In winter, when the cold is severe, if people are exposed to it, the cold qi lies hidden within the muscles and bones without causing immediate illness. It manifests in spring, which is termed "Wen Bing" (Warm Disease). Its symptoms also include high fever (Zhuang Re), generally resembling Shang Han (Cold Damage). Furthermore, if winter should be cold but turns warm instead, the warm pathogen attacks the body and causes immediate illness, also resulting in high fever; this is also called Wen Bing. If a pregnant woman encounters such diseases, the heat pathogen contends with the fetus, all of which damage the fetus.
When exposed to severe winter cold, injury occurs, and the cold qi hides within the muscles and bones. It manifests at the Summer Solstice (Xia Zhi) with high fever, which is also called "Shu Bing" (Summerheat Disease). Shu Bing is synonymous with Re Bing (Heat Disease). This is because accumulated cold qi becomes toxic upon manifestation. If a pregnant woman contracts this, it often leads to miscarriage (Duo Tai).
The disease of alternating chills and fever in pregnancy still belongs to the category of seasonal pathogens (Shi Qi). This illness arises from deficiency and impairment of blood and qi (Xue Qi), allowing wind pathogen (Feng Xie) to take advantage. This results in the obstruction of Yin and Yang. Excess Yang leads to heat, while excess Yin leads to cold. As Yin and Yang contend and their energies clash, alternating chills and fever occur. When a pregnant woman contracts this disease, severe heat will injure the fetus.
As for Nue (Malaria), it originates from summerheat injury in summer, where the pathogen resides in the skin. In autumn, due to labor and movement disturbing blood and qi, the cou li (pores and interstices) become deficient, allowing wind pathogen to invade. This stirs up the previously latent summerheat, causing righteous qi and pathogenic factors to clash and Yin-Yang to contend. Excess Yang causes heat, excess Yin causes cold. As Yin and Yang alternate between deficiency and excess, chills and fever manifest; when Yin and Yang temporarily separate, both chills and fever cease. If the pathogen moves again and qi arrives, they clash once more, causing recurrence; thus, malaria has periodic intervals of onset and remission.
If the onset time gradually delays, it is because the pathogen resides in Feng Fu (GV16, Wind Pool acupoint) and moves down along the Du Meridian (Spine). Wei Qi (Defensive Qi) gathers at Feng Fu once every day and night. The next day, it descends one vertebral level, so the onset date is delayed progressively. If the onset time advances earlier, it is because as Wei Qi travels through Feng Fu, it descends one level daily. After twenty-one days, it reaches the coccyx; on the twenty-second day, it enters the spine internally and ascends into the Fu Chong Mai (Hidden Rushing Vessel). This qi ascends for nine days and emerges inside Que Pen (ST12, Clavicle Acupoint). Since the qi has ascended, the disease manifests even earlier.
If the disease occurs every other day, it is because the pathogen presses against the Five Zang Organs and connects horizontally to the Mu Yuan (Sea of Origin/Membrane Source). The pathway is distant, the qi is deep, and its movement is slow, preventing daily occurrence; hence, it manifests intermittently.
For pregnant women contracting malaria, the chills and heat qi force and injure the fetus, often leading to fetal disturbance or damage.
