喉症以吹药为外治之要。不可不深究也。盖外科每每不知药性。修合几种通治之药。若遇重症。非但不效。必至

The application of insufflation powders (Chui Yao) is the key external treatment for throat diseases. This requires deep investigation. Surgeons often lack knowledge of drug properties, preparing several general-purpose remedies. When encountering severe cases, these are not only ineffective but may also lead to medical errors. Moreover, insufflation powders for throat diseases must be compounded according to specific symptoms. Each ingredient should be pre-ground into an extremely fine powder so that the medicine corresponds precisely to the condition, thereby achieving rapid efficacy.
In the initial stage, presenting with swelling, pain, trismus (lockjaw), and upward surging of phlegm-saliva, Yushu Dan (Jade Pivot Pill) is appropriate. It can be ground in tea water for gargling, or Ya Zao (Gleditsia sinensis fruit) can be used to open the jaw. Alternatively, using tung oil (Tung Oil) dipped on a goose feather quill to insert into the throat and induce vomiting of phlegm will clear the orifices, allowing resumption of diet. If trismus is severe and cannot be forced open, Ya Zao powder can be insufflated into the nose, causing the mouth to open. Alternatively, Li Lu (Veratrum nigrum rhizome) powder insufflated into the nostrils can also induce expectoration and open the jaw. Rubbing Ban Xia (Pinelliae Rhizoma) on both sides of the cheek (Jiache acupoint) is also effective.
If pus has formed causing swelling, the affected area should be punctured to drain pus, toxins, and bad blood. If one fears needles or scalpels, human fingernails, Bi Qian (a type of spider web), and tiles baked until dry and charred can be ground into powder, mixed with a small amount of Bing Pian (Borneolum Syntheticum), and insufflated into the lesion. This can also penetrate and drain, known as Da Dao San (Knife Replacement Powder).
For swelling, pain, and severe phlegm obstruction, crushing Yushu Dan, Tu Niu Xi Gen (Achyranthes bidentata root), and Wan Nian Qing Gen (Rhododendron molle root) to extract juice, adding two or three drops of vinegar, and holding it in the mouth is optimal. Peng Sha (Borax), Pu Xiao (Mirabilite), Dan Fan (Alumen), Niu Dan (Bovis Gallus), and Xiao Fan (Alums) can all reduce swelling, resolve toxins, and remove necrotic tissue. For redness, swelling, and excessive fire-heat, Huang Lian (Coptidis Rhizoma) and Huang Qin (Scutellariae Radix) are suitable. Qing Yu Dan (Green Fish Gallbladder), Qing Dai (Indigo Naturalis), and Xi Gua Shuang (Watermelon Frost) can all clear fire and resolve toxins. For wind-fire surging upward, Bo He (Menthae Haplocalycis Herba), Jiang Can (Bombyx Batryticatus), Bing Pian (Borneolum Syntheticum), and Qing Dai (Indigo Naturalis) etc., which have cooling and dispersing properties, are appropriate. For severe toxicity, Xi Niu Huang (Indian Bovis Calculus), Xiong Jing (Realgar), and Ren Zhong Bai (Human Urine Sediment) are excellent detoxifying agents.
For ulcers where new flesh does not grow, Zhen Zhu (Pearl), Xi Huang (Moschus), Zhu Sha (Cinnabaris), and Hu Po (Amber) etc., which promote tissue regeneration, are suitable. However, they must only be used after all necrotic tissue has been removed. If necrotic tissue remains and regenerative medicines are applied, it may instead cause toxins to linger internally, preventing healing. This point must be understood. The number of drugs required for treating throat diseases is not large. Understanding their properties and compounding them according to symptoms naturally yields extraordinary efficacy, rescuing critical conditions in an instant.
