The methods for treating measles must be highly appropriate. If one insists on using a specific method, it is necessary to distinguish based on the seasonal climate. It may still be acceptable during the cold months of spring and winter, but it must absolutely not be applied during the hot days of summer and autumn.
Ancient texts have discussed "measles." Although this approach is close to reason, it too must be distinguished according to the seasonal climate. For instance, during the severe cold of spring and winter, if measles are suppressed by wind-cold pathogens, causing them to remain closed and unable to erupt, using acrid-warm methods to induce sweating, thereby opening the couli (pores), can be considered acceptable. However, during the heat of summer and autumn, it is only appropriate to let the patient sit or lie in a place without wind, covered with a single blanket, avoiding exposure to wind or summer-heat. How could one use acrid-warm methods to assist yang? If such methods are used during hot weather, the fire-heat will become even more severe, the measles rash will likely turn scorched purple, leading to complicated and deteriorating syndromes, from which many patients cannot be saved.
Measles syndromes have always favored coolness; tonifying the middle jiao formulas should not be used from beginning to end. Why discuss tonifying the middle after the resolution of measles? This is to rescue the spleen and stomach from damage caused by excessive cooling.
Measles syndromes belong to fire pathogens; there are many cases of excess heat in the lung and stomach, and few cases of deficiency-cold. Among a thousand people, deficiency-cold occurs occasionally in only one or two individuals. Therefore, the treatment of measles generally favors the initial use of dispersing and cooling herbs; this is what is meant. Then why do ancient texts propose the use of tonifying the middle jiao? This refers to the stage after the measles have resolved, not the initial fever phase when they are erupting or resolving. The reason is that if too many cooling herbs are used initially in measles treatment, the spleen and stomach may be injured and damaged. Consequently, after the measles resolve, patients often experience vomiting, green diarrhea, pale lips, and a cold body. One must assess the severity and apply the method of tonifying the middle jiao. Thus, herbs such as Renshen (Ginseng Radix et Rhizoma), Baizhu (Atractylodis Macrocephalae Rhizoma), Baifuling (Poria), Sharen (Amomi Fructus), Huoxiang (Agastachis Herba), Chenpi (Citri Reticulatae Pericarpium), Yiyiren (Coicis Semen), and Lianrou (Nelumbinis Semen) are not prohibited. However, they should be accompanied by cooling herbs, such as Chuan Huanglian (Coptidis Rhizoma) and Ku Huangqin (Scutellariae Radix), both lightly stir-fried before use. Since the middle jiao is being tonified while adding cooling agents, the rationale is likely to prevent the resurgence of pathogenic fire due to an excess of middle qi, hence the combined use. As for Baizhu (Atractylodis Macrocephalae Rhizoma), it is suitable only when vomiting and diarrhea are severe. If the symptoms are mild, it should not be used lightly. If a patient has a constitutionally weak physique, and during the eruption of measles, overly dispersing methods were used; or after eruption, excessive cooling and detoxifying herbs were applied, leading to extreme weakness, emaciation, fatigue, lack of ruddy complexion, and inability to eat much (vomiting upon overeating), one should immediately use Xiangsha Liujunzi Tang (Six Gentlemen Decoction with Aucklandia and Amomum) with Banxia (Pinelliae Rhizoma) removed, and add Maiya (Hordei Fructus Germinatus) and Shihu (Dendrobii Caulis) to tonify.
