生地黄黄连汤
川芎 生地黄 当归（各七钱） 赤芍药 栀子 黄芩 黄连（各三钱） 防风（一两）
上为粗末，每服三钱，水一盏，煎至七分，去渣，取清饮，无时，徐徐与之。若脉
实者，加大黄下之。
大抵此证，非大实即大虚，当审其因，察其脉，参其证，而分若黑白矣。实而便秘，大
承气泻之；虚而便滑，独参汤补之；厥逆者，加附子。娄全善云∶尝治循衣摸床者数人，皆
用大补气血之剂。唯一人兼 振脉代，遂于补剂中略加桂二分，振亦止，脉和而愈。
黄连解毒汤（见发热。）
治喜笑欲死者。针列缺二穴，在手大指后臂上三寸，及大陵二穴，在掌后横纹中，针三分。
治喜死，四肢冷，气绝色不变者。刺阳池穴，用口温针，勿令针入三分，徐徐出针，
以手扪其穴，即复苏也。
半夏汤 治胆腑实热，精神恍惚，寒湿泄泻，或寝汗憎风，善太息。
半夏（一钱五分） 黄芩 远志（各一钱） 生地黄（二钱） 秫米（一合） 酸枣仁（三钱，炒） 宿姜（一
钱五分）
上长流水煎服。
五饮汤（见痰饮。）
温胆汤（《三因》） 治心胆虚怯，触事易惊，或梦寐不祥，遂致心惊胆慑，气郁生涎，涎
与气搏，变生诸证，或短气悸乏，或复自汗。
半夏（汤洗） 枳实 竹茹（各一两） 橘皮（一两半，去白） 甘草（炙。四钱） 白茯苓（七钱）
每服四钱，水一盏半，生姜七片，枣一枚，煎七分，食前热服。
十四友丸 补诸虚不足，益血，收敛心气。治怔忡不宁，精神昏愦，睡卧不安。

平补镇心丹（《和济》） 治心血不足，时或怔忡，夜多异梦，如堕崖谷。常服安心肾，益荣卫。

远志丸 治因事有所大惊，梦寐不祥，登高涉险，神魂不安，心志恐怯。

妙香散（见狂。）
琥珀养心丹 治心血虚，惊悸，夜卧不宁，或怔忡心跳者。
琥珀（另研，二钱） 龙齿（ ，另研，一两） 远志（黑豆、甘草同煮，去骨） 石菖蒲 茯神 人参

上为细末，将牛黄、朱砂、琥珀、龙齿研极细，以猪心血丸，如黍米大，金箔为衣。
每服五十丸，灯芯汤送下。
定志丸 治心气不足，惊悸恐怯。

上为末，炼蜜为丸，如桐子大，朱砂为衣。每服五十丸，米汤下。一方，去茯神，名
开心散，服二钱匕，不时。
宁志丸 治心虚血少，多惊。
人参（去芦） 茯神（去木） 白茯苓（去皮） 柏子仁 远志（酒浸，去心，焙） 酸枣仁（酒浸，去壳，
微炒） 当归 琥珀（以上各半两） 石菖蒲 朱砂（另研） 乳香（各二钱半）

人参远志丸 治心气不安 惊悸恍惚。

真珠母丸（《本事》） 治肝经因虚，内受风邪，卧则宽散而不收，若惊悸状。
珠母（研细，七钱五分） 当归 熟地黄（各一两半） 人参 酸枣仁 柏子仁 犀角 茯苓
（各一两） 沉香 龙齿（各半钱）
上为细末，炼蜜丸，如桐子大，辰砂为衣。每服四五十丸，金银薄荷汤下，日午后卧时服。
独活汤
独活 羌活 防风 人参 前胡 细辛 半夏曲 五味子 沙参 白茯苓 酸枣仁
（炒） 甘草（各一两）
上为粗末，每服四大钱，水一盏半，姜三片，乌梅半个，同煎至七分，去渣，不拘时服。
绍兴癸丑，予待次四明，有董生者，患神气不宁，每卧则魂飞扬，觉身在床，而神
魂离体，惊悸多魇，通夕无寐，更医不效。予为诊视，询之曰∶医作何病治？董曰∶众皆
以为心病。予曰∶以脉言之，肝经受邪，非心病也。肝气因虚，邪气袭之，肝藏魂者也，
游魂为变。平人肝不受邪，卧则魂归于肝，神静而得寐。今肝有邪，魂不得归，是以卧则
魂飞扬若离体也。肝主怒，故小怒则剧。董欣然曰∶前此未之闻，虽未服药，已觉沉
去体矣，原求治之。予曰∶公且持此说，与众医议所治之方，而徐质之。阅旬日复至云∶
医遍议古今方，无与病相对者，故予处此二方以赠，服一月而病悉除。此方用真珠母为
君，龙齿佐之，真珠母入肝经为第一，龙齿与肝同类也。龙齿、虎睛，今人例以为镇心
药，殊不知龙齿安魂，虎睛定魄，各言其类也。盖东方苍龙木也，属肝而藏魂；西方白
虎金也，属肺而藏魄。龙能变化，故魂游而不定，虎能专静，故魄止而有守。予谓治魄
不宁者，宜以虎睛，治魂飞扬者，宜以龙齿。万物有成理而不失，在夫人达之而已。
羌活胜湿汤（见腰痛。） 寿星丸（见痫。） 控涎丹（见行痹。） 黄连安神丸（见虚烦。）
寒水石散（《三因》） 治因惊心气不行，郁而生涎，涎结为饮，遂成大疾，松悸陨获不
自胜持。少遇惊则发，尤宜服之。但中寒者不可服。

上为末，每服二钱，热则新汲水下，寒则姜、枣汤下。加龙胆少许尤佳。
加味四七汤（《得效》） 治心气郁滞，豁痰散惊。

上为细末，入牛黄、金银箔，更研令匀，每服一钱，薄荷酒调下，不拘时候。
铁粉散 治风惊。
铁粉（研） 光明砂（水飞） 铅霜（研） 天竺黄（研。各一两）
上细研如粉，每服半钱，不拘时，竹沥调下。
铁精丸 治惊风恍惚，寝寐不安。
铁精（另研） 龙齿（研） 犀角屑 麦门冬（去心） 人参（去芦） 茯神（去木） 防风（去芦，各一两）
石菖蒲 远志（各七钱半，去心） 生干地黄（一两半）
上为细末，炼蜜和捣三二百下，丸如梧桐子大。每服二十丸，不拘时，粥饮送下。
菖蒲丸 治同前。
石菖蒲 远志（去心） 铁粉（研） 朱砂（各一两，水飞） 金箔（五十片） 羚羊角屑（七钱半） 防
风（去芦，七钱） 白茯苓（去皮） 人参（去芦。各一两半）
上为细末，入研令匀，炼蜜和丸，如梧子大。每服二十丸，粥汤下，不拘时。
茯神丸 治心脏风虚，惊悸心忡，常多健忘。
茯神（去木芦） 人参（去芦） 麦门冬（去心） 熟干地黄 黄芩 薏苡仁 柏子仁 犀角屑（各
一两） 龙齿（研） 云母粉（各一两半） 防风（去芦） 黄 （各七钱半）
上为细末，入研令匀，炼蜜和捣二三百下，丸如梧子大。每服二十丸，温粥饮下，无时。
人参丸 治心脏风虚，惊悸心忡，或因忧虑之后，时有恍惚，心神不安。
人参（去芦） 熟干地黄 龙齿（各一两，研） 茯神（去木，一两半） 白术（去芦） 甘草（炙） 麦门
冬（去心。各半两） 防风（去芦，七钱半） 金箔 银箔（各五十片）
上为细末，入研令匀，炼蜜和捣二三百下，丸如梧子大。每服十五丸，不拘时，粥饮送下。
生地黄黄连汤
川芎、生地黄、当归（各七钱）、赤芍药、栀子、黄芩、黄连（各三钱）、防风（一两）。
以上药物捣成粗末，每次服用三钱，加水一盏，煎煮至七分，滤去药渣，取清液，随时慢慢饮用。如果脉象实大，加大黄以泻下。
大抵这种病证，不是大实证就是大虚证，应当审察病因，诊察脉象，参考症状，就能分辨清楚了。如果是实证且便秘，用大承气汤泻下；如果是虚证且大便滑泄，用独参汤补益；出现四肢厥逆的，加附子。娄全善说：我曾经治疗过几个循衣摸床的病人，都用大补气血的方剂。只有一个人兼有脉象结代，于是在补益剂中略加桂枝二分，震颤也就停止了，脉象调和而痊愈。
黄连解毒汤（见发热章节）
治疗喜欢笑甚至想死的病人。针刺列缺穴两个，位于手大拇指后手臂上三寸处，以及大陵穴两个，位于掌后横纹中，进针三分。
治疗喜欢死、四肢冰冷、气息断绝但面色不变的病人。刺阳池穴，用口温针，不要让针进入超过三分，慢慢出针，用手按压穴位，病人就会苏醒。
半夏汤 治疗胆腑实热，精神恍惚，寒湿泄泻，或者睡觉出汗怕风，喜欢叹气。
半夏（一钱五分）、黄芩、远志（各一钱）、生地黄（二钱）、秫米（一合）、酸枣仁（三钱，炒）、宿姜（一钱五分）。
以上用长流水煎服。
五饮汤（见痰饮章节）
温胆汤（出自《三因极一病证方论》） 治疗心胆虚怯，遇事容易惊恐，或者梦中梦见不祥之事，导致心惊胆战，气郁生涎，涎与气搏结，变生各种病症，或者气短心悸乏力，或者再次自汗。
半夏（汤洗）、枳实、竹茹（各一两）、橘皮（一两半，去白）、甘草（炙，四钱）、白茯苓（七钱）。
每次服用四钱，加水一盏半，生姜七片，大枣一枚，煎至七分，饭前热服。
十四友丸 补益各种虚弱不足，益血，收敛心气。治疗怔忡不安，精神昏沉，睡卧不安。
柏子仁（另研）、远志（汤浸，去心，酒洒蒸）、酸枣仁（炒香）、紫石英（明亮的）、干熟地黄、当归（洗）、白茯苓（去皮）、茯神（去木）、人参（去芦）、黄芪（蜜炙）、阿胶（蛤粉炒）、肉桂（去粗皮，各一两）、龙齿（二两）、辰砂（别研，二钱半）。
以上制成粉末，炼蜜为丸，如梧桐子大小。每次服用三四十丸，饭后用枣汤送下。
平补镇心丹（出自《和济局方》） 治疗心血不足，时常怔忡，夜里多怪异之梦，如同堕入崖谷。常服可安心肾，益荣卫。
酸枣仁（去皮，炒，二钱半）、车前子（去土）、白茯苓（去皮）、麦门冬（去心）、五味子（去枝梗）、茯神（去木）、肉桂（去皮，不见火，各一两二钱半）、龙齿、熟地黄（酒浸，蒸）、天门冬（去心）、远志（去心，甘草水煮）、山药（姜汁制，各一两半）、人参（去芦）、朱砂（细研为衣，各半两）。
以上制成粉末，炼蜜为丸，如桐子大小。每次服用三十丸，空腹用米汤或温酒任服。
又平补镇心丹 治疗的证候同前。
熟干地黄、生干地黄、干山药、天门冬、麦门冬（去心）、柏子仁、茯神（各四两，一方用七两）、辰砂（别研，为衣）、苦梗（炒，各三两）、远志（去心，甘草煮三四沸，七两）、石菖蒲（节密的，十六两）、当归（去芦，六两）、龙骨（一两）。
以上制成细末，炼蜜为丸，如梧桐子大小。每次服用三十丸，空腹用米饮吞下，温酒也可以，逐渐加至五十丸，宜常服。
远志丸 治疗因事受到大惊，梦中不祥，登高涉险，神魂不安，心志恐惧怯弱。
远志（去心，姜汁淹）、石菖蒲（各五钱）、茯神（去皮木）、茯苓、人参、龙齿（各一两）。
以上制成粉末，炼蜜为丸，如桐子大小，辰砂为衣。每次服用七十丸，饭后临睡前用熟水送下。
妙香散（见狂症章节）
琥珀养心丹 治疗心血虚，惊悸，夜间睡眠不宁，或者怔忡心跳的。
琥珀（另研，二钱）、龙齿（煅，另研，一两）、远志（黑豆、甘草同煮，去骨）、石菖蒲、茯神、人参、酸枣仁（炒，各五钱）、当归、生地黄（各七钱）、黄连（三钱）、柏子仁（五钱）、朱砂（另研，三钱）、牛黄（另研，一钱）。
以上制成细末，将牛黄、朱砂、琥珀、龙齿研极细，用猪心血为丸，如黍米大小，金箔为衣。每次服用五十丸，灯芯汤送下。
定志丸 治疗心气不足，惊悸恐惧怯弱。
菖蒲（炒）、远志（去心，各二两）、茯神、人参（各三两）。
以上制成粉末，炼蜜为丸，如桐子大小，朱砂为衣。每次服用五十丸，米汤送下。一方去掉茯神，名为开心散，每次服用二钱匕，随时服用。
宁志丸 治疗心虚血少，易受惊吓。
人参（去芦）、茯神（去木）、白茯苓（去皮）、柏子仁、远志（酒浸，去心，焙）、酸枣仁（酒浸，去壳，微炒）、当归、琥珀（以上各半两）、石菖蒲、朱砂（另研）、乳香（各二钱半）。
以上制成细末，炼蜜为丸，如梧桐子大小。每次服用三十丸，饭后用枣汤送下。
人参远志丸 治疗心气不安，惊悸恍惚。
人参（去芦）、远志（去心）、酸枣仁（炒）、黄芪（以上各半两）、桔梗（去芦）、官桂（去皮）、丹砂（各二钱半）、天门冬（去心）、白茯苓（去皮）、菖蒲（各七钱半）。
以上制成细末，炼蜜为丸，如桐子大小，每次服用三十丸，食远用米汤送下。
真珠母丸（出自《本事方》） 治疗肝经因虚，内受风邪，睡觉时身体宽散而不收敛，像惊悸的样子。
珠母（研细，七钱五分）、当归、熟地黄（各一两半）、人参、酸枣仁、柏子仁、犀角、茯苓（各一两）、沉香、龙齿（各半钱）。
以上制成细末，炼蜜为丸，如桐子大小，辰砂为衣。每次服用四五十丸，金银薄荷汤送下，日午后卧时服。
独活汤
独活、羌活、防风、人参、前胡、细辛、半夏曲、五味子、沙参、白茯苓、酸枣仁（炒）、甘草（各一两）。
以上捣成粗末，每次服用四大钱，加水一盏半，生姜三片，乌梅半个，同煎至七分，滤去药渣，不拘时候服用。
绍兴癸丑年，我在四明等待任职期间，有个董生，患神气不宁，每次睡觉则魂魄飞扬，觉得身在床上，而神魂离体，惊悸多梦魇，整夜无法入睡，更换医生无效。我为他诊视，询问他说：医生当作什么病治疗？董生说：众人都认为是心病。我说：从脉象来说，是肝经受邪，不是心病。肝气因虚，邪气侵袭，肝藏魂，游魂为变。普通人肝不受邪，睡觉时魂归于肝，神静而能入睡。现在肝有邪，魂不得归，所以睡觉时魂飞扬若离体。肝主怒，所以稍微发怒病情就加重。董生高兴地说：以前没听过这个说法，虽然还没服药，已经觉得沉重感离体了，请求治疗。我说：您先持此说法，与众医商议所治之方，再慢慢验证。过了十天他又来，说：医生遍议古今方，没有与病相对应的，所以我开这两个方赠予他，服用一个月病全部消除。这个方用真珠母为君药，龙齿为佐药，真珠母入肝经为第一，龙齿与肝同类。龙齿、虎睛，今人通常认为它们是镇心药，殊不知龙齿安魂，虎睛定魄，各言其类。东方苍龙属木，属肝而藏魂；西方白虎属金，属肺而藏魄。龙能变化，所以魂游而不定；虎能专静，所以魄止而有守。我认为治疗魄不宁者，宜用虎睛；治疗魂飞扬者，宜用龙齿。万物有成理而不失，在于人能通达它罢了。

十味温胆汤 治疗的证候见前温胆汤下。兼治四肢浮肿，饮食无味，心虚烦闷，坐卧不安。

水二盅，生姜五片，红枣二枚，煎一盅，食前服。加槟榔、赤茯苓，治疗停水怔悸。
附方
茯神散 治疗风惊，心神不定，常多恐怖。
茯神（去木）、生干地黄、人参（去芦）、石菖蒲、沙参（去心，各一两）、天门冬（去心，一两半）、甘草（炙）、远志（去心）、犀角屑（各半两）。
以上切碎，每次服用五钱，水一中盏，入赤小豆二十粒，同煎至七分，滤去药渣温服，不拘时候。
人参散 治疗风惊，闷乱恍惚。
人参（去芦）、甘草（炙）、龙齿（各二两）、犀角屑、生干地黄、白茯苓（去皮，各一两）、麦门冬（去心，一两半）。
以上切碎，每次服用五钱，水一中盏，煎至七分，滤去药渣温服，不拘时候。
金箔散 治疗风惊，手足颤掉，神昏错乱。
金箔、银箔（各五十片）、铁粉（二两，另研）、人参（去芦）、琥珀（另研）、酸枣仁、犀角屑（各一两）、龙齿（另研）、茯神（去木）、麦门冬（去心，各一两半）、防风（去芦）、葳蕤、玄参（去芦）、露蜂房（各七钱半）、牛黄（半两，另研）。
以上制成细末，加入牛黄、金银箔，再研令均匀，每次服用一钱，薄荷酒调下，不拘时候。
铁粉散 治疗风惊。
铁粉（研）、光明砂（水飞）、铅霜（研）、天竺黄（研，各一两）。
以上细研如粉，每次服用半钱，不拘时候，竹沥调下。
铁精丸 治疗惊风恍惚，寝寐不安。
铁精（另研）、龙齿（研）、犀角屑、麦门冬（去心）、人参（去芦）、茯神（去木）、防风（去芦，各一两）、石菖蒲、远志（各七钱半，去心）、生干地黄（一两半）。
以上制成细末，炼蜜和捣三二百下，丸如梧桐子大小。每次服用二十丸，不拘时候，粥饮送下。

以上制成细末，加入研匀，炼蜜和捣二三百下，丸如梧子大小。每次服用二十丸，温粥饮下，无时。
人参丸 治疗心脏风虚，惊悸心忡，或因忧虑之后，时有恍惚，心神不安。
人参（去芦）、熟干地黄、龙齿（各一两，研）、茯神（去木，一两半）、白术（去芦）、甘草（炙）、麦门冬（去心，各半两）、防风（去芦，七钱半）、金箔、银箔（各五十片）。
以上制成细末，加入研匀，炼蜜和捣二三百下，丸如梧子大小。每次服用十五丸，不拘时候，粥饮送下。
Shengdi Huang Lian Tang (Rehmanniae Radix and Coptidis Decoction)
Chuanxiong Rhizoma (Chuanxiong), Shengdi Huang Radix Rehmanniae Recens (Rehmanniae Radix Recens), Danggui Radix Angelicae Sinensis (Danggui) (each 7 qian), Chishao Radix Paeoniae Rubra (Red Peony Root), Zhizi Fructus Gardeniae (Gardenia Fruit), Huangqin Radix Scutellariae (Scutellaria Root), Huanglian Rhizoma Coptidis (Coptis Rhizome) (each 3 qian), Fangfeng Radix Saposhnikoviae (Saposhnikovia Root) (1 liang).
The above herbs are crushed into coarse powder. Each dose is 3 qian, boiled in one *zhan* of water until reduced to 0.7 *zhan*. Strain the dregs and drink the clear liquid slowly at any time. If the pulse is excess (*shi*), add Dahuang Rhizoma Rhei (Rhubarb Rhizome) to purge.
Generally, this syndrome is either a severe excess or a severe deficiency. One must examine the etiology, observe the pulse, and correlate with symptoms to distinguish them clearly. In cases of excess with constipation, Da Chengqi Tang (Major Order the Qi Decoction) is used to purge; in cases of deficiency with loose stools, Dushen Tang (Single Ginseng Decoction) is used to tonify; for cold limbs (*jue ni*), Fuzi Radix Aconiti Lateralis Preparata (Prepared Lateral Root of Monkshood) is added. Lou Quanshan stated: "I have treated several patients with 'grasping at clothes and picking at empty air' (*xun yi mo chuang*) using major qi- and blood-tonifying formulas. Only one patient presented with intermittent pulse (*zhen mai dai*). I added a small amount of Guizhi Twig (Guizhi) to the tonifying formula, which stopped the tremors, harmonized the pulse, and cured the disease."
Huanglian Jiedu Tang (Coptis Decoction for Resolving Toxicity) (See Fever.)
Treats patients who laugh excessively to the point of wanting to die. Needle Lieque (LU 7) twice, located three cun proximal to the wrist crease on the radial side of the arm, and Daling (PC 7) twice, located in the center of the transverse crease of the wrist. Needle depth: 0.3 cun.
Treats patients who desire death, with cold limbs, absent breath, but unchanged complexion. Stimulate Yangchi (TE 4). Use mouth-warming needle technique. Do not insert the needle deeper than 0.3 cun. Withdraw the needle slowly and press the acupoint with the hand; the patient will regain consciousness.
Banxia Tang (Pinellia Decoction)
Treats excess-heat in the Gallbladder Fu organ, mental confusion, diarrhea due to cold-dampness, night sweats with aversion to wind, and frequent sighing.

Tonifies various deficiencies, benefits Blood, and consolidates Heart Qi. Treats palpitations (*zheng chong*) with restlessness, mental dullness, and restless sleep.

In the year Gui Chou of Shaoxing, while waiting for appointment in Siming, I encountered a patient named Dong suffering from unsettled Spirit Qi. Whenever he slept, his Hun (Ethereal Soul) would fly upward; he felt his body was in bed, but his Hun had left it. He suffered from palpitations, frequent nightmares, and insomnia all night. Other doctors failed. Upon diagnosis, I asked: "How do other doctors treat this?" Dong replied: "They all consider it a Heart disease." I said: "Based on the pulse, this is a pathogen in the Liver Channel, not a Heart disease. The Liver Qi is deficient, allowing pathogens to invade. The Liver stores the Hun. Wandering Hun causes transformation. In healthy people, the Liver is free of pathogens; when sleeping, the Hun returns to the Liver, the Shen becomes quiet, and sleep ensues. Now, because the Liver has pathogens, the Hun cannot return, so when sleeping, the Hun flies as if leaving the body. The Liver governs anger, so slight anger exacerbates the condition." Dong happily said: "I have never heard this before. Although I haven't taken medicine yet, I already feel the heaviness leaving my body. Please treat me." I said: "Hold onto this theory, discuss the treatment plan with other doctors, and verify it later." Ten days later, he returned saying: "Doctors discussed ancient and modern formulas, none matched the disease. So I prescribed these two formulas for you. After one month, the illness was completely cured." This formula uses Zhenzhumu (Pearl Shell) as the Sovereign herb, supported by Longchi (Dragon's Teeth). Pearl Shell enters the Liver Channel primarily; Dragon's Teeth shares the nature of the Liver. People usually regard Dragon's Teeth and Tiger Eyes as Heart-calming agents, unaware that Dragon's Teeth calms the Hun, while Tiger Eyes stabilizes the Po (Corporeal Soul), each corresponding to its category. The Eastern Azure Dragon belongs to Wood, corresponds to the Liver, and stores the Hun; the Western White Tiger belongs to Metal, corresponds to the Lung, and stores the Po. The Dragon changes and transforms, hence the Hun wanders and is unstable; the Tiger is still and focused, hence the Po stops and has control. I believe that for unstable Po, Tiger Eyes should be used; for flying Hun, Dragon's Teeth should be used. All things have inherent principles that do not fail; it depends on human understanding.
