Mr. Ni, the pulse is wiry and choppy on the left, and soggy and slippery on the right; the tongue edges are red, with a thin greasy coating in the center. The manifestations include chest tightness, rising of qi, belching with nausea, a feeling of obstruction when swallowing food, copious phlegm and cough, no bowel movement for more than ten days, and menstruation persisting for eighty days without cessation. This is primarily due to deficiency of Ying blood (nutritive blood), rebellious Liver Qi invading the Stomach and overacting on the Spleen, causing damp-phlegm to linger in the Middle Jiao, and loss of the descending function of the Lung and Stomach. With the illness lingering for a month, how can we allow it to spread further? Urgently prescribe to soothe the Liver and unblock the Stomach, regulate Qi and transform phlegm, to observe the dynamics.

Mr. Wang, latent pathogenic factors of Damp-Warmth have sunk into the Shaoyin channel, stirring up the Chong vessel Qi to strike upward, invading the Stomach and impacting the Lung, rendering the descending command powerless. Manifestations include panting, hiccups, body heat that is not obvious, thin greasy tongue coating. The pulse is wiry, small, and rapid at the left Guan position; soggy and fine on the right; deficient, wiry, and rapid at the Duyang position; faintly present or absent at the Taixi position; speech is disjointed (Zhengsheng), consciousness is confused, alternating between clarity and obscurity. Deficiency of righteous Qi and sinking of pathogenic factors, with the Spirit failing to reside in its house, is clearly evident. The transformation of Jue (cold extremities) and Tuo (collapse) is imminent. Prescribe tentatively to grasp and subdue the Chong Qi, harmonize the Stomach and calm the Spirit, as a method of last resort; perhaps there will be efficacy, who knows.

Mr. Yu, in advanced age, Ying fluids are originally deficient, making Liver Qi prone to rebellion, and the Stomach losing harmony and descent. Yesterday after eating, hiccups occurred frequently, stopping only after a long time. The pulse is wiry, small, and slippery; the tongue is bare without coating. Treating the Liver requires softening methods; treating the Stomach requires unblocking methods. Temporarily focus on nourishing Yin and softening the Liver, assisted by harmonizing the Stomach and regulating Qi.
