Postpartum lochia containing blood clots is called "Erzhen" (Fetal Pillow). The world generally adopts the method of eliminating stasis first and tonifying later, not realizing that although old blood should be resolved, new blood also needs to be nourished. If one exclusively attacks the old blood, the new blood will also be disturbed. There is a medicine in the world called Jikun Dan (Ji Kun Pill), which attacks blood clots and expels retained placenta. Although it shows rapid effects at once, the primordial Qi is inevitably depleted. It should only be used when absolutely necessary, taking only one pill each time, not suitable for excessive consumption, and must be followed by tonifying agents to avoid the harm of weakness. In cases of severe fatigue leading to bloody discharge, physical collapse, continuous sweating, and incessant panting, Jiashen Shenghua Tang (Ginseng-Added Decoction for Generating New Blood) should be administered urgently for several doses.
Jiashen Shenghua Tang: Ren Shen (Ginseng Root) 2 qian, Chuan Xiong (Ligusticum Rhizome) 2 qian, Dang Gui (Angelica Sinensis Root) 4 qian, Jiang Tan (Charred Ginger) 4 fen, Tao Ren (Peach Seed) 14 grains, Zao (Jujube Fruit) 3 pieces. Modifications:
After pain subsides, do not abruptly add Ren Shen (Ginseng Root), [missing character in source], Bai Zhu (Atractylodes Macrocephala Rhizome), as this may trap the stasis, leading to dire consequences.
Within one or two days postpartum, although clot-related pain persists, if the parturient suffers from Qi and Blood collapse, presenting with bloody discharge or syncope, profuse sweating with cold limbs, loss of complexion and spirit, cooling breath, incessant irritability and thirst, or shortness of breath regardless of pain, one should pragmatically add Ren Shen (Ginseng Root) and [missing character in source] to support critical conditions. If there is fatigue and weakness, Qi deficiency with shortness of breath, increase Ren Shen (Ginseng Root) in the base formula; if accompanied by sweating, add Huang Qi (Astragalus Root); if irritable, add Zhu Ru (Bamboo Shavings); if thirsty, add Mai Dong (Ophiopogon Tuber) and Wu Wei Zi (Schisandra Berry); if panting, add Xing Ren (Apricot Seed) and Jie Geng (Platycodon Root).
Constipation postpartum, lasting up to eight or nine days, is due to blood deficiency causing intestinal dryness. It is appropriate to use Shenghua Tang (Decoction for Generating New Blood) added with Ma Ren (Hemp Seed) 3 qian and Cong Rong (Cistanche Stem) 5 qian to moisten the intestines, allowing natural passage over time. For deficiency patterns, add Ren Shen (Ginseng Root) 2 qian; do not use Chengqi Tang (Chengqi Decoction).
Within seven days postpartum, if cold damage causes blood clots to coagulate with severe pain, use Shenghua Tang (Decoction for Generating New Blood) added with Rou Gui (Cinnamon Bark) 5 fen and Yuan Hu (Corydalis Rhizome) 1 qian. If eating cold food causes heart and abdominal pain, add Wu Yu (Evodia Fruit) 7 fen.
Obese women postpartum often have phlegm-dampness; it is suitable to use Shenghua Tang (Decoction for Generating New Blood) added with Hua Fen (Trichosanthes Root), Zhu Li (Bamboo Juice), and Jiang Zhi (Ginger Juice), or add Ju Hong (Citrus Reticulata Peel) 4 fen. Do not lightly use Niu Huang (Calculus Bovis) or cold-natured herbs.
Women who were constitutionally weak before pregnancy, after one or two days postpartum, having taken three or four doses of Shenghua Tang (Decoction for Generating New Blood) with slight reduction in clot pain, can be stabilized by massage; it is suitable to use Shenghua Tang (Decoction for Generating New Blood) added with Ren Shen (Ginseng Root) 3 qian.
Postpartum, even in summer months, warmth must be maintained. If covered improperly and the abdomen becomes cool, wind-pathogen may enter through deficiency. Even if Shenghua Tang (Decoction for Generating New Blood) is taken, clot pain may not cease; it is suitable to add Rou Gui (Cinnamon Bark) 4 fen and Wu Ling Zhi (Trogopterus Dung) 1 qian.
For postpartum fainting due to labor and fatigue, after taking the first decoction of one dose of Shenghua Tang (Decoction for Generating New Blood), immediately prepare another dose adding Ren Shen (Ginseng Root) 1-2 qian; add Ren Shen (Ginseng Root) if sweating is profuse. If physical collapse has not occurred, use Lu Jiao Hui (Deer Horn Ash) 2 qian, taken with the base decoction. Yi Mu Wan (Motherwort Pill) is also acceptable. External treatment: finely crush fresh Chinese chives, place them in a porcelain bottle with a spout, pour in two bowls of boiling vinegar, seal the large mouth, and direct the small spout towards the parturient's nose to inhale the fumes; this gathers the spirit and restores consciousness.
Postpartum fever with sweating, cold limbs, or bloody discharge with shortness of breath, coma, and impending Qi collapse, constitutes a major deficiency syndrome requiring urgent strong tonification to save life. Use Jiashen Shenghua Tang (Ginseng-Added Decoction for Generating New Blood), administer rapidly for several doses, then substitute tea with Shengmai San (Generate Pulse Powder). Do not trust books such as *Chan Shi Bai Wen* (Hundred Questions of the Delivery Room) and *Fu Ren Liang Fang* (Good Prescriptions for Women).
If postpartum anorexia persists for a long time, with vomiting upon smelling medicine, use Dushen (Single Ginseng) 2-3 qian, or a little ginger, and a large pinch of white rice, decocted in water and taken orally to soothe Stomach Qi. If one allows the Stomach Qi to remain weak and unable to receive medication, the condition becomes critical.
