Post-measles rectal prolapse. The condition can be either excess or deficiency. One type is caused by accumulated heat in the Large Intestine, which attacks downward causing prolapse, with swelling, hardness, and pain of the anus, sometimes accompanied by blood in the stool, or small-diameter stools, red lips, dry teeth, a firm abdomen, and a surging, rapid, and forceful pulse. Ganjie Shengma Decoction (Gardenia and Licorice Ascending Herb Decoction) is appropriate, added with Zhiqiao (Bitter Orange Fruit), Louren (Trichosanthes Seed), Huangqin (Scutellaria Root), Shanzha (Hawthorn Fruit), Baishao (White Peony Root), Huashi (Talcum), Xuanshen (Scrophularia Root), Danpi (Moutan Cortex), Heijingjie (Black Schizonepeta Spike), etc. Another type is due to deficiency of both Qi and Blood, failing to retain the organs internally, leading to prolapse, with a cyanotic complexion, pale lips, cold fingertips, mental fatigue, and a deep, fine, and weak pulse. Buzhong Yiqi Tang (Tonify the Middle and Augment the Qi Decoction) is appropriate, added with Qianghuo (Notopterygium Rhizome), Baishao (White Peony Root), and Wei Jiang (Prepared Ginger) as the main treatment. Alternatively, Sijunzi Tang (Four Gentlemen Decoction) can be used with added Huangqi (Astragalus Root) and Shengma (Cimicifuga Rhizome), or Siwu Tang (Four Substances Decoction) with added Huangqi (Astragalus Root) and Shengma (Cimicifuga Rhizome). If there is intestinal wind due to blood heat, add Huangbai (Phellodendron Bark), Jingjie (Schizonepeta Spike), and Huaihua (Sophora Flower), used according to circumstances. For severe deficiency, Shiquan Dabu Tang (Ten Thousand Benefits Great Tonification Decoction) is used, added with herbs that lift and raise.

Wan Mizhai stated: Regarding the course of measles from eruption to resolution, if the pulse at the Cun and Guan positions on the right hand is surging and forceful, even if other symptoms are present, they will not cause harm. This is the meaning of "Yang syndrome obtaining Yang pulse." Zhang Jingyue said: If the pulse is fine, soft, and weak, it means "Yang syndrome obtaining Yin pulse." When pulse and symptoms do not correspond, one must understand flexibility and quickly rescue the Yuan Shen (Original Spirit). It is appropriate to use warming and tonifying methods to support the exterior, used with discretion. If one stubbornly considers measles as Yang toxin and generally uses cooling drugs, death is inevitable. Feng Chuzhan said: The excess in disease refers to the excess of pathogenic factors, not the true fullness of the body; once the disease recedes, the Zheng Qi (Upright Qi) must be deficient, which is the true deficiency. Current doctors merely adhere to the ancient theory that rashes are mostly excess-heat, using drugs for tangible excess to attack intangible and variable deficiency, not knowing that the excess of Yang toxin actually stems from insufficiency of Yin Blood. Abandoning the real deficiency to attack the intangible toxin will inevitably lead to increased heat and irritability, and exacerbate diarrhea and panting, often resulting in death.

Quanzhen Yiqi Tang
Renshen (Ginseng Root, 1 qian), Baizhu (Atractylodes Macrocephala Rhizome, stir-fried, 3 qian), Fuzi (Prepared Aconite Lateral Root, light salt water processed, 3 qian), Wuweizi (Schisandra Berry, ground, 0.8 fen), Shudi (Prepared Rehmannia Root, 8 qian), Maidong (Ophiopogon Tuber, 3 qian), Niuxi (Achyranthes Root, 2 qian).

Feng created this formula to treat cases after measles spots appear, characterized by high fever and lethargy, panting and coughing with irritability, thirst without desire to eat, diarrhea with expulsion of roundworms, or head and face rash disappearing first, hot forehead and cold feet, etc., which it treats effectively in all cases.

Furthermore, for those who previously suffered from chronic cough due to Yin deficiency, with strong Cun pulse and weak Chi pulse, and measles eruption being latent, where the pulse indicates suitability for Dihuang Tang (Rehmannia Decoction) rather than exterior-releasing methods, Feng established the prescription based on the pulse, using Liuwei Dihuang Tang (Six-Ingredient Rehmannia Pill).

During the Yi Hai year of the Kangxi reign, macules and rashes erupted widely, critically endangering patients. Physicians used either exterior-releasing and supporting methods or clearing and resolving methods, none of which were effective. Some had rashes visible on the body but faintly retracting on the face; others had rashes on both body and face, with red face, high fever, panting, coughing, irritability, anorexia, and diarrhea. It must be known that these are cases of Yang floating outward and Yin exhausting inward, with weak Middle Qi and injured Lung Qi. The excess of fire is due to insufficiency of water. Ignorant people generally call it residual measles toxin, not realizing it is actually severe damage to Qi and Blood. For those with strong Cun and weak Chi pulses, or fine, rapid, and weak pulses, Quanzhen Yiqi Tang minus Renshen (Ginseng Root) was used, curing within one or two doses.

Xue believed that fever with thirst should be treated with Renshen Maidong Tang (Ginseng and Ophiopogon Decoction); fatigue with convulsions should be treated with He Gan Bu Pi Tang (Harbor Liver and Tonify Spleen Decoction). Yu Jiayan used Renshen Baidu San (Ginseng Rescue the Extremities Powder) to treat macule-rash syndromes. Wu Hegao used Renshen (Ginseng Root), Guizhi (Cinnamon Twig), and Fuzi (Prepared Aconite Lateral Root) to treat macules. These are all variations in method. If physicians do not understand flexibility, how can they discuss such matters? Only when the pulse is surging and forceful, and the person is strong with robust Qi, should the standard treatment be used.

Zhang Jingyue said: Nine out of ten cases of panting are deficiency. If it is originally not a Fire syndrome, nor caused by external pathogens, but results from massive sweating or purging, it must be a sign of Qi collapse. It is appropriate to tonify the Yuan Qi (Primary Qi). To mistake shortness of breath for panting is a major error. Panting is caused by pathogenic obstruction of the clear pathways. Cu (rapid/intermittent pulse) is caused by insufficiency of True Qi. Therefore, Cu is a sign of impending termination. Do not mistakenly identify Cu in medical contexts solely as Excess Fire, ignoring the harm caused by Deficient Fire.

Xu Donggao said: In patients with weak Stomach Qi originally, if triggered by diarrhea and dysentery, the eruption may not fully develop and then recede immediately, allowing pathogenic factors to gradually enter the Stomach. With continuous diarrhea, eruption appearing and then disappearing again, plus panting, the condition becomes critical. It is urgent to first tonify the Spleen and Stomach.
