When the two Yin meridians and one Yang meridian are affected simultaneously, distension is prone to occur, accompanied by chest fullness and frequent belching. This indicates that Qi from the Kidney and Gallbladder rebels together, the Sanjiao (Triple Burner) fails to circulate normally, causing Qi to accumulate in the upper body.
When the pathogenic factors of the three Yang meridians become excessive and invade the Yin aspect, the disease manifests as abdominal distension and headache. This refers to the excess of pathogenic factors in the three Yang meridians; when excessive, it leads to fullness, and when fullness reaches its peak, it overflows and invades the Yin regions.
The head belongs to Yang, while the abdomen belongs to Yin. Since the disease affects the Yin, abdominal distension and fullness result.
If there has been a fall or injury, causing blood stasis to remain inside, it leads to abdominal fullness and distension, with inability to urinate or defecate. This is because the upper part injures the vessels of the Jueyin Liver meridian, and the lower part injures the collaterals of the Shaoyin Kidney meridian.
Abdominal distension located between the stomach, spleen, and the Hui-mo (Blind Membrane) results in reduced food intake. If Qi becomes obstructed within the five Zang organs, it leads to shortness of breath, anorexia, and critical illness.
Therefore, diseases at the exterior level are easier to treat. For abdominal distension and fullness with blocked Qi, add Houpo (Magnolia Bark) to break up stagnant Qi. If there is abdominal stuffiness without true distension, but rather Qi scattering and failing to consolidate, add Shaoyao (Peony Root) to astringe it. Thus, distension caused by Qi knotting should be dispersed with Houpo, while distension caused by Qi scattering should be astringed with Shaoyao.
Dampness returns to the Spleen, becoming obstructed and failing to circulate. The pulse is soggy, the body feels heavy, urination is difficult, and stool is loose and unsmooth. The Classics state: "All dampness-induced swelling and distension belong to the Spleen." Furthermore, when Earth Qi is郁 (stagnant) and erupts, people suffer from heart and abdominal distension, and edema of the dorsum of the foot. Additionally, when Spleen Earth is invaded by dampness and cannot control Water, Water infiltrates the intestines and stomach and overflows into the skin, producing gurgling sounds, palpitations, and wheezing, which constitutes Shuizhang (Water Distension).
Xiaowenzhong Wan (Small Warm Middle Pill) treats Spleen deficiency with Liver excess, leading to failure of transformation and transportation; purgatives should not be used.
Ingredients: Chenpi (Tangerine Peel), Banxia (Pinellia Rhizome), Shenqu (Medicated Leaven), Fuling (Poria) (each 1 liang); Baizhu (Atractylodes Macrocephala Rhizome) (2 liang); Sheng Xiangfu (Cyperus Rotundus Rhizome), Zhensha (Hematite) (vinegar-fried until red, each 1.5 liang); Kushen (Sophora Flavescens Root) (fried), Chuanlian (Coptis Rhizome) (fried), Houpo (Magnolia Bark) (each 0.5 liang); Gancao (Licorice Root) (3 qian).
Grind into powder, make pills using a paste of vinegar and water (one cup of each) sized like paulownia seeds. Take 70-80 pills per dose. Taking them with a decoction of Baizhu (6 qian) and Chenpi (1 qian) and ginger is beneficial. Add Renshen (Ginseng Root) (1 qian) for severe deficiency. For mild cases, taking 6-7 liang of these pills will restore normal urination; for severe cases, taking 1 jin after administration will normalize urination.
Weiling Tang (Stomach-Cinnamon Decoction) harmonizes the Stomach and Spleen, removes dampness, and eliminates distension.
Ingredients: Cangzhu (Atractylodes Lancea Rhizome), Houpo (Magnolia Bark) (fried with ginger juice), Chenpi (Tangerine Peel), Baizhu (Atractylodes Macrocephala Rhizome), Fuling (Poria) (each 1 qian); Zexie (Alisma Rhizome), Zhuling (Polyporus) (each 1 qian); Gancao (Licorice Root) (6 fen); Guigui (Cinnamon Twig) (5 fen).
Decoct with ginger.
Yuyuliang Wan (Halloysitum Pill) (from *San Yin Fang*): Xu Xueshi and Zhu Danxi stated: This is a crucial medicine for treating Zhanghang (Distension and Swelling).
Main ingredients: Shutanshi (Ore of Hematite containing Goethite) (large pieces, 3 liang, placed in a new iron pot, heated in charcoal fire until red-hot like the pot, then quenched in vinegar, cooled, removed, and ground extremely fine); Yuyuliangshi (Halloysitum) (3 liang); Zhen Zhensha (Authentic Hematite) (5 liang, washed clean, fried dry, combined with Halloysitum, boiled in 2 sheng of rice vinegar in a copper vessel until dry. Then placed in an iron pot, heated in charcoal fire until red-hot, poured onto a clean brick, cooled, and ground extremely fine).
These three substances are the main components. Secondly, based on the patient's deficiency or excess, add the following:
Qianghuo (Notopterygium Rhizome), Muxiang (Vladimiria Root), Fuling (Poria), Chuanxiong (Ligusticum Chuanxiong Rhizome) (soaked in wine), Niuxi (Achyranthes Bidentata Root) (soaked in wine), Guixin (Cinnamon Bark), Baikou (Cardamom Seed) (fried), Huixiang (Fennel Fruit) (fried), Pengzhu (Curcuma Longa Rhizome), Fuzi (Aconite Lateralis Radix Praeparata), Qingpi (Aurantii Fructus Immaturus Pericarpium), Jingsanleng (Sparganii Rhizoma), Baijili (Tribulus Terrestris Fruit), Danggui (Angelica Sinensis Radix) (soaked in wine, each 0.5 liang).
Grind into powder, mix evenly with the previous powders. Soak steamed leaven cake in hot water, wring out the water, mix with the medicine, pound thoroughly, and form pills sized like paulownia seeds. Take 30-50 pills on an empty stomach with warm wine or plain water. Salt is strictly forbidden; even a trace can worsen the disease. Upon taking the medicine, the pathogenic factor will immediately exit through urine without damaging the Zang-Fu organs. The disease will resolve with two to three doses daily. Assist with mild Qi and Blood tonifying herbs. This is truly a miraculous prescription.
