Malaria of the Foot-Taiyang Channel causes lower back pain and a heavy sensation in the head, with coldness arising from the back. It begins with chills followed by fever; when the fever subsides, sweat is discharged, and the condition is difficult to cure. One should puncture Weizhong (BL-40) to bleed. Malaria of the Foot-Shaoyang Channel causes the body to feel flaccid and weak, with mild chills and mild fever. The patient dislikes seeing people and feels fearful upon contact. There is much heat and profuse sweating; one should puncture the Foot-Shaoyang channel. Malaria of the Foot-Yangming Channel causes initial chills, shivering violently, with coldness persisting for a long time before heat appears. When the heat subsides, sweat is discharged. The patient finds relief in sunlight, moonlight, or firelight; one should puncture the point on the dorsum of the foot of the Foot-Yangming channel. Malaria of the Foot-Taiyin Channel causes dissatisfaction, frequent sighing, loss of appetite, alternating chills and fever with sweating. Upon onset, vomiting often occurs, and the condition improves after vomiting; treat it immediately. Malaria of the Foot-Shaoyin Channel causes severe vomiting, frequent chills and fever, with more heat than cold. The patient desires to stay indoors with doors and windows closed; this condition is difficult to cure. Malaria of the Foot-Jueyin Channel causes lower back pain and fullness in the lower abdomen, difficulty urinating resembling strangury but not true strangury, frequent urination, fear, qi deficiency, and abdominal depression; one should puncture the Foot-Jueyin channel.
Lung malaria causes coldness in the heart region, severe cold followed by heat, and fright during the intervals of fever, as if seeing something; puncture the Hand-Taiyin Lung channel and Hand-Yangming Large Intestine channel. Heart malaria causes extreme mental restlessness, desire for cold water, yet predominance of cold with little heat; puncture the Hand-Shaoyin Heart channel. Liver malaria causes a pale green complexion, frequent sighing, and a state resembling death; puncture the Foot-Jueyin Liver channel until blood appears. Spleen malaria causes chills and abdominal pain; when heat arises, there is borborygmus in the intestines, and sweating follows the cessation of borborygmus; puncture the Foot-Taiyin Spleen channel. Kidney malaria causes a shivering sensation, pain in the waist and spine with difficulty turning the body, difficulty in defecation, blurred vision, and cold hands and feet; puncture the Foot-Taiyang Bladder channel and Foot-Shaoyin Kidney channel. Stomach malaria causes hunger without ability to eat; after eating, there is distension and fullness in the abdomen; puncture the transverse vessels of the Foot-Yangming Stomach channel and Foot-Taiyin Spleen channel to bleed.
When malaria fever is just rising, puncture the arterial pulse on the dorsum of the foot, open the aperture, and discharge blood; the heat will subside immediately. When malaria chills are about to begin, puncture the Hand-Yangming Large Intestine channel, Hand-Taiyin Lung channel, Foot-Yangming Stomach channel, and Foot-Taiyin Spleen channel. If the pulse is full and large, urgently puncture the Back-Shu points on the back, using medium needles at one Back-Shu point beside each other, adjusting the amount of bleeding according to the patient's fatness or thinness. If the pulse is small and firm, urgently moxa the lower leg Shaoyin channel and puncture the Jing-well points of the fingers. If the pulse is full and large, urgently puncture the Back-Shu points, using one of each of the Five-Shu points and Back-Shu points on the back, allowing bleeding to occur naturally. If the pulse is slow, large, and deficient, medication is suitable, but acupuncture is not.
Generally, treating malaria should be done approximately the time it takes to eat a meal before the attack; delaying past this loses the correct timing. For various malarias where the pulse is imperceptible, puncture the spaces between the ten fingers to bleed; once blood is discharged, the disease will certainly cease. First, observe any red spots on the body like small beans and puncture them all. The twelve types of malaria have different onset times; observing the symptoms allows determination of which channel is affected. Puncture approximately the time it takes to eat a meal before the onset: one puncture reduces the severity, two punctures bring improvement, three punctures result in cure. If not cured, puncture the two vessels under the tongue to bleed. If still not cured, puncture the full vessels at Weizhong (BL-40) to bleed. Also puncture the points below the neck along the spine; this will certainly cure. The two vessels under the tongue are Lianquan (CV-23). In treating malaria, first ask where the symptoms began first, and puncture that area initially. If headache and heaviness start first, puncture the head, temples, and between the eyebrows to bleed. If neck and back pain start first, puncture that area. If lower back and spine pain start first, puncture Weizhong (BL-40) to bleed. If arm pain starts first, puncture the spaces between the ten fingers of the Hand-Shaoyin Heart channel and Hand-Yangming Large Intestine channel. If lower leg pain starts first, puncture the spaces between the ten toes of the Foot-Yangming Stomach channel to bleed.
Wind malaria presents with sweating and aversion to wind upon onset; puncture the Back-Shu points of the Three Yang channels to bleed. Severe bone pain that cannot be relieved by pressure is called Marrow Disease; puncture Xuanzhong (GB-39) with a Bi needle to bleed, and it will stop immediately. Mild body pain is treated by puncturing Zhiyin (BL-67). Do not bleed the Jing-well points of Yin channels; puncture every other day. For malaria without thirst occurring every other day, puncture the Foot-Taiyang Bladder channel. For malaria with thirst occurring every other day, puncture the Foot-Shaoyang Gallbladder channel. For Warm Malaria without sweating, apply the Fifty-Nine Acupoints Needling method.
