Bi Du (Perineal Abscess), also known as Kua Ma Yong (Horse-Straddling Carbuncle), is a disease of the Chong and Ren vessels of the Extraordinary Vessels, manifesting as a carbuncle in the territory of the Jueyin Meridian (Liver Meridian of Foot-Jueyin). This meridian is characterized by scanty blood, hence it is also called Xue Shan (Blood Hernia). It may arise following a chancre (Gan Chuang) or suddenly present as a painful nodule. All such cases are caused by heat stagnation and blood accumulation. In the initial stage, dispersing and purgative methods should be employed to resolve the swelling; once suppuration has occurred, standard internal tonifying and pus-expelling formulas should be used.
Bi Du is caused by damp-heat in the Jueyin meridian, triggered by fatigue. Use She Gan (Belamcanda Rhizome), three cun in length, decocted with Sheng Jiang (Fresh Ginger), taken before meals. Passing stool two or three times after administration yields immediate effect. Only She Gan that produces purple flowers should be used.
Bi Du occurs between the inner thighs. Formula: Gui Wei (Tail of Angelica Sinensis), Bai Shao (White Peony Root), Jin Yin Hua (Honeysuckle Flower), Tian Hua Fen (Trichosanthes Root), Bai Zhi Shao (Tip of Angelica Dahurica) (each 1 qian); Mu Bie Zi (Mumie Fruit) (10 pieces); Jiang Can (Bombyx Batryticatus) (2 qian); Da Huang (Rhubarb Root) (3 qian); Mang Xiao (Mirabilite) (2 qian); Chuan Shan Jia (Pangolin Scale) (3 slices, stir-fried with soil). The above herbs are coarsely crushed for one dose. Decoct in two bowls of good wine until reduced to one bowl. Then add Mang Xiao and Da Huang and boil twice more. Leave the pot uncovered outdoors overnight. Take warm at dawn (Wu Geng time). Cover thickly to induce sweating. Recovery occurs after one or two bowel movements.
Shen Qi San (Miraculous Powder), treats Bi Du and Yu Kou (Fish Mouth Ulcer). Ingredients: Chuan Shan Jia (Pangolin Scale) (3 slices, stir-fried with soil); Mu Bie Zi (Mumie Fruit) (shelled, 3 pieces); Mu Li (Oyster Shell); Da Huang (Rhubarb Root) (each 3 qian); Huang Lian (Coptis Rhizome); Huang Qin (Scutellaria Root); Huang Bo (Phellodendron Bark); Jin Yin Hua (Honeysuckle Flower); Lian Qiao (Forsythia Fruit) (each 1.5 qian); Huang La (Yellow Beeswax) (3 qian). The above herbs are coarsely crushed for one dose. Decoct in half wine and half water. Take on an empty stomach.
Ban Bai San (Spotted White Powder). Ingredients: Ban Mao (Mylabris Beetle) (wings and feet removed, stir-fried, 1 qian); Bai Zhi (Angelica Dahurica Root) (8 fen). Grind all into a fine powder. Take 6 fen per dose, on an empty stomach, washed down with yellow wine. Immediate effect is observed.
Long Dan Xie Gan Tang (Gentian Liver-Draining Decoction), treats damp-heat in the Liver meridian, or Nang Yong (Scrotal Carbuncle), Bi Du, Xia Gan (Chancre), Xuan Yong (Suspended Carbuncle) causing swelling and pain, astringent and difficult urination, or women's vulvar retention, itching, and pain, or men's penile swelling and discharge of pus. Ingredients: Long Dan Cao (Gentiana Root) (wine-washed and stir-fried); Ze Xie (Alisma Rhizome) (each 1.5 qian); Che Qian Zi (Plantago Seed); Mu Tong (Akebia Stem); Huang Qin (Scutellaria Root); Sheng Di Huang (Rehmannia Root) (mixed with wine); Gui Wei (Tail of Angelica Sinensis) (wine-washed); Shan Zhi (Gardenia Fruit). The above herbs are coarsely crushed for one dose. Decoct in water. Take warm on an empty stomach.
When Bi Du ulcerates, it is called Yu Kou Chuang (Fish Mouth Ulcer). Ingredients: Da Huang (Rhubarb Root) (2 qian); Jiang Can (Bombyx Batryticatus); Chuan Shan Jia (Pangolin Scale) (stir-fried until pearl-like); Wu Ling Zhi (Trogopterus Dung) (stir-fried. each 1 qian); Jin Yin Hua (Honeysuckle Flower) (2 qian). Grind all into a fine powder. Take 3 qian per dose, on an empty stomach, washed down with yellow wine.
Li Xiao San (Instant Resolution Powder), treats Yu Kou and Bi Du. Take one large Toad (Xia Wa), peel off the skin, and pound it thoroughly including the entrails. Add 5 qian of Cong Bai (Scallion White) and pound again. Apply this paste to the swollen area. Finally, cover the opening with the peeled skin.
