Heat in the Lung interstitium serves as the Sovereign Drug (Jun Yao). Ju Hong (Tangerine Peel), Xuan Shen (Scrophulariae Radix) are bitter and cold; Sheng Gan Cao (Raw Licorice Root) is sweet and cold; they clear fire and tonify Qi, serving as Ministers (Chen Yao). Lian Qiao (Forsythiae Fructus), Shu Niu Zi (Arctii Fructus), Bo He Ye (Menthae Haplocalycis Folium) are bitter, acrid, and neutral; Ban Lan Gen (Isatidis Radix) is sweet and cold; Ma Pi Bo (Euphorbiae Humifusae Herba), Bai Jiang Can (Bombyx Batryticatus) are bitter and neutral; they dissipate swelling, detoxify, and calm asthma, serving as Assistants (Zuo Yao). Xin Sheng Ma (Cimicifugae Rhizoma), Chai Hu (Bupleuri Radix) are bitter and neutral; they unblock the Qi stagnation of the Shaoyang and Yangming meridians. Jie Geng (Platycodonis Radix) is acrid and warm; acting like a boat or oar, it prevents the medicinal action from descending. All are ground into fine powder. Mix with decoction and take frequently; or mix with honey to form pills for sucking/melts, cure achieved after finishing the course. This prescription was applied clinically, saving many lives, named Pu Ji Xiao Du Yin Zi (Universal Benefit Detoxification Decoction). Li Dongyuan (Haizang) stated: For Da Tou Bing (Big Head Plague), although the lesion is in the upper half of the body, heat lies latent in the meridians. It is caused by contracting non-seasonal epidemic qi from heaven and earth. When it ulcerates and discharges pus, and infects others, it is called Yi Li (Epidemic Pestilence). Generally, excessive pathogenic heat in the Foot-Yangming meridian relies on the Minister Fire of Shaoyang to become blazing. The disease location is mostly in Shaoyang or Yangming, and in severe cases, it transmits reversely. Observe which part the swelling occupies, and treat according to the affected meridian. Damp-heat causes swelling; excess Wood causes pain. This pathogen originates in the head, mostly concentrating around the ears. The area where symptoms first appear is the main manifestation and root. Treatment should be early; medication should not be too rapid or harsh, lest the drug force exceeds the pathology. As the saying goes, if upper heat is not cleared, middle cold arises, harming life. This disease enters from outside to inside, involving the Blood level. Moreover, the head is high and receives pathogen in formless areas; treatment should be slow first, then fast. "Slow first" means the pathogen is above, attaching to formless parts. Since it attaches to formless areas, its transmission is unpredictable. Using heavy purgatives will fail to remove the pathogen and instead harm the body beyond necessity. Even using mild drugs, if taken hastily, before meals, or in one dose, the "slowness" principle is lost, and the drug cannot cure. One must slowly permeate and clear formless pathogens. Medicinal properties, flavors, forms, and administration methods based on image theory must adhere to slowness and cold-natured drugs, such as wine-frying or wine-steeping. "Fast later" means the previous mild agents have drained pathogenic qi from high positions into the middle, reaching Yin parts and tangible tissues. If not removed quickly, Yin will be damaged. This becomes a Guest Pathogen (Ke Xie) and must be removed urgently; this is treating guests with urgency. Treating the Host (Zhu) requires slowness: Yang pathogens above, Yin pathogens below, each is a primary disease of its own meridian. Urgent treatment fails to resolve complexity and causes chaos. Thus, treating hosts requires slowness. Treating guests requires urgency: Yang receiving Yang pathogens, Yin receiving Yin pathogens are hosts; Yin receiving Yang pathogens, Yang receiving Yin pathogens are guests. Guests must be removed urgently; this is treating guests with urgency. For example, in Shaoyang and Yangming diseases: Shaoyang pathogen manifests around ears; Yangming pathogen manifests as massive head/face swelling. First use Huang Qin (Scutellariae Radix), Huang Lian (Coptidis Rhizoma), Gan Cao (Glycyrrhizae Radix et Rhizoma), stir-fried together, cut, and decocted. Take small amounts frequently. After finishing, use Da Huang (Rhei Radix et Rhizoma), wine-soaked or baked, plus Shu Niu Zi fried fragrant on tile, cut, decocted, strained, mixed with equal parts Mang Xiao (Natrii Sulfas). Drink slowly after meals. Obtain slight diarrhea until pathogen clears, then only take the first medicine. If ineffective, take the second medicine again, following the sequence. Stop when stool is free and pathogen cleared. If Yangming thirst, add Shi Gao (Gypsum Fibrosum). If Shaoyang thirst, add Gua Lou Gen (Trichosanthis Radix) decoction. For Yangming channel involvement, add Sheng Ma, Ge Gen, Bai Shao; for Taiyang channel involvement, add Qiang Huo, Fang Feng, Jing Jie. Select and add to the main formula; do not use powders alone. Hei Bai San (Black and White Powder): Gan Jie Tang (Licorice and Platycodon Decoction) plus Shu Niu Zi, Lian Qiao, Da Huang, Xuan Ming Fen (Mirabilite), Bai Jiang Can, Jing Jie. 1 liang Bai Jiang Can, 2 liang Jin Wen Da Huang, pill size of bullet with Ginger Juice. Take with fresh spring water and raw honey. Externally, apply well-bottom mud mixed with Da Huang and Mang Xiao powder.
[Mei Leng Gu Tong] The brow bone is where the Eye System passes, reaching up to the brain, as eyes belong to the brain. If various Yang meridians harbor external pathogens,郁结 into Wind-Heat-Toxin, attacking the head, sinking into the eyes, passing through the Eye System over the brow bone, causing pain via binding. If Heart/Liver heat obstructs and attacks the eyes, pain also involves the Eye System and brow bone. Phlegm-Wind attacking from chest/diaphragm causes similar pain. If Taiyin dampness prevails, dampness stirs internally, cold presses lower jiao, pain stays in neck, radiating to brows. Pain may be sour, pulling, heavy, or dizzying; diagnose which Qi prevails. Li Dongyuan's Xuan Qi Tang treats unbearable brow bone pain effectively. Zhu Danxi said: Belongs to Wind-Heat and Phlegm, treated like Head Wind. For Wind-Heat, use Qu Feng Qing Shang San. For Phlegm, Er Chen Tang plus Jiu Huang Qin, Bai Zhi. For Wind-Cold, Qiang Wu San. Dai Yun said: Eye socket pain has two patterns, both Liver-related. Liver deficiency pain: severe pain upon seeing light, use Sheng Shu Di Huang Wan. Liver channel fluid retention: brow bone pain preventing eye opening, quiet by day, worse by night, use Dao Tan Tang or Xiao Xiong Xin Tang plus Ban Xia, Ju Hong, Nan Xing, Fu Ling.
[Tou Feng Mei] Luo Qianfu said: Liver channel Wind excess causes Wood to shake. *Shangshu* says: Fullness invites loss. Laozi says: Strength leads to old age. Wood insults Spleen Earth; Metal controls it; this is the child avenging the mother. Snow skin (dandruff) during combing indicates Lung syndrome, as Lung governs skin/hair. Dry stool: treat with Xie Qing Wan. Deficiency: Ren Shen Xiao Feng San. Wan Bing Zi Wan treats excessive dandruff. Take 3-7 pills with Ginger Decoction. Based on above treatments, these are used only after Wind-Heat attacks head/eyes causing dizziness/pain. If only dandruff exists, use Bai Zhi, Ling Ling Xiang externally only.
[Tou Zhong] Cause? Heaven's dampness invading externally, person's damp-phlegm steaming upward, Yin Qi reversing upward from below—all cause heavy head. Why? Head resembles Heaven, Qi is clear/light; Earth is Yin, soil is damp. External invasion/internal steaming blocks clear orifices, causing Qi/Blood stagnation in meridians, resulting in heaviness. *Neijing*: "Yang Qi, due to dampness, feels wrapped." This is external dampness obscuring. Also: "Spleen Heat disease starts with heavy head," Stomach meridian guiding heat up. Taiyin resurgence: fluid retention internally, dampness reverses to Taiyang, staying above causing heavy pain, chest convulsions severe. Taiyang excess: internal dampness郁, heavy head. Juyue Jue Ni: swollen head/heavy, sudden fall. *Lingshu*: Du Meridian branch Changqiang ascends spine to neck, spreads to head, descends to shoulder blades, connects to Taiyang. Deficiency: heavy head shaking. Lesion beside spine: treat at branch point. Li Dongyuan: Heavy head like mountain is dampness in head. Hong Dou San insufflated into nose. Another method: Qiang Huo Gen, roasted Lian Qiao (3 qian each), Hong Dou (0.5 qian), powder, insufflate. Oral damp-draining drugs pass through disease site, punishing the innocent; thus nasal route is used, like shooting high targets. Zhu Danxi: Robust people with phlegm/headache/dizziness: Da Huang wine-soaked 3 times, powder, tea wash.
[Tou Yao] Wind and Fire both move; meeting at vertex causes shaking. *Neijing*: "Xun Meng Zhao You, closed eyes/deafness, lower excess/upper deficiency, fault in Foot-Shaoyang/Jueyin." Note: Xun=fast, Meng=blurry vision, Zhao=shaking, You=severe. Blurry vision, severe head shaking. Taiyin resurgence: head/neck pain/heaviness, severe convulsions. Note: Dampness reverses to Taiyang, stays in chest causing convulsions. *Lingshu*: Du branch Changqiang deficiency causes heavy/shaking head. Pathogenesis states: All wind/shaking/dizziness belongs to Liver Wood. Vertex is Foot-Taiyang passage, Du/Jueyin convergence. Fire from these three meridians gathering here follows Wind-Wood to cause shaking. Zhang Zhongjing: Heart exhaustion causes staring and head shaking.
