Diagram of the Mingmen Area
Zhongting (Middle Court) and Tianting (Heavenly Court), Sikong (Public Space) and Yintang (Between Eyebrows), and the broad areas at the corners of the forehead: if pathological colors appear here, life or death can be determined.
If cyanotic or blackish, it is suspected to be due to Wind-anger; if the skin is gentle, smooth, and lustrous, there is no major danger. If the complexion is sunken and damaged, especially if the lips are black, it is the most difficult and ominous condition.
Excessive cyanosis requires worry about acute severity. A dull and dark complexion is also distressing. This is the area of Mingmen, requiring physicians to make skillful judgments.
"Xijiang Yue" on Observing Form and Color
Generally, observing a child's form and color: visible blue veins indicate Liver Heat generating Wind; redness in both cheeks indicates attacking Heat; yellowish hue indicates Spleen Deficiency requiring tonification. Blackish qi indicates abdominal pain and Zhong-E (sudden severe illness); white indicates Gan malnutrition with worms. If both eyes are severely red and swollen, this is like an illusion (or refers to a critical/vague state).
To recognize pediatric syndromes, one must deduce from external appearance: floating yellow with emaciated muscles, stagnation masses in the hypochondrium or abdomen, retracted lips, and bluish face indicate pain. Tongue protrusion and dry lips indicate Internal Heat; drowsiness indicates Spleen Exhaustion; raising hands and kicking feet indicates Convulsion caused by Wind. Gan disease presents with exposed blue veins and a distended abdomen.
Discharge from the eye corners indicates Liver Heat; drooling from the mouth indicates Spleen Cold; sparse and erect hair indicates Blood depletion; swollen eyelids indicate Dampness in the Spleen. Blackened and scorched nostrils indicate Lung Heat; dry and rough ear helix indicates Kidney transmission; chest fullness and rapid breathing indicate Pneumonia; heat exhaustion with swelling and depression at the Chuangmen point (supraorbital ridge).
Sudden reduction in spirit, with irregular yellow-white facial skin, is certainly due to internal injury from milk and food, blocked by cold/raw or greasy items. This may lead to borborygmus and diarrhea/dysentery, or unbearable malaria. Sudden abdominal distension followed by gradual emaciation indicates four conditions: Pi accumulation, insect Gan, etc.
Reddish facial skin, with legs as if painted with vermilion, suggests external contraction of Wind-Cold. How is this? Irregular tidal fever comes and goes without time. It may be treated as Convulsion Syndrome, or banal toxins may be expelled. Dry mouth, crying, eyes protruding like beads, drowsiness, and refusal to nurse.
Pediatric disease forms vary; carefully observe with patience. Shrinking into arms, fearing wind and cold; sleeping face-down indicates Heat-Damp. Night crying, heat irritability, and abdominal pain; straight gaze and convulsions require prevention; long sighs and short breaths indicate Heat hidden inside; phlegm-wheezing rushing upward indicates Hyperactive Fire.
To distinguish fatal signs in children: fontanelle sunken into a pit; rattling sound in throat with Qi and Phlegm; eyes closed, spiritless, and rigid. Pink-white lips and gums; hands and feet cold as ice; crow-like voice, tight mouth, eyes rolling frequently; refusal to nurse, enuresis, and confusion/oppression.
