Translation
It is said that there is a folk custom involving "Mie Pian Gu" (Bamboo Splinter Gu). A bamboo splint about four or five cun long is placed on the road. When a person walks over it, this splint jumps onto their legs, causing unbearable pain. Over time, the splint enters the knee, and the foot gradually becomes thin like a crane's knee. Within four or five years, the patient will inevitably die. The treatment involves chewing raw Realgar (Xionghuang), garlic cloves, and Acorus tatarinowii (Changpu), swallowing them with boiling water. Recovery occurs through vomiting and diarrhea. Subsequently, mix five qian of Realgar powder, one jin of garlic cloves, and one jin of Acorus tatarinowii, pound them into a paste, and place them in boiling water. Wash the feet and knees several times with this decoction to achieve self-healing. No dietary restrictions are required.
This type of Gu poison is golden in color and resembles a silkworm. It can enter the human abdomen, consume the stomach and intestines, and its feces can also poison others. Those who raise Gu keep it to harm people. They use gold, silver, or other objects to send the Gu to the roadside; when someone encounters it, the Gu follows that person away. This is called "Jia Jin Can" (Married Golden Silworm). This Gu is immune to fire, water, knives, and spears, making it extremely difficult to eradicate. It fears only Ci (a thorny plant). Individuals infected with this Gu will find that eating Alum (Bai Fan) tastes sweet, and chewing black beans does not taste fishy/gamey; this indicates infection. Additionally, shortly after taking medicine, if the skin and flesh all over the body feel as if hundreds of insects are crawling, causing extreme itching, this is also a symptom of Gu infection. One can drink a decoction made from pomegranate peel roots to induce vomiting of the poison. Alternatively, burn the bark of the thorny plant (Ci) until charred, grind it into powder, and take one or two qian mixed with water. Ci is also known as Mao Ci (Hairy Thorn) or Shu (Mouse-like), with a head and mouth resembling a mouse and body covered in quills like those of a hedgehog.
Another prescription: Changshan (Dichroa Febrifuga Radix) 4 qian, Shandougen (Sophorae Tonkinensis Radix) 5 qian, one dried Centipede (Wugong), Huangbai (Phellodendri Cortex) 5 qian, five dried Spiders (Zhi Zhu), Qianshanjia (Manis Squama) 5 qian, and one whole dried pigeon blood (Baige Xue). Grind these seven ingredients together into a powder. Divide into three doses, soak in boiling wine, and take orally. The poison will naturally dissolve. Patients with severe poisoning will surely recover with this formula. No dietary restrictions are required. Note: I have not personally verified the above five types of Gu. However, administering Suhed Tang (Perilla and Mint Decoction) should theoretically be effective.
Suhed Tang (Perilla and Mint Decoction): Zi Su Ye (Perillae Folium), Nan Bo He (Menthae Haplocalycis Herba), and Qing Hao (Artemisiae Annuae Herba) each 1 liang; Tiao Shen (Codonopsis Radix), Lian Qiao (Forsythiae Fructus) each 8 qian; Huai Hua (Sophorae Flos), Yuan Shen (Scrophulariae Radix) each 7 qian; Chai Hu (Bupleuri Radix) 6 qian; Chuan Xiong (Ligustici Chuanxiong Rhizoma) 2 qian; Sheng Huang Qi (Astragali Radix) 5 qian. Note: For Snake Gu, add Bai Zhi (Angelicae Dahuricae Radix) 1 liang and San Qi (Notoginseng Radix et Rhizoma) 2 qian. Patients with severe poisoning should take it boiled in water. If symptoms gradually decrease after taking the medicine, it indicates the correct diagnosis; prolonged use leads to self-healing. For constipation, adding Shi Gu Cai (Colocasia esculenta leaves) 1 liang is even better.
In the above formula, Tiao Shen clears Lung Fire and reduces swelling; Lian Qiao drains Heart Fire and detoxifies; Zi Su enters the Lung, Heart, and Spleen meridians, kills snakes, and releases the exterior; Bo He enters the Lung meridian and kills Gu; Huai Hua clears Liver Fire and detoxifies; Sheng Huang Qi detoxifies and releases the exterior; Yuan Shen clears Kidney Fire and detoxifies; Chuan Xiong releases the exterior; Qing Hao kills Gu and detoxifies. Generally, the principle of treating Gu infection lies in using herbs that kill snakes, detoxify, promote the expulsion of toxins, and resolve toxicity. Note: In domestic treatments for Gu, inducing vomiting and diarrhea through medication is considered excellent. Poisoned individuals who cannot abstain from salt and meat should at least avoid chicken, duck, fish, shrimp, clams, and mussels.
Huai Hua Tang (Sophora Flower Decoction): Indicated for dry mouth and excessive internal heat. Huai Hua 1 liang, Qing Hao 1 liang, Sheng Di Huang (Rehmanniae Radix), Zi Su, Nan Bo He, Lian Qiao each 7 qian; Sheng Huang Qi, Tian Dong (Asparagi Radix), Yuan Shen, Fen Hua (Trichosanthis Radix) each 5 qian; Huang Bai 3 qian. For headache, add Bai Zhi 3 qian and Chuan Xiong 2 qian. Take boiled in water.
Guilian Tang (Angelica and Coptis Decoction): Dang Gui (Angelicae Sinensis Radix), Sheng Bai Shao (Paeoniae Radix Alba) each 3 qian; Sheng Di Huang, Lian Qiao, Bai He (Lilii Bulbus), Zi Su, Su Bo He each 5 qian; Chuan Xiong, Huai Hua, Huang Lian (Coptidis Rhizoma), Sheng Gan Cao (Glycyrrhizae Radix et Rhizoma) each 2 qian. Take boiled in water. Remove Gan Cao if abdominal distension is present; add Shi Gu Cai 1 liang for constipation.
Shen [Missing Character] Tang: Tiao Shen 7 qian; Sheng Huang Qi, Zi Su, Nan Bo He, Mai Dong (Ophiopogonis Radix, heart removed), Qing Hao each 5 qian; Chuan Xiong 4 qian; Fu Ling (Poria)... [Text cuts off here].
The above four formulas are key medicines for treating Gu. They are particularly effective for Gan Gu (Childhood Malnutrition due to Parasites/Poison). Poisoned individuals may adjust and take them according to circumstances. The medicinal ingredients do not exceed those in these four formulas. For extremely severe poisoning, abstain from salt, meat, and sexual activity. After several months of medication, resuming salt, meat, and sexual activity is permissible, but one must continue taking the prescribed formula for one year, or two to three years, until bowel movements and food excretion return completely to normal, indicating the poison is fully eliminated. Reduce dosages for patients with weak constitutions.
Suhed Shengdi Tang (Perilla, Mint, and Rehmannia Decoction): Zi Su, Nan Bo He, Qing Hao each 1 liang; Sheng Di Huang, Tiao Shen, Lian Qiao each 8 qian; Huai Hua 7 qian; Chai Hu 6 qian; Chuan Xiong 2 qian; Sheng Huang Qi 5 qian. This is an excellent prescription for treating Snake Gu, Gan Gu, Swelling Gu, and Epilepsy Gu. It has the same efficacy as the previous four formulas. For constipation, increase the dosage of Huai Hua, Huang Bai, Huang Qin (Scutellariae Radix), and Shi Gu Cai. For red urine, add Yuan Shen, Zhi Zi (Gardeniae Fructus), and Fu Ling. For excessive Liver Fire, add Sheng Bai Shao. For headache, add Bai Zhi; for Snake Gu, add Bai Zhi 1 liang and San Qi 2 qian. If accompanied by hot cough or hemoptysis, reduce Zi Su and Bo He by half, and add Bai He, Mai Dong, and Sheng Bai Shao each 1 liang. If occasionally suffering from wind-cold with cold cough, remove Tiao Shen, Lian Qiao, Huai Hua, Qing Hao, and Sheng Di Huang; add Gan Jiang (Zingiberis Rhizoma), Dang Gui, Ban Xia (Pinelliae Rhizoma), Chen Pi (Citri Reticulatae Pericarpium), and Bai Zhi. If the patient is weak with dysentery, remove Tiao Shen, Lian Qiao, Huai Hua, Chai Hu, Sheng Huang Qi, Qing Hao, and Sheng Di Huang; add Bai He, Bai Shao, Fu Ling, Mai Dong, Sha Ren (Amomi Fructus), Bai Zhu (Atractylodis Macrocephalae Rhizoma), and Gan Jiang. Ultimately, skillful application depends on the practitioner. However, do not administer tonifying or astringent medicines while the Gu poison remains. If concurrent diseases exist, treat them simultaneously, but Zi Su and Bo He must never be omitted. Furthermore, if there is heat-induced ulceration of the mouth and tongue due to blazing excess fire, remove Zi Su and Bo He, and heavily increase Huai Hua, Huang Bai, Huang Qin, Fu Ling, Bai Shao, Yuan Shen, Ze Xie (Alismatis Rhizoma), Tian Dong, and Shi Gao (Gypsum Fibrosum). Simmer in a mixture of wine and water.
Regarding Gan Gu (Poison-related Childhood Malnutrition), if toxic vapors travel to the hands and then shift to the head, or if the body turns red in patches upon initial medication, or if there is whole-body numbness with large muscle twitching, or if blood vessels on the head and face open and close intermittently, or if there are sounds in the brain, the treatment method is the same.
Regarding Snake Gu, symptoms include pain under the skin of both flanks that suddenly shifts to under the skin of both shoulders, perpetual absence of sweating, and aversion to cold. After taking medicine, one might hear sounds moving into the abdomen, experience moving pain within the abdomen, feel bloating at the heart region with salivation, unmovable pain, and frequent passage of roundworms in stool, or feel as if lice are crawling all over the body, with legs and feet jumping when lying down. The treatment method is the same.
