For elderly and weak individuals, the pulse should be slow and weak; if it is excessively vigorous, it indicates disease. For young and robust individuals, the pulse should be full and solid; if it is excessively weak, it indicates disease. However, there is another perspective. When an elderly person has a vigorous pulse but not one that is restless or rapid, this indicates strong congenital endowment and longevity; such a person is called a "Longevity Pulse" (Shou Mai). If the pulse is restless and rapid, appearing superficial without depth (having exterior but lacking interior root), it represents solitary Yang floating outward, indicating death is near. When a robust adult has a fine yet harmonious and slow pulse, with equal strength in all three positions (Cun, Guan, Chi), this indicates a tranquil congenital constitution and refined character; such a pulse is called a "Yin Pulse". If the pulse is fine, small, stiff, and straight, with unequal strength between the anterior (Cun) and posterior (Chi) positions, the time of death can be determined.
(Addendum)
Examining each pulse meticulously is a fixed standard; adapting flexibly to individual circumstances belongs to the wise and versatile physician. In obese individuals, Qi resides at the surface, so the six pulses often carry a Floating-Hong (Superficial-Excessive) quality. In thin individuals, Qi is gathered within, so the six pulses often carry a Chen-Shu (Deep-Rapid) quality. In persons with irritable temperaments, five beats per respiration cycle constitute a normal pulse; in persons with calm temperaments, four beats per respiration cycle should be viewed as Heat syndrome. For tall people, fingers should be placed more sparsely; for short people, fingers should be placed more closely. People from the North tend toward Excess patterns; people from the South tend toward Deficiency patterns. The pulse after drinking wine is usually Rapid; the pulse after eating is usually Hong (Excessive/Expansive). After long travel, the pulse must be Rapid; after prolonged fasting, the pulse must be Weak. Unmarried women's pulses are often Ru (Soggy); infants' pulses are often Shu (Rapid). The Classic states: "When Form and Qi are compatible, life prevails; when the pulse is chaotic and irregular, death ensues." How can one fail to examine this carefully!
Speaking of the Cun, Guan, and Chi positions: Chi is the Root, Guan is the Trunk, and Cun is the Branches and Leaves. If the Chi position lacks Shen (Spirit/Vitality), there is no Root. Speaking of the Fu, Zhong, and Chen depths: Chen (Deep) is the Root, Zhong (Middle) is the Trunk, and Fu (Superficial) is the Branches and Leaves. If the Chen depth yields no response, there is no Root.
When the Yin pulse at the Chi position beats vigorously and distinctly differs from the Yang pulse at the Cun position, it signifies pregnancy. (This means the Yin pulse at Chi is large and forceful, clearly distinct from the Yang pulse at Cun, indicating pregnancy.) When Yin is deficient and Yang pulses are vigorous, it signifies Beng (Metrorrhagia/Metrostaxis). (Yin deficiency implies blood depletion below, causing Yang Fire to flare upward. Blood is forced by fire and cannot remain in its proper place, resulting in Beng-Lou [profuse uterine bleeding].) When the Hand-Shaoyin (Heart) pulse moves significantly, it indicates pregnancy. (Hand-Shaoyin refers to the Heart pulse. "Moving significantly" means the pulse feels like a bean grain, urgent, rapid, and forceful. The Heart governs Blood; abundant blood enables conception. A significant Heart pulse movement is a sign of abundant Blood, thus indicating pregnancy.)
Hua Bo-ren said: "If the three positions (Cun, Guan, Chi) have equal Fu-Zhong-Chen qualities, and menstruation has ceased without other diseases, it is pregnancy. Obtaining a Taiyin pulse indicates a male fetus; obtaining a Taiyang pulse indicates a female fetus. Taiyin pulse is Deep; Taiyang pulse is Superficial. A rapid pulse on the left indicates a male; a rapid pulse on the right indicates a female. Rapid pulses on both sides indicate twins. A larger Chi pulse on the left indicates a male; a larger Chi pulse on the right indicates a female. Larger Chi pulses on both sides indicate twins." "A deep-solid pulse on the left hand indicates a male; a floating-large pulse on the right hand indicates a female. Deep-solid pulses on both hands indicate multiple males; floating-large pulses on both hands indicate multiple females." "Floating pulses on both Chi positions indicate two males; otherwise, it is a mixed pair where the female fetus dies and the male survives." "Deep pulses on both Chi positions indicate two females; otherwise, it is a mixed pair where the male fetus dies and the female survives." "When a woman's Yin and Yang pulses are both excessive, it is called 'Shuang Qu' (Double Body), meaning both Chi positions are large and forceful. If the Shaoyin pulse is slightly tight, it indicates blood stasis and turbidity, insufficient nourishment for the fetus, leading to unilateral fetal demise (one dies, one lives). If the dead fetus is not expelled, it will harm the mother and cause the loss of the living fetus."
"How do we know childbirth is imminent? Qibo said: 'The body has symptoms, but the pulse is free of pathological signs.'" (Symptoms include things like abdominal pain and cramping. No pathological signs mean the absence of disease-specific pulses.) When a woman is about to give birth, her pulse deviates from the regular pattern. If she perceives this change at midnight (Zi hour), she will deliver at noon (Wu hour). (Deviation from the regular pattern means changes such as small to large, choppy to slippery, or superficial to deep. Midnight perception and noon delivery occur due to the opposition of Zi and Wu hours.) If a woman has amenorrhea due to pregnancy but the pulse is Xian (Wiry), she will subsequently experience vaginal bleeding and fail to carry the fetus to term. (Xian pulse corresponds to the Liver. The Liver governs free flow and storage of blood. A Xian pulse indicates Liver excess, implying an inability to store blood.)
If a woman's Chi pulse is micro-slow (Wei-Chi), it is called Ju Jing (Residing Menses), where menstruation occurs once every three months. (Wei-Chi is a pulse of deficiency-cold. Ju Jing implies arrested menses. Occurring once every three months indicates blood insufficiency.)
If a woman's Chi pulse is weak, faint, and choppy, accompanied by cold lower abdomen and aversion to cold, if acquired in youth, it indicates infertility; if acquired in old age, it indicates sterility.
After recent childbirth, if Yin is damaged and bleeding does not stop, and the Chi pulse cannot reach up to the Guan position, death ensues.
