Lumbago, although belonging to Kidney deficiency, is also caused by the Three Causes. Externally, it involves pathogenic invasion of the Zang-Fu organs and meridians; internally, it involves sorrow, worry, fear, and anger, as well as sexual exhaustion or falls, all of which can cause lumbago. The five types of lumbago described in medical texts have not been discussed in detail. As time has passed since the Sages, literature has become sparse and difficult to discuss. Although there are missing texts, they should not be discarded. By extracting their outlines based on availability, future students may infer and seek them out, serving as a canonical basis for treatment.
Taiyang lumbago pulls the neck, spine, buttocks, and back, feeling heavy; Yangming lumbago prevents looking back; when looking back, one sees hallucinations and feels prone to sadness; Shaoyang lumbago feels like needle pricks in the skin, continuous and unrelenting, preventing bending and stretching or looking back; Taiyin lumbago involves irritability and heat, with a sensation of a horizontal blockage below the waist, and severe cases involve urinary incontinence; Shaoyin lumbago pain radiates into the interior of the spine; Jueyin lumbago involves stiffness and tension in the waist, resembling a drawn bowstring. This lists the Six Channels as preparations for treating external causes. Generally, Taiyang and Shaoyin channels often suffer from Cold pathogens, Shaoyang and Jueyin channels often suffer from Wind-Heat pathogens, while Taiyin and Yangming channels often suffer from Dryness-Dampness pathogens. Inferring from this, differentiation should follow the pulse. If there are Jie Mai (Resolving Pulse), San Mai (Scattered Pulse), and acupoints such as Tongyinhui, Yin-Yang Wei, Hengluo, Zhiyang, Feiyang, Roulai, and Kaojiao, these do not belong to the flow and distribution of the Six Channels but are separate pathways with specific dominions. To avoid excessive citation, please consult "Stimulating Lumbago Discussion" in Neijing for clarification. Based on this, treatment should be administered via sweating or purging methods according to the cause.
