[Da Lun] Heart and abdominal pain in women caused by blood and qi results from the deficiency of the zang-fu organs, where wind pathogens invade and strike against the true qi, causing pain. This pain moves up and down with the flow of qi; sometimes it rushes upward to the heart region, and sometimes attacks downward into the abdomen. Hence, it is termed "blood and qi attacking the heart and abdomen."
[Xue Ji's Commentary] If the aforementioned condition is due to qi stagnation and blood stasis, use Moyao San (Myrrh Powder). If it is due to exhaustion damaging the primordial qi, use Buzhong Yiqi Tang (Tonify the Middle and Augment the Qi Decoction) (from Miscellaneous Diseases: Injury from Overwork). If it is due to liver-spleen depression and knotting, use Siqi Tang (Four Seven Decoction) (from Miscellaneous Diseases: Qi Disorders). If it is triggered by anger stirring up liver fire, use Xiaochaihu Tang (Minor Bupleurum Decoction) (for Heat Entering the Blood Chamber). If it is due to liver-spleen blood deficiency, use Siwu Tang (Four Substances Decoction) (general formula). If it is due to spleen-lung qi deficiency, use Sijunzi Tang (Four Gentlemen Decoction) (from Miscellaneous Diseases: Deficiency Consumption). If it is due to middle-jiao qi weakness, use Buzhong Yiqi Tang (Tonify the Middle and Augment the Qi Decoction). If both qi and blood are deficient, use Bazhen Tang (Eight Treasures Decoction) (which is the Bayi Wu Tang for deficiency consumption in miscellaneous diseases).
A certain woman experienced heart and abdominal pain every time she became angry, which persisted for a long time without cure. This was due to liver fire injuring spleen qi. She was prescribed one liang of fried Gardenia (Shanzhi) and five slices of fresh ginger, decocted and taken orally, after which the pain ceased. Subsequently, she took Erchen Tang (Two Aged Substances Decoction) added with Gardenia and Platycodon (Jiegeng), and the condition did not recur. Another woman suffered from intermittent heart and abdominal pain due to emotional depression; various medicines were ineffective. She was cured using Guipi Tang (Restore the Spleen Decoction) (from Miscellaneous Diseases: Forgetfulness), with doubled amounts of fried Gardenia.
The mother of Lu Xiaocun from Chenhu suffered from chronic heart and abdominal pain. Each episode was accompanied by chest fullness and vomiting, cold hands and feet, flushed face, numb lips, dry throat, and dry tongue. She experienced alternating chills and fever, restless nights for over a month, and her pulse was surging (Hong). The physicians treated it as phlegm-fire; although symptoms temporarily subsided, they frequently recurred. In the spring of the Yisi year, the attacks became frequent and severe. Continuing with previous medications aggravated the condition. This was due to cold-natured drugs damaging the true yang, resulting in internal true cold and external false heat. Furthermore, the pulse was surging, wiry, and irregular, indicating spleen qi deficiency with liver wood overacting on spleen earth. Treatment should focus on warming and tonifying stomach qi. Thus, Buzhong Yiqi Tang (Tonify the Middle and Augment the Qi Decoction) was used with additions of Pinellia (Banxia), Poria (Fuling), Evodia (Wuzhuyu), and Aucklandia (Muxiang). After one dose, she slept soundly until dawn; the surging pulse immediately normalized, and the irregular pulse disappeared, with all symptoms resolving.
