons un-nourished. Thus, excessive sweating/purging in Shanghan or pus/blood loss in ulcers often causes this, a failing sign. Tonifying Qi and Blood largely ensures safety; attacking Wind guarantees death. A patient with tight jaw, back arching, limb convulsions, and eye twitching was diagnosed by me as excessive blood loss, Yuan Qi deficiency, and blazing Yin Fire. One dose of Shiquan Dabu Tang plus Paojiang revived her; several doses stabilized her. At Shi Wanhu's residence in Wujiang, hearing noise, a new mother collapsed with trismus, declared dead. I suspected labor-induced Qi/Blood injury causing spasm. Urgently prepared Shiquan Dabu Tang plus Fuzi. Had assistants straighten body and face, pry mouth, and administer hot decoction. She refused to swallow initially. Cooled, tilted head, expelled, re-straightened, reheated, repeated five times until swallowed. Revived immediately.
Yijian Zhi records: Du Ren treated Hao family's daughter-in-law, day 4 postpartum, convulsions, upward staring, back arching. Diagnosed Spasm, gave Da Dou Zi Tang and Duhuo Tang, cured. During Zhenghe era, my wife delivered, still in bed, suddenly developed this, head and feet touching, nearly two chi apart. Family panicked, tried forcing straight unsuccessfully. Remembered this text, had Duhuo ready. Before doctor arrived, gave three doses, straightened. Doctor arrived, she was cured. No need for Da Dou Zi Tang. Ancient prescriptions are often miraculous.
Xiao Xuming Tang: Treats postpartum wind-stroke, Gang Jing, and Rou Jing.
Chen Linchuan says so. Chen Wuyue comments: Postpartum sweating often changes to spasm, yet prescribing Xuming Tang is hard to believe. With heavy sweating, how can one take Mahuang, Guigui, Fangji, Huangqin? Da Dou Zi Tang is better. Ju Fang's Dasheng San is also good. Zhu Fengyi says both Hard and Soft Spasms can use modified Xiao Xuming Tang. For Soft Spasm with sweating, removing Mahuang and adding Gegen has basis. Though Da Dou Zi Tang and Dasheng San are good, do not allow biased views to hinder ancient wisdom.
Da Dou Zi Tang: Treats wind-stroke vertigo, aversion to wind/sweating, vomiting cold water, and postpartum hundred diseases. Or wind-stroke aphasia/spasm, back stiffness/trismus, staring, irritability. Pulse tight/large is untreatable. Duanju Fang treats postpartum wind-stroke, severe illness, back stiffness/trismus, restlessness, heaviness, itching, vomiting, staring. All suitable.
Chuan Duhuo (de-gorged, 1.5 liang), Da Dou (0.5 sheng), Wine (3 sheng).
Soak Duhuo in wine, boil briefly. Fry beans until smoking. Throw into wine, seal, cool, remove beans. Take 1-2 he per dose. Mild sweating indicates cure. Ten doses daily. Removes wind and resolves blood stasis. Useful for pregnancy trauma or retained fetus.
Chen Linchuan says: Postpartum trismus, back stiffness, opisthotonos are all named Spasm. Ancients distinguished Hard/Soft/Yin/Yang by presence of sweat. Chanbao books list Trismus and Opisthotonos separately, but they are one. Chills/fever resemble Shanghan, yet are ignored. How can one treat with only one or two drugs?
Da Dou Tang: Treats postpartum wind-stroke causing collapse and unconsciousness, and pregnancy wind-complications, and various puerperal diseases.
Da Dou (5 sheng, fried yellow), Duhuo (de-gorged), Gegen (each 8 liang), Fangji (peeled, 6 liang).
Coarsely crush. Take 5 qian with 2 sheng wine. Decoct to 1.5 sheng, strain, take warm at any time, three times daily.
Lingyangjiao Yinzi: Treats postpartum Qi excess, hard abdomen, distended hypochondrium, heart irritability, thirst, impending Gang Jing or wind-stroke.
Lingyangjiao (0.5 liang, shaved), Fangfeng, Qianghuo, Jiegeng (de-gorged), Baijiang (each 8 qian), Guixin, Chaihu (de-gorged)...
