There is a condition called "Red Thread Sore" (Hong Si Chuang). Although it is not carbuncle or rash (Dan Zhen), its toxicity is actually the same. It mostly occurs on the joints of the middle fingers of both hands, and is particularly severe in males on the left side and females on the right side. Its presentation is a single blister, clear, transparent, bright, and glossy, about the size of a chick's head. The base has dozens of tiny needle-like holes. It is neither itchy nor painful and causes no significant harm. However, a vein like a red thread runs faintly beneath the skin at the edge of the blister. This thread spreads very rapidly along the arm upwards; if it passes the elbow, the condition becomes critical, and if it reaches the heart, death ensues. For those with this condition, one should urgently pierce and cut off the leading end of the red thread with a needle, or plug the wound with earwax, or apply chewed white plum to seal it, which will stop the spread of the thread. In the Gengwu year of the Shaoxing era, Fang Zi (courtesy name Chuzai), the magistrate of Wuwei County, who was a contemporary of my late father, died three days after his term ended and he returned home as an acting official. When asked about the cause of his death, he said he had no illness, only a small blister on his left middle finger joint. He likely did not know it was a Red Thread Sore. Additionally, Lady Yiren, wife of Inspector Wei, said: A neighbor girl suddenly developed a blister on her middle finger joint. The color was extremely clear, the bottom was full of fine needle holes clearly countable, and there was a strand of red thread beside it. The whole family laughed and ignored it. Suddenly, an old woman came and saw it, exclaiming in alarm, "This is a Red Thread Sore! It will cost you your life!" She urgently applied dozens of moxa cones on the blister and pierced the red thread in several places, thus avoiding disaster.
Chao Yuanfang stated: When children encounter wind-heat toxic qi, it clashes with blood and qi, forming stubborn nodules that arise on the neck and nape. If constrained by wind-cold, they fail to dissipate, forming scrofula (Luo Li). Over time, they ulcerate, suppurate, and become sores. Dr. Tang suggested taking Qingliang Yinzi (Cooling Drink) and Shengma Tang (Cimicifuga Decoction), etc. Lianqiao Wan (Forsythia Pills) and Longdan Tang (Gentian Decoction) from *Qianjin Fang* are also suitable. [Zeng] Regarding scrofula, ancient sages called it "Jiu Lou" (Nine Leaks). Investigating its etiology suggests heat involvement, while examining its morphology shows variety; names are generally derived from appearances. Generally, it first appears between the muscles of the neck and nape. Before suppuration occurs, treating the root can influence the branch, helping to reduce the condition. Treatments include acupuncture, moxibustion, and topical applications, administered according to the specific cause. However, young children are frail and weak, unable to withstand acupuncture and moxibustion; therefore, treatment relies on internal medication and external pastes. This disease often arises behind the ears and on both sides of the neck. Initially, there is one nodule, which subsequently multiplies into a dozen or so, wrapping around the neck, resembling beads on a string. Each swells, suppurates, and ruptures. Mild cases can be cured, but severe ones are difficult to eradicate. If pus leaks from acupoints and remains unhealed for years, it is called "Lou Xiang" (Leaking Neck). Tracing back to the onset, it originates from the Three Yang meridians contracting wind-heat, which clashes with blood and qi. Treatment involves Baijie San (Hundred Resolutions Powder) combined with Danggui San (Angelica Sinensis Powder), decocted with water, ginger, green onion, and Jiedixin (Centella Herb). Subsequently, Xuanshen Yin (Scrophularia Drink), Niubang Tang (Arctium Decoction), Mutong San (Akebia Powder), and Neixiao Wan (Internal Dissolving Pills) are used to vent heat and resolve toxicity. Externally, Hupi San (Oak Bark Powder) is used for washing, and Baiji San (Bletilla Powder) and Erxiang San (Two Fragrances Powder) for application, promoting qi and blood circulation until pus dries and fluid ceases, leading to spontaneous recovery. One must avoid spicy, toxic foods and game meat to prevent recurrence.
[Xue] Scrofula caused by fetal toxin results from stagnant qi and郁 fire (depressed heat) in the Liver and Gallbladder meridians. Since these meridians run along the sides of the body, pathology arising from Liver fire manifests in these areas. One must investigate the cause and medicate accordingly. Sometimes it is due to the nursing mother's anger or blood deficiency with internal heat; in such cases, the mother should be regulated according to the cause. Harsh medicines should be avoided to prevent damaging Yuan Qi (Original Qi). An infant born with five nodules on the neck: investigating the mother revealed she was prone to anger, suffered alternating chills and fever, had breast pain, and slight swelling in the hypochondrium—all signs of Liver-Gallbladder meridian pathology. Xiao Chaihu Tang (Minor Bupleurum Decoction) plus Danggui (Angelica Sinensis) and Shaoyao (Peony) was used first, resolving the chills and fever immediately. Jiawei Xiaoyao San (Modified Free and Easy Wanderer Powder) was then administered to the mother for over two months, curing the child. Another infant suffered from nodules around the ears and front due to the mother's Liver meridian heat; treating the mother with Jiawei Xiaoyao San cured the child spontaneously. Another child had nodules in the neck and below the ears for three years. Long-term use of detoxifying agents worsened the lesions and depleted Yuan Qi. Diagnosis showed the mother’s chronic depression and anger led to Liver-Spleen blood deficiency with heat. Jiawei Guipi Tang (Modified Restore the Spleen Decoction) was the main formula, assisted by Jiawei Xiaoyao San. As the mother's heat subsided, giving the child a few spoonfuls daily resulted in recovery after two months. Another child had four nodules since birth; by age five, nodules around the ears resembled beads. Yuan Qi was severely deficient, with alternating chills/fever and incomplete feeding. This indicated Qi stagnation in the Liver-Gallbladder meridians. Bazhen Tang (Eight Treasures Decoction), Xiaoyao San, and other formulas were given to a healthy adult woman to nurse the child. After half a year, the child's physique improved, nodules shrank, and subsequent use of Dihuang Wan (Rehmannia Pills) and Xiaoyao San led to complete recovery. Another newborn had three scrofula nodules on the neck, spreading to the front of the ear before scheduled topical treatment. I stated this resulted from Liver-Gallbladder meridian constitution. The mother's Liver-Gallbladder pulse was surging and rapid. I advised treating the mother would cure the child. Disbelieving, she took Bixiao San (Highly Effective Powder) once, resulting in vomiting and diarrhea, and died overnight.
Heat-toxin scrofula is a wind-heat syndrome of the Hand-Foot Shaoyang and Foot-Jueyin meridians, or caused by Liver malnutrition and food accumulation. Symptoms appear in the neck, axilla, or around the ears, sometimes like beads. Differentiate exterior-interior and deficiency-excess. Redness and swelling indicate Liver meridian heat-toxin; treat with Renshen Baidu San (Ginseng Detoxification Powder). Pain with stifling heat indicates internal Liver fire; treat with Jiawei Xiao Chaihu Tang. Non-painful yellow urine indicates Liver blood deficiency; treat with Liuwei Dihuang Wan (Six-Ingredient Rehmannia Pill). Nodules hidden deep in flesh with unchanged color indicate internal Liver malnutrition; treat with Jiawei Luohua Wan (Nine-Ingredient Aloe Vera Pill). Suppuration without rupture or rupture without healing indicates Spleen Qi deficiency; treat with Yiqi Yangrong Tang (Tonify Qi and Nourish Blood Decoction). Swelling, pain, chills, fever, and thirst belong to excess pathogenic qi and deficient form qi; treatment should clear Liver fire and generate Liver blood. Hard swelling without suppuration or non-healing ulcers belong to simultaneous deficiency of pathogenic and form qi; treatment should tonify Kidney Water and strengthen Spleen Earth. If caused by the mother's anger transmitting Liver fire, treat according to the cause. An infant with dripping pus and unresolved nodules, fever, and aversion to cold has Liver meridian Qi-blood deficiency with heat; treat mainly with Buyin Bazhen Tang (Nourish Yin Eight Treasures Decoction), interspersed with Qinggan Yirong Tang (Clear Liver and Nourish Prosperity Decoction) for recovery. Later recurrence with dark scanty urine and afternoon fever/thirst was treated with Shenzhu Chailing Tang as main formula, assisted by Liuwei Dihuang Wan material plus Chaihu (Bupleurum) and Zhizi (Gardenia), and Simei Feier Wan (Four-Ingredient Child-Nourishing Pill) for closure. A fifteen-year-old boy with fever, thirst, flushed cheeks at dusk, and rapid weak pulse had Yin deficiency with heat. Fifty doses of Buyin Bazhen Tang, plus twenty doses with Renshen (Ginseng), led to suppuration. Clear thin pus and lack of muscle growth indicated Spleen Qi deficiency. Main treatment was Renshen, Danggui (Angelica), Baizhu (Atractylodes), assisted by Shaoyao (Peony), Shudihuang (Prepared Rehmannia), Maidong (Ophiopogon), and Wuweizi (Schisandra). Qi and blood recovered, followed by one dose of Bixiao San to expel toxin and heal. A boy treated with harsh drugs developed chills, fever, and abdominal distension, indicating Liver-Spleen malnutrition. Wuyi Yigong San (Five Agents Extraordinary Skill Powder) plus Chaihu and Shengma (Cimicifuga), assisted by Jiawei Luohua Wan, gradually reduced symptoms. Simemei Feier Wan and Wuyi Yigong San eliminated them. A nine-year-old boy with constant greenish complexion, hard non-suppurating nodules, and unchanged skin color underwent Liver-attacking phlegm-resolving therapy. I advised regulating and tonifying Liver-Spleen. Disbelieving, deficiency syndromes arose. Upon re-evaluation, he still wanted Liver-attacking therapy. I explained: Green face indicates Liver deficiency revealing true color; white face indicates Lung deficiency; rushing phlegm indicates Spleen deficiency failing to control; eye twitching indicates Liver blood deficiency generating wind. The Classics state: The Stomach is the root of the Five Viscera; save Stomach Qi first. Wuyi Yigong San plus Shengma and Chaihu restored some Yuan Qi. Morning: Buzhong Yiqi Tang (Middle-Warmer Tonifying Qi Decoction); Evening: Wuyi Yigong San, assisted by Jiawei Luohua Wan. Complexion turned yellow, nodules shrank. Intermittent Dihuang Wan and Simemei Feier Wan completed the cure.

[Xue] Convulsive nodules belong to wind-wood and ministerial fire activity in Liver-Gallbladder meridians. Wood excess generates wind, heat transforms similarly. Symptoms include spasms and agitation due to wind-heat and blood dryness. Wind moves causing Liver fire excess; fire excess consumes Liver blood; blood fails to nourish tendons causing spasms and bead-like nodules. Neck sides correspond to Liver-Gallbladder meridians. Treatment should nourish Kidney Water, clear Liver fire, nourish Yin blood, and strengthen Spleen Earth. Strong Kidney Water clears Liver fire; cleared fire generates Yin blood; generated blood calms ministerial fire; calm fire prevents damage to Yuan Qi and invasion of Spleen Earth. An infant with neck nodules and opisthotonos sought detoxification. I diagnosed Liver blood dryness from prior convulsions and Cinnabar use. Liuwei Dihuang Wan nourished Liver blood, curing him. Another infant with neck nodules, profuse phlegm, and convulsions took metallic aromatic-dry drugs, causing limb spasms. This re-injured Liver blood, constituting acute convulsion. Jiawei Xiao Chaihu Tang plus Gouteng (Uncaria), Zhizi, Chuanxiong, Danggui, and Liuwei Wan material plus Wuweizi, Maidong cured him. Another infant had neck nodules, fever, poor appetite, and mild limb spasms during sleep upon fright. This is Liver wood insulting Spleen earth. Wuyi Yigong San plus Chaihu, Shengma, Gouteng cured him. Post-marriage, nodules in legs/arms/wrists appeared after mistaken use of Qi-moving/Blood-breaking drugs, causing spasms and emaciation. In Beijing, I used Dihuang Wan to generate Liver-Kidney blood, assisted by Buzhong Yiqi Tang to tonify Spleen-Lung Qi, curing him. Another infant had ear/nodules, worsening with fright, grinding teeth, shaking head, and slow convulsion from aromatic-dry drugs, leading to death.
