[Patient Ye, Left] Initially suffered from throat scarlet fever (an acute infectious disease characterized by sore throat and skin rash). After cure, due to re-infection with food stagnation, damp-warmth disease developed. The body is hot with sweat, but the heat does not resolve; there is aversion to cold in the evening, the heat intensity increases at night, and feels lighter at dawn. This has persisted for three *hou* (approximately 21 days). Dry mouth but little desire to drink, scanty dark red urine, prolonged urination time with powder-like sediment. Thin yellow tongue coating, soggy and rapid pulse. History of hemoptysis, constituting a Yin deficiency constitution. Successive administration of heat-clearing and damp-resolving agents resulted in no reduction of heat; instead, skin edema and foot edema appeared, umbilical and abdominal fullness, facial edema with cough. Carefully analyzing the condition: pathogenic factors in the Taiyang channel have not been resolved, dampness in the Bladder organ has not been transformed. Prolonged dampness traps the Taiyin Spleen, leading to loss of transportation power. Dampness is a Yin pathogen, prone to transforming into water. Water-dampness overflowing causes skin and foot edema; middle Yang fails to transport, turbid Yin condenses causing abdominal fullness; water-dampness rebels against the Lung causing cough and facial edema; Yang is repelled to the exterior causing persistent fever. The condition has entered a dangerous stage, how can it be disregarded? Now prescribing Wu Ling San (Five-Ingredient Powder with Poria) modified, to warm and open the Taiyang channel to transform water-dampness. Do not be constrained by the Yin deficiency constitution and fear administering warming agents, which would lead to one error after another. Is this correct? Submitted to experts!
Cassiae Ramulus (8 fen), Poria with Skin (4 qian), Stir-fried Atractylodis Macrocephalae Rhizoma (3 qian), Polyporus (3 qian), Pinellia Ternata (3 qain), Areca Peel (2 qian), Amomum Fruit (8 fen), Apricot Seed (3 qian), Alisma Rhizome (1 qian), Ginger Peel (8 fen), Tangerine Peel (1 qian), Benincasa Seed and Peel (each 3 qian).
Second Visit: After two doses of Wu Ling San, symptoms showed no movement. The Taiyang channel is the channel of Cold-Water, originally Yin but marked by Yang; Taiyang and Shaoyin are interior-exterior pairs, Shaoyin is the organ of Water-Fire, originally Heat but marked by Cold. If Taiyang Yang does not move, Shaoyin Yin is also injured. The "Lesser Fire" cannot generate Earth, the Central Spleen-Stomach lacks healthy movement, water-dampness accumulates daily, overflows, turbid Yin condenses, Yin excess repels Yang, the Lung loses regulation, water passages do not flow, danger arises continuously, most worrying. Pulse: Cun position soggy and rapid, Guan and Chi positions slow and weak. True Yang is buried, Yin-cold fills the body. If panting adds, it will be difficult to treat. Again prescribing Wu Ling San combined with Zhen Wu Tang (True Warrior Decoction) to stir Kidney Yang, warm and transform water-dampness. This is a critical moment, only this move counts. Such boldness, what do the wise think?
Prepared Aconite Root (1 qian), Cassiae Ramulus (8 fen), Tangerine Peel (1 qian), Large Amomum Fruit (8 fen), Poria with Skin (4 qian), Polyporus (2 qian), Areca Peel (2 qian), Capsicum Seeds (14 grains), Stir-fried Atractylodis Macrocephalae Rhizoma (3 qian), Alisma Rhizome (1.5 qian), Water-processed Mulberry Root Bark (1.5 qian), Light Ginger Peel (8 fen).
Third Visit: Since continuous intake of Wu Ling San and Zhen Wu Tang, skin edema, dorsal foot edema, and abdominal fullness have reduced slightly. Urine volume has increased slightly. True Yang shows signs of stirring, water-dampness shows signs of descending. Evening chills and fever, resolving with sweat at dawn. The pivot mechanism shows signs of rotation, all good omens. Dry mouth, thirst preferring hot drinks, profuse phlegm and cough, poor appetite, white coating transforms to pale. Taiyin Spleen has been trapped by dampness for long, lacking healthy movement, Lung loses descending and clearing functions. Pulse: Guan and Chi positions slow and weak, slightly rising. Although the dangerous ridge is crossed, entering a smooth road remains. Still adhering to previous method, pressing forward.
Cassiae Ramulus (6 fen), Atractylodis Macrocephalae Rhizoma (3 qian), Prepared Aconite Root (1 qian), Soft Bupleurum Root (7 fen), Areca Peel (2 qian), Poria (4 qian), Alisma Rhizome (1.5 qian), Large Amomum Fruit (8 fen), Pinellia Ternata (2 qian), Water-processed Mulberry Root Bark (1.5 qian), Honey-fried Licorice Root (5 fen), Fresh Ginger (2 slices), Jujube (4 pieces), Stir-fried Coix Seed and Malt (each 3 qian).
Fourth Visit: Warming Shaoyin, opening Taiyang, moving Middle Yang, expelling water-dampness. After two more doses, swelling receded, abdominal fullness gradually diminished, evening chills/heat lightened. Only profuse phlegm/cough, poor intake, unclear urine, thin greasy slightly yellow coating, slow slippery pulse. Spleen fails to transport, Stomach fails to circulate, damp-phlegm accumulates in Lung, Lung loses descending authority. Again following previous intent, reducing dosage.
Jilin Ginseng Radicula (8 fen), Poria with Skin (4 qian), Stir-fried Atractylodis Macrocephalae Rhizoma (1.5 qian), Apricot Seed (3 qian), Benincasa Seed and Peel (each 3 qian), Tangerine Peel (1 qian), Prepared Aconite Root (8 fen), Stir-fried Malt (each 3 qian), Soft Bupleurum Root (8 fen), Alisma Rhizome (1.5 qian), Honey-fried Licorice Root (5 fen), Large Amomum Fruit (8 fen), Pinellia Ternata (2 qian).
Fifth Visit: Swelling and distension eliminated, chills/heat retreated. Only poor intake, sweet taste in mouth, profuse phlegm/cough, unclear urine, soggy slippery pulse. Remaining dampness lingers in Middle Jiao, Spleen-Stomach transformation fails, fluids fail to distribute becoming phlegm, hence profuse phlegm and cough. Now regulating Spleen-Stomach to transform remaining dampness, controlling diet and cautioning daily routine.
Stir-fried Atractylodis Macrocephalae Rhizoma (5 qian), Chenpi Tangerine Peel (1 qian), Clear Water Soybean Sprout (4 qian), Stir-fried Coix Seed and Malt (each 3 qian), Benincasa Seed and Peel (each 3 qian), Poria with Skin (4 qian), Pinellia Ternata (2 qian), Pogostemon Herb (1.5 qian), Large Amomum Fruit (7 fen), Apricot Seed (3 qian), Fritillaria Bulb (2 qian), Talcum (8 fen), Honey-fried Loquat Leaf (de-haired, wrapped, 2 qian).
[Patient Ha, Right] Damp-warmth persisting for a month. High fever, profuse sweating, aversion to cold, chest oppression, vomiting, inability to eat, irritability and restlessness, lower abdominal distension/pain with refusal to press, painful urination, dysuria, dry mouth/lips, thirst preferring hot drinks, white greasy tongue coating, left pulse wiry-slow-tight, right pulse deep-thin-weak. Illness began after menstruation. Previous prescriptions included Gardenia and Perilla Seed Decoction, Er Chen Decoction, Xie Xin Decoction, Ba Zhen Decoction, Jin Ling Zi San. Analyzing the condition: Pathogen initially in Taiyin and Yangming. Continuous bitter-cold herbs caused pathogen to sink into Shaoyin and Jueyin. Clear Yang blocked, stasis accumulated in Lower Jiao. Bladder excretion/transformation failed. Irritability appears Yang, but is actually Yin restlessness. Yin excess below, repelling Yang above. Further bitter-cold descending herbs would deepen pathogen entrapment. Now prescribing Evodia Soup modified, to warm meridians, expel dampness, regulate Qi, remove stasis, hoping for turnaround.
Light Evodia Fruit (6 fen), Prepared Aconite Slice (8 fen), Red Poria (3 qian), Cardamom with Shell (8 fen), Charred Hawthorn Fruit (3 qian), Ginger-processed Pinellia (2 qian), Amomum Fruit (8 fen), Tangerine Peel (8 fen), Corydalis Rhizome (1 qian), Trogopterus Dung (1.5 qian), Acronychia Pedunculata Stem (wine-soaked, wrapped, 1.5 qian), Alisma Rhizome (1 qian), Fresh Ginger (2 slices).
Second Visit: Two doses of Evodia Soup. Vomiting and irritability reduced, lower abdominal pain relaxed. Instead, loose stools increased to 7-8 times. Cold-stagnation has a downward trend, Middle Yang shows signs of recovery, good omen. Fever persists, dry mouth/lips, thirst preferring hot drinks, coating slightly transformed, pulse still wiry-slow. Do not change treatment due to dry mouth/lips. Despite fever, do not worry. Once Wei-Yang enters Yin, fever will self-resolve. Stick to original method, advance further.
Original formula minus Fresh Ginger, Cardamom with Shell; add Charred Ginger (6 fen), Stir-fried Atractylodis Macrocephalae Rhizoma (1 qian).
Third Visit: Vomiting and loose stools stopped, lower abdominal pain reduced significantly. Only dysuria, painful urination, dry lips/mouth but little desire to drink. Pulse: Cun/Guan soggy-slippery, Chi choppy-slow. Stasis and dampness linger in Lower Jiao, Bladder Qi transformation powerless, Spleen cannot transport fluids for Stomach, flooding upwards. Condition turning, yet must be cautious. Reduce dosage, add herbs to unblock passage and nourish Kidney, allowing stasis/dampness to exit via lower orifice.
Evodia Fruit (4 fen), Pinellia Ternata (2 qian), Prepared Aconite Slice (8 fen), Red Poria (3 qian), Tangerine Peel (1 qian), Stir-fried Atractylodis Macrocephalae Rhizoma (2 qian), Charred Ginger (4 fen), Honey-fried Licorice Root (4 fen), Amomum Fruit (8 fen), Amber Dust (taken with medicine, 6 fen), Tongtian Grass (5 qian), Zishen Tongguan Wan (wrapped, 3 qian).
Fourth Visit: Symptoms reduced 70-80%, urination clear. Only poor intake, fatigue, dry lips/mouth little desire to drink, pale yellow coating, soggy-slow pulse. Spleen and Stomach both injured, transformation abnormal. Now awakening Spleen, harmonizing Stomax, disseminating remaining dampness. Take every other day, imitating Classic: "Great toxicity treats disease, remove 60%; small toxicity removes 80%." Do not overdo, lest damage Zheng-Qi.
Stir-fried Atractylodis Macrocephalae Rhizoma (2 qian), Cloud Poria (3 qian), Honey-fried Licorice Root (5 fen), Tangerine Peel (1 qian), Pinellia Ternata (2 qian), Large Amomum Fruit (8 fen), Charred Malt (5 qian), Pogostemon Herb (5 qian), Jiangtong Cao (8 fen), Tongtian Grass (5 qian), Fresh Ginger (2 slices), Jujube (4 pieces).
[Patient Zheng, Left] Damp-warmth 18 days. Initially fever, then no fever. Malar flush, urinary incontinence, drowsiness, gray thin greasy coating, dry mouth no desire to drink, deep thin spiritless pulse. Pathogen sunk into Shaoyin, Kidney Yang buried. Dragon-Leopard fire flies upward, Dai Yang syndrome (floating Yang on face), very worrying! Urgently prescribing warming meridians, supporting Zheng-Qi, submerging floating Yang. Wonder if rescue is possible.
Dangshen (5 qian), Dragon Bone (3 qian), Roasted Alpinia Oxyphylla Fruit (1.5 qian), Honey-fried Polygala Root (1 qian), Prepared Aconite Block (3 qian), Oyster Shell (3 qian), Honey-fried Licorice Root (5 fen), Stir-fried Atractylodis Macrocephalae Rhizoma (1.5 qian), Antler Frost (5 qian).
Follow-up Visit: Add Honey-fried Astragalus Root, Large Amomum Fruit.
[Patient Zhou, Left] Damp-warmth over a month. Fever, profuse sweat, confused consciousness, delirium and low voice, dry lips/mouth no desire to drink, unable to eat, dry greasy coating, deep thin pulse. Dampness long trapped Taiyin, sunk into Shaoyin. Damp is Yin pathogen, easily injures Yang. Wei-Yang fails external protection -> profuse sweat. Floating Yang escapes body -> fever. Shen fails to reside -> confusion. Totally different from Heat entering Heart Pericardium. Slight panting on movement -> Kidney Qi fails to grasp. No bowel movement for 10+ days -> Yin constipation. Pulse/syndrome match, Zheng-Qi scattered, Yin-Yang separation imminent. Urgently prescribing Ginseng-Aconite to return Yang, Dragon-Oyster to submerge Yang. If Yang returns and Shen settles, hope for safety.
Korean Ginseng (2 qian), Prepared Aconite Block (2 qian), Left Oyster Shell (3 qian), Large Amomum Fruit (8 fen), Pinellia Ternata (2 qian), Honey-fried Polygala Root (1 qian), Flower Dragon Bone (3 qian), Cinnabar-Poria Spirit (3 qian), Stir-fried Ziziphus Seed (3 qian), Sorghum (wrapped, 3 qian), Floating Wheat (4 qian).
Second Visit: Two doses of Ginseng-Aconite returning Yang, Dragon-Oyster submerging Yang. Sweat stopped, Shen clear, Yang returning internally. Good state. Dry mouth, thirst preferring hot drinks, poor intake, mental fatigue, no bowel movement 10+ days, slight abdominal distension, no refusal to press, dry greasy coating, deep thin pulse. Yang fails to circulate, Yin Qi condenses, intestinal waste cannot descend. Like severe cold where water freezes and earth cracks. Dangerous ridge crossed, not yet on smooth road. Again prescribing supporting Zheng-Qi, assisting Yang, warming/unblocking Fu organs.
Korean Ginseng (1.5 qian), Prepared Aconite Block (1.5 qian), Cinnabar-Poria Spirit (3 qian), Honey-fried Polygala Root (1 qian), Stir-fried Ziziphus Seed (3 qian), Pinellia Ternata (3 qian), Chenpi Tangerine Peel (1 qian), Hemp Seed (crushed, 4 qian), Prunus Seed (crushed, 3 qian), Charred Malt (4 qian), Ban Liu Wan (wrapped, 2 qian). External use: Honey suppository.
Third Visit: After two doses, Fu Qi unblocked, other symptoms remain. Original formula minus Ban Liu Wan, Prunus Seed, Hemp Seed; add Rice-roasted Atractylodis Macrocephalae Rhizoma.
[Infant Zhu] Damp-warmth extended over a month. Persistent fever, abdominal pain, diarrhea, abdominal distension, facial/body edema, gray-yellow coating, soggy rapid pulse, fingerprint blue-purple exceeding Qi Guan. Specialist prescribed Yin Qiao San, Scutellaria, Coptis, Alisma, Talcum, Hawthorn, Medicated Leaven, Chicken Gizzard Lining, Poria, Atractylodes, suspecting malnutrition accumulation. But pulse/syndrome analysis: Three Yang pathogens transmitted to Three Yin. In Taiyin -> abdominal distension/pain. In Shaoyin -> diarrhea/body edema. In Jueyin -> abdominal pain/cold limbs. Wei-Yang fails entering Yin -> fever. Water-dampness overflows -> whole body edema. Child's tender Yang, condition like overloaded boat, fear capsizing! Now prescribing Zhen Wu Tang, Li Zhong Tang, Xiao Chai Hu Tang compound, hoping for 1/10 recovery.
Prepared Aconite Slice (8 fen), Stir-fried Dried Ginger (5 fen), Stir-fried Atractylodis Macrocephalae Rhizoma (1.5 qian), Poria with Skin (3 qian), Tangerine Peel (1 qian), Stir-fried Dangshen (1 qian), Soft Bupleurum Root (5 fen), Honey-fried Licorice Root (5 fen), Capsicum Seeds (10 grains), Amomum Fruit (8 fen), Areca Peel (2 qian), Six-God Medicated Leaven (3 qian).
Second Visit: Since compound of Li Zhong, Zhen Wu, Xiao Chai Hu Tang, abdominal distension/limb edema reduced, diarrhea stopped, good sign. Only evening fever, poor milk intake, dry mouth thirst. Fingerprint blue-purple returned inside Qi Guan, pulse still soggy-rapid-weak. Yin excess repelling Yang, true cold false heat. Do not change treatment due to fever. Stick to original method, press forward.
Original formula add: Tender Anemarrhena Rhizome (1 qian).
Third Visit: Swelling reduced 70-80%, fever gradually receding. Only fatigue/emaciation, poor intake, water-dampness transformed, Zheng-Qi weak/exhausted, Spleen-Stomach Yang decline, lack of driving force. Still modifying original formula.
Original formula minus Bupleurum, add Charred Malt (3 qian), Eupatorium Stem (1.5 qian).
Note: This case is hardest to distinguish due to similarities. Identifying Three Yin manifestations, administering warming herbs, thus no worries. Otherwise, further cold-cool herbs would cause pathogen sinking and Yang escaping, leading to death.
