Pian Tou Tong (Hemicrania) arises when Wind pathogenic factors invade the Yang meridians. When a specific meridian is relatively deficient, the pathogenic factors gather on one side, causing pain that radiates to the frontal angle of the head. If this condition persists for a long time without resolution, it is termed Pian Tou Tong (Hemicrania).
Wang Jinggong (Wang Anshi) suffered from Pian Tou Tong (Hemicrania). Emperor Shenzong of Song transmitted a secret prescription from the imperial palace: take fresh Radish Juice (Lai Fu Zhi) in the quantity of one shell of a clam. The patient lies supine, and the juice is instilled into the nasal cavity. If the pain is on the left, instill into the right nostril; if on the right, instill into the left nostril; or both nostrils may be instilled simultaneously. A chronic condition lasting decades could often be cured with a single application.
A woman suffered from Pian Tou Tong (Hemicrania), with nasal congestion on one side, loss of smell, frequent discharge of clear mucus, and occasional bursts of foul odor. Treatments for headache proved ineffective.
A physician instructed her to take Xiong Xi Wan (Platycodon and Rhinoceros Horn Pill). After less than ten doses, she suddenly sneezed out a strip of thick pus, and her illness was subsequently cured.
Xiong Xi Wan (Platycodon and Rhinoceros Horn Pill): This formula excels in dispelling Wind and clearing Heat. It also incorporates Ren Shen (Ginseng Root) and E Jiao (Donkey-hide Gelatin) to stabilize Qi and Blood, making it suitable even for patients with deficiency. It secures the interior while expelling pathogens externally, allowing both actions to proceed without contradiction.
Chuan Xiong (Ligusticum Chuanxiong Rhizome); Zhu Sha (Cinnabar, processed by water flotation, used as an inner coating, 1 liang); Shi Gao (Gypsum Fibrosum); Long Nao (Borneol) (each 4 liang); Ren Shen (Ginseng Root); Fu Ling (Poria); Zhi Gan Cao (Prepared Licorice Root); Xi Xin (Asari Root) (each 2 liang); Sheng Xi Jiao (Rhinoceros Horn, raw); Zhi Zi (Gardenia Fruit) (each 1 liang); E Jiao (Donkey-hide Gelatin, stir-fried, 1.5 liang); Mai Dong (Ophiopogon Tuber) (3 liang, heart removed).
Grind all ingredients into a fine powder. Mix with honey to form pills the size of quince seeds. Take one pill per dose, chew thoroughly after meals, and swallow with tea or wine as preferred.
Jie Zhai stated: For chronic Tou Feng (Head Wind), where slight exposure to Wind-Cold triggers chills and fever, and the head requires wrapping in thick cotton and multiple scarves for relief, this pattern belongs to "Root Heat and Branch Cold." People of the world do not recognize this and uniformly use Acrid-Warm dispersing methods. When the condition is mild, it seems effective, leading to the mistaken belief that it is a Cold pattern. However, the root involves stagnant Heat, and the pores are often loose, allowing Wind to enter easily. External Cold binds the internal Heat, causing obstruction and rebellious Qi flow, resulting in pain. Acrid-Warm herbs can open obstructions and disperse external Cold, but using Warmth to assist Heat deepens the root pathology and exacerbates aversion to cold. One should instead purge Fire and cool Blood, assisted by Acrid-Warm exterior-releasing agents, treating according to the correct principle. Then the disease can be cured, and its root eradicated.
